Güngör","doi":"10.4274/TJO.25991","DOIUrl":"https://doi.org/10.4274/TJO.25991","url":null,"abstract":"In this case report we aimed to present a case of Lyme disease presenting as peripheral retinal vasculitis, intermediate uveitis and multifocal white dots in the posterior pole. The patient exhibited vitritis and snowball opacities in both eyes. A diagnosis of Lyme disease was made based on clinical, angiographic and laboratory findings. Fundus fluorescein angiography revealed optic nerve and retinal venous leakage as well as multiple hyperfluorescent foci in both eyes. The patient's symptoms and ocular findings significant improved after treatment with a combination of systemic antibiotics and steroids. MATERIALS AND METHODS: The records of 48 eyes of 24 patients with ocular rosacea were retrospectively reviewed. Patients' ocular signs and symptoms were scored between 1 and 4 points according to disease severity; tear film break-up time (BUT) and Schirmer's test results were recorded before and after the treatment. Preservative-free artificial tears, topical antibiotic eye drops/ointments, short-term topical corticosteroids, topical 0.05% cyclosporine and oral doxycycline treatment were applied as a standard therapy to all patients. Additional treatments were given as needed. Complications were recorded. RESULTS: Twenty-four patients with a mean age of 48.5±35.4 (32-54) years were followed for a mean 15±9.4 (8-36) months. Ocular findings included meibomitis in 100% of cases, anterior blepharitis in 83% (40 eyes), punctate keratopathy in 67% (32 eyes), chalazia in 50% (24 eyes), corneal neovascularization in 50% (24 eyes) and subepithelial infiltrates in 16.6% (8 eyes). Significant improvement of symptoms and clinical findings were achieved in all patients with treatment. The increases in Schirmer's test and BUT were 3.3±1.5 and 4.5±2.8, respectively (p<0.05). Descemetocele and small corneal perforation occurred in 2 eyes; re-epithelialization was achieved in both eyes with tissue adhesive application (1 eye) and additional amniotic membrane transplantation (1 eye). Four eyes of three patients showed significant regression of corneal neovascularization with topical bevacizumab therapy. CONCLUSION: Ocular rosacea may present with a variety of ophthalmic signs. Ge rec ve Yon tem: 2013 Mart-2013 Mayis tarihleri arasinda femtosaniye laser yardimli katarakt ve RLD ameliyati yapilan 29 hastanin 50 gozu calismaya alindi. Tum hastalara Femtosaniye laser cihazi (LenSx®, Alcon Inc.) ile komple prosedur (kapsulotomi, lens fragmantasyonu ve korneal kesiler) uygulandi. Ameliyatlar fakoemulsifikasyon ve goz ici lensi yerlestirilerek tamamlandi. Intraoperatif komplikasyonlar retrospektif olarak hasta dosyalarindan ve ameliyat videolarindan degerlendirildi. Bulgular: Hastalarin yas ortalamasi 63,8±11,7 (SD) yil idi. Vakum kaybi ya da anterior kapsul yirtigi hicbir gozde olusmadi. On bir gozde (%22) kucuk on kapsul etiketleri oldu. Yedi gozde (%14) serbest kapsulotomi, 40 gozde (%60) mikroadezyonlarla birlikte parsiyel tamamlanmis kapsuloreksis ve 3 gozde (%6) inkomplet kapsuloreksis izlendi. Bir gozde (%2) intrakapsuler basinc artisina bagli arka kapsul rupturu gelisti. Korneal kesiler 8 gozde (%16) kullanilmak istenmedi, 4 gozde (%8) tamamlanamadi. Miyozis 20 gozde (%40) meydana geldi. Vizyon kaybi ya da nukleus drop gozlenmedi. Sonuc: Femtosaniye laserlerin katarakt ve RLD cerrahisinde kullanimi guvenlidir. Komplikasyonlara bagli gorme kaybi gelismemistir. (Turk J Ophthalmol 2015; 45: 97-101) Anah tar Ke li me ler: Femtosaniye lazer, katarakt cerrahisi, komplikasyonlar Objectives: To analyze the early experience and intraoperative complications of femtosecond laser-assisted cataract and refractive lens exchange (RLE) surgery. Ma te ri als and Met hods: The initial 50 eyes of 29 patients who underwent cataract or RLE surgery between March 2013 and May 2013 were included in this study. All patients underwent anterior capsulotomy, lens fragmentation, and corneal incisions with the femtosecond laser (LenSx®, Alcon Inc.). The operation was completed by phacoemulsification and implantation of an intraocular lens. Intraoperative complications were evaluated from patient charts and video reviews retrospectively. Re sults: The mean age of the patients included was 63.8±11.7 years. No suction break or anterior capsule tear occurred in any case. Small anterior capsular tags occurred in 11 eyes (22%). The capsulotomy buttons were free-floating in 7 eyes (14%), while capsulorrhexis was partially completed with microadhesions in 40 eyes (60%) and uncompleted in 3 eyes (6%). One eye (2%) had a posterior capsule rupture secondary to increase in the intracapsular pressure. Corneal incisions either were not preferred to be used in 8 eyes (16%) or could not be completed in 4 eyes (8%). Miosis occurred in 20 eyes (40%). No vision lost or dropped nuclei were observed. Conclusion: The use of femtosecond lasers in cataract and RLE surgery is safe. There had been no vision lost secondary to complications.
