However, it is crucial to optimize the radiation dose to ensure it is only as much as needed. The purpose of this study is to minimize the patient's radiation exposure during interventions and produce optimal image quality. To accomplish this, dose optimization and image quality were evaluated during PAC and PCI procedures in the Cathlab room at Adam Malik Hospital using Philips Allura Xper FD20. The study was retrospective, using patient dose audits from January 2020 to February 2023, with 250 PACs and 139 PCI mature patients. The median dose value (Q2) was compared to the National diagnostic reference level (NDRL) third quartile (Q3) value to determine optimal patient dose levels. We used a 28 cm PMMA phantom and a 2 cm imaging quality in-house phantom to represent the patient and a solid-state detector to measure surface dose. Medical physicists, radiographers, cardiologists, and nurses worked together in optimization cycles to ensure dose levels were kept to a minimum. Image quality was assessed by calculating the signal difference-to-noise ratio (SDNR), and images were recorded in DICOM format and analyzed using Image J software. Entrance Surface Air Kerma (ESAK) values were recorded to calculate Figure of Merit (FOM) values for each change in physical parameters. The study showed that the distance between the image detector and the object significantly affects the patient's dose. A distance of 14 cm resulted in a higher dose than a distance of 2 cm. The SDNR value varied with the object image distance (OID) factor, and increasing the object's distance to the image detector did not increase the SDNR value. Setting the OID spacing to a minimum is recommended for PAC and PCI procedures to minimize the patient's radiation exposure. The FOM values were affected by both the ESAK and SDNR values. Therefore, optimization is required regarding chest examination, both in terms of image quality and the radiation dose received by the patient. This study discusses the relationship between the deviation index and the patient's radiation dose produced. The deviation index is a parameter that can determine whether the resulting image is underexposed or overexposed. This study's varied parameter is the exposure index target (EI_T). The study observed 100 patients, with 50 patients having an EI_T of 400 and 50 having an EI_T of 250. Based on the research findings, varying the EI_T parameter resulted in an average deviation index value of 1.86 ± 0.16 (for EI_T 400) and 2.18 ± 0.133 (for EI_T 250).Meanwhile, the Entrance Surface Air Kerma (ESAK) value calculation was done manually as the Si-INTAN account is currently undergoing maintenance. The resulting ESAK values from the study were 0.318 mGy ± 0.027 (for EI_T 400) and 0.296 mGy ± 0.020 (for EI_T 250). Based on the research findings, it can be concluded that the deviation index value remains an effective tool for measuring the quality of images produced during the optimization process of patient radiation dose. EKG-triggered techniques are used to improve the resolution of coronary artery images and minimize artifacts caused by cardiac motion. Retrospective cardiac computed tomography remains the first choice for maximum image quality, especially in relative contraindications such as arrhythmias and high heart rates during the cardiac cycle. The prospective technique can significantly reduce the radiation dose compared with the retrospective technique. This study aimed to see the effectiveness of this prospective technique in terms of radiation dose and image quality. The effective dose analysis method is with indoseCT software and calculated with a formula from CTDIvol and DLP. Image quality was assessed by calculating the SNR by comparing prospective and retrospective techniques. The study results regarding image quality show that the retrospective method has an SNR value of 44, and the SNR value of the prospective method is 35. Dose estimation by indoseCT software obtained results. Retrospective techniques yield the following results: The effective dose is 24.09 mSv, and the prospective technique provides an effective dose of 12.54 mSv. Using the prospective technique shows that the dose can be reduced by 47.94% compared with the retrospective technique. The estimated dose was manually calculated based on CTDIvol and DLP, resulting in an effective dose of 33.5 mSv with the retrospective technique and an effective dose of 11.95 mSv with the prospective technique. Using the prospective technique shows that the dose can be reduced by 64.32% compared with the retrospective technique. The rationale for choosing a potential cardiac CT regimen must be careful and precise, as it may not apply to all patients. The research was conducted by first calculating the ESD using AndroidTM-based software from a previous study (Mulia et al., 2020) with retrospective data from 2 (two) hospitals (20 patients each) that utilized the Philips Allura XPer FD20 interventional fluoroscopy system. The results showed that DAP and fluoroscopy time did not exhibit a linear correlation with the skin dose represented by ESD, with R2 values of 0.11 for Hospital A and 0.79 for Hospital B. Consequently, the DAP and fluoroscopy time dose metrics were deemed insufficient in representing the actual skin dose received by the patients. Therefore, various efforts are necessary to reduce radiation doses. This study aims to obtain an optimal radiation dose while maintaining image quality. The optimization efforts were carried out by the radiology team at RS Sentra Medika Cibinong using the CT Scan GE OPTIMA 660. The method involved scanning head and abdomen phantoms using optimization factors (Noise Index, Slice Thickness, Pitch, and Rotation Time) and then analyzing the images for noise uniformity, MTF (Modulation