In 36 (3 male and 33 female) nursing homes residents aged 91 (IQR 85-94) years, their serum 25-hydroxyvitamin D (25(OH)D) levels were 17.3 (IQR 12.9-23.4) ng / ml distributing 8.3% (3/36) of <10 ng / ml, 47.2% (17 / 36) of 10-<20 ng / ml and 44.4% (16/36) of 20-<30 ng / ml, respectively. Serum 25(OH)D levels were significantly associated with parathyroid hormone (PTH) level, Barthel Index, diaper wearing rate, nursing care level and swallowing function, but not with nutritional intake. By daily 1000 IU vitamin D supplementation for 6 months in 7 residents (male 2 / female 5), serum 25(OH)D levels increased from 14.1 (13.0-21.8) ng / ml to 35.9 (34.0-47.1) ng / ml but physical functions did not improve. These data suggest that other factors such as insufficient protein and energy intake, chronic comorbidities and limited physical activity in addition of vitamin D status might be involved in the manifestations of nursing homes residents. The V3 loop in CCR5-tropic HIV-1 Env-gp120 is a key domain responsible for interacting with CCR5. Based on virus mutation and adaptation studies, our structural modeling predicted that the IXI triplet residues within the V3 loop of Env-gp120 interact with hydrophobic cores in CCR5, potentially modulating viral infectivity. In this study, we investigated the functional significance of the IXI triplet in HIV-1 replication. Substitutions at the central \"X\" position of the IXI triplet significantly affected viral replication in both macaque and human cell lines without altering Env expression. Combinatorial mutations with the V3 tip residue G310R showed either enhancing or suppressive effects on replication in macaque cells, highlighting a complex interplay between the IXI triplet and V3 tip. Furthermore, the impact of these mutations varied between different HIV-1 strains, suggesting that some strain-specific sequence/structural context(s) is critical for the observed biological effect. Taken together, these findings indicate that the IXI triplet can be a novel motif influencing HIV-1 replication and underscore the importance of V3 loop variability in coreceptor interaction and viral growth potential. This study was conducted in three stages. Data were obtained from 92 fathers in the pilot study and 396 fathers in the main survey. We analyzed construct validity, reliability, and criterion-related validity using the collected data. The APR was ultimately confirmed through confirmatory factor analysis (CFA) to comprised 4 factors and 27 items:\"Positive responses to children,\" \"Imagining life with a child,\" \"Sense of gender role division,\" and \"Imagining the practice of childcare.\" Cronbach's alpha coefficient of the total scale was .87. The CFA results indicated a consistent fit between the model and data, with values of χ²/df ratio=2.42, Goodness-of-Fit Index (GFI)=0.870, Adjusted Goodness-of-Fit Index (AGFI)=0.845, Comparative Fit Index (CFI)=0.895, Root Mean Square Error of Approximation (RMSEA)=0.060. The composite reliability (CR) value for the entire scale was 0.774, with values ranging from factors 1 to 4 of 0.697, 0.810, 0.794, and 0.941, respectively. This was considered to be sufficiently reliable. Thus, APR is a valid and reliable tool for assessing readiness to become a father from the perspective of \"A Parental,\" it can also be used to verify the effectiveness of preparatory education for becoming a parent. The mean operation time was 80.7±15.9 min, and the mean anesthesia time was 149.2±16.3 min. Significant improvements were observed in visual analog scale scores for back and leg pain after surgery, with respective mean decreases of 2.7±2.0 and 6.3±2.3 points. Oswestry Disability Index scores showed a mean recovery of 55%±10%. Patients could return very quickly to their daily activities within a mean of 1.5 days and to work within a mean of 2.7 weeks postoperatively. No complications or recurrences were reported during follow-up. The results were good or excellent in all patients according to MacNab's criteria.Conclusion: The results in this preliminary case series suggest that TF-FESS under local anesthesia is a feasible and effective treatment for lumbar radiculopathy. Patients with T3-T4 lower rectal cancer were irradiated once daily (2 Gy) with a total dose of 40 Gy, and chemotherapy was administered with tegafur-uracil (300 mg / m2 / day) or S-1 (80 mg / m2 / day) on radiation days.Results: Five-year overall survival was 77.8% and 5-year disease-free survival was 65.1%. Recurrence was observed in 20 patients (35.1%) and local recurrence in 9 patients (15.8%). Multivariate analysis of prognostic factors for overall survival identified pre-CRT lateral lymph node metastasis and circumferential resection margin as independent prognostic factors, and ypStage as an independent prognostic factor for disease-free survival.Conclusions: Evaluation of lateral lymph node before CRT is useful in predicting prognosis in patients with locally advanced lower rectal cancer treated with preoperative chemoradiotherapy with oral DPD-inhibiting fluoropyrimidines, and surgical planning to ensure a 1-mm circumferential resection margin is important for improving prognosis. The mutation was accidentally discovered when the patient was 80 years old.Methods: We performed genome sequencing (GS) on using a whole blood sample from the patient. The nucleotide sequences obtained were compared to the FXI reference GS and mutations were assessed using BioEdit.Results: Notably, exon 14 was not amplified;therefore, all other exons were analyzed. Four heterozygous nucleotide mutations were noted:c.1556G>A, c.1812G>T, c.1839G>A, and c.1852A>T;c.1556G>A and c.1852A>T were associated with amino acid substitutions p.Trp519* and p. Ile618Phe, respectively. Some cases of the amino acid substitution p.Trp519* have been registered in the database;however, to the best of our knowledge, p.Ile618Phe was likely discovered for the first time in this study.Conclusion: Because both gene mutations were heterozygous, the patient had compound heterozygous mutations that decreased FXI activity. GSM symptoms were assessed using the Vulvovaginal Symptoms Questionnaire and Core Lower Urinary Tract Symptom Score. Gynecologists evaluated the vulvar findings and measured vaginal pH. GSM was diagnosed when symptoms and objective findings were present.Results: Among the 62 postmenopausal women, 17 met the symptom and objective criteria for GSM. The prevalence was 35%, 36.4%, and 10% among those in their 50s, 60s, and 70s, respectively. GSM prevalence was significantly higher in postmenopausal women than in premenopausal women and in sexually active women than in inactive women.Conclusion: Among postmenopausal Japanese women, GSM prevalence was lower than that reported internationally;however, it was higher in sexually active women and increased with time since menopause. CAM5.2 is widely available in routine pathology laboratories, yet its diagnostic significance in hepatic lesions is unclear. We investigated whether CAM5.2 immunostaining visualizes hemodynamic-driven phenotypic changes of hepatocytes and whether its spatial pattern is informative.Methods: CAM5.2 immunohistochemical staining was analyzed in surgically resected specimens of FNH, HCA (HNF1α-inactivated and inflammatory subtypes), and HCC. The localization, distribution, and spatial regularity of CAM5.2 expression in hepatocytes and bile duct structures were evaluated and correlated with histological features of vascular remodeling.Results: In FNH, CAM5.2 showed a regular staining pattern confined to nodules and aligned along fibrous septa or abnormal vessels, highlighting ductular structures and adjacent hepatocytes. This pattern was absent in background liver parenchyma. In contrast, hepatocellular tumors showed irregular patterns:HNF1α-inactivated HCA exhibited weak focal positivity, inflammatory HCA diffuse staining, and HCC heterogeneous, patchy, or negative staining.Conclusions: CAM5.2 visualizes hemodynamic-driven, ductular reaction-like phenotypic modulation of hepatocytes. A regular spatial pattern supports circulatory disturbance-related hyperplastic lesions e.g., FNH and aids exclusion of HCC and HCA. J. Med.