Transfer Function), and NPS (Noise Power Spectrum) while recording radiation dose in terms of CTDIvol and DLP. Based on the research data using GE standard Head protocol and respective optimizations, the radiation dose values obtained were CTDIVol = 68 mGy; DLP = 2042 mGy.cm and CTDIVol = 48 mGy; DLP = 1662 mGy.cm. Furthermore, the image quality produced by the GE standard Head protocol and respective optimizations resulted in MTF values of 0.55 (1/mm) and 0.91 (1/mm), noise uniformity of 0.6 HU and 1.6 HU, and NPS deviation of 8.7%. Regarding the Abdomen protocol, using GE standard and respective optimizations, the radiation dose values obtained were CTDIVol = 32 mGy; DLP = 1025 mGy.cm and CTDIVol = 22 mGy; DLP = 955 mGy.cm. Additionally, the image quality produced by the GE standard Abdomen protocol and respective optimizations resulted in MTF values of 0.58 (1/mm) and 0.6 (1/mm), noise uniformity of 0.6 HU and 1.6 HU, and NPS deviation of 7.6%. Thus, the protocols using optimization factors were proven to reduce radiation dose by approximately 41.6-45.4% while maintaining image quality compared to the GE standard protocol, which focuses more on improving image quality rather than optimizing the radiation dose received by patients. However, it usually comes at the cost of increased radiation exposure for the patient. Therefore, it's essential to evaluate CT scan image quality to ensure that it provides maximum information for diagnostic purposes while keeping the radiation dose received by the patient as low as possible. Several parameters can be used to evaluate the quality of CT scan images, such as accuracy, linearity, uniformity of the CT number, and high contrast resolution (spatial resolution). A phantom scan is carried out routinely once a week to test the quality of CT scan images. In this study, the image quality phantom was scanned using a Philips MX Evo 16 slice CT Scan over three months, which resulted in twelve CT scan image quality phantom image scan data. The CT scan data was processed and analyzed automatically using the IndoQCT application to determine the image quality. The value obtained was then compared with the test pass value stipulated in the Regulation of the Nuclear Energy Regulatory Agency (BAPETEN) Number 2 of 2018 concerning Compliance Tests for Diagnostic and Interventional Radiology X-ray. The average CT number accuracy value obtained was 0.6 HU, with a maximum value of 0.9 HU and a minimum value of 0.2 HU. The average CT number uniformity value obtained was 0.55 HU with a maximum value of 0.8 HU. The average CT number linearity value obtained was 0.9957 HU with a minimum of 0.9956. Meanwhile, the average MTF value obtained was 0.65, with a maximum value of 0.72. This study concludes that the values obtained still meet the CT Scan image quality test set by BAPETEN. This radioactive waste poses potential radiation hazards for workers, members of the public, and the environment. Therefore, it is crucial to manage it correctly by established regulations. Radioactive waste resulting from the utilization of Nuclear Medicine varies in terms of activity value and mass, necessitating different storage times and appropriate management. Considering these factors, this study aims to develop an Android-based application called 'Si Mbah KeCE Kariadi' (Kalkulasi Limbah Kedokteran-Nuklir Cepat dan Efektif di RSUP Dr. Kariadi) for the management of low and medium-level open radioactive waste in Nuclear Medicine. This application aims to estimate when radioactive waste can be released before being discharged to the hospital's sanitation department. The research method employed in this study involves applying the principles of radioactive waste management, specifically the decay of activity. The radioactive waste release calculator application is generated by utilizing the radioactive decay equation derived from measuring the initial radioactive waste radiation exposure rate using a survey meter radiation measuring instrument and referencing the clearance level value to estimate the release time of the radioactive waste. The evaluation of this application indicates that the radiation exposure value of the radioactive waste is equivalent to the background radiation level at its release, suggesting that it can be disposed of in the hospital sanitation department for further management as infectious waste from patients. Using passive dosimeters such as TLD-900 can assist in monitoring and controlling radiation exposure by evaluating accumulated dose readings over a specific period. Due to its utilization in diverse radiation fields, the dose evaluation method should be able to differentiate the energy range employed. Energy dependence can pose a challenge in reading the received dose, particularly in the case of low-energy X-ray usage. Therefore, this study aims to develop an algorithm for individual equivalent dose evaluation Hp(10) in low-energy (<20 KeV) usage. The data will be obtained from the Nuclear Agency of Malaysia's Secondary Standard Dosimetry Laboratory (SSDL), including various ISO narrow series X-ray sources and several radioactive sources such as Cs-137, Co-60, Sr-90, Kr-85, and Pm-147. The evaluation method will involve a trumpet curve and ANSI bias plot, enabling an assessment of the performance of the developed algorithm. Ultimately, the algorithm will be used for long-term dose evaluation intercomparisons in low-dose reviews. The low-energy energy evaluation algorithm for equivalent doses has successfully been conducted. The algorithm performs very well in distinguishing energy types using the parameter r12. In contrast, the low-energy dose evaluation algorithm encounters difficulties due to the low-energy characteristics in energy deposition, which are similar for each element D.
