{"title":"The prevalence of scapular dyskinesia in patients with back, neck, and shoulder pain and the effect of this combination on pain and muscle shortness.","authors":"Gonca Sağlam, Hilal Telli","doi":"10.14744/agri.2022.87059","DOIUrl":"https://doi.org/10.14744/agri.2022.87059","url":null,"abstract":"<p><strong>Objectives: </strong>The aim of the study was to evaluate the prevalence of scapular dyskinesia in patients with neck, back, and shoul-der pain and examine the variations in clinical parameters cause by this combination.</p><p><strong>Methods: </strong>A total of 121 patients with neck, back, or shoulder pain were included in this prospective cross-sectional study. De-mographic and clinical data of the patients were recorded. It was evaluated the intensity of pain with the visual analog scale (VAS), the presence of muscle shortness with muscle shortness tests, and scapular dyskinesia with the Lateral Scapular Slide Test.</p><p><strong>Results: </strong>The prevalence of scapular dyskinesia was 41.9% in the study population. Patients were divided into groups, with or without scapular dyskinesia for evaluation, and compared. The presence of scapular dyskinesia was significantly higher in pa-tients with back and shoulder pain (p<0.05). When the distribution of scapular dyskinesia pathological types was evaluated, it was found that Type 1 was the most common in the study population. No significant difference was observed in pain intensity at rest and during activity between the groups (p>0.05), but the VAS score at night was significantly higher in patients with scapular dyskinesia (p<0.05). Although it is rarely seen in the vertebrae, it should be considered in the differential diagnosis of spondyloarthropathies, especially for those which occur in young people, along with back and lumbar pain, which increases during the night. Early diagnosis is essential to alle-viate symptoms and prevent the risk of structural spinal deformities, such as scoliosis. {"title":"Physical activity and health anxiety in people with and without spinal pain during the COVID-19 lockdown: A comparison study.","authors":"Gamze Yalçınkaya, Müge Kırmızı, Yeşim Salık Şengül","doi":"10.14744/agri.2021.90692","DOIUrl":"https://doi.org/10.14744/agri.2021.90692","url":null,"abstract":"<p><strong>Objectives: </strong>In the coronavirus disease 2019 (COVID-19) pandemic, physical inactivity and health anxiety which are common risk factors for musculoskeletal pain have become widespread due to strict precautions and isolation. Thus, we aimed to com-pare physical activity, health anxiety, and spinal pain history in people experiencing and not experiencing spinal pain during the COVID-19 lockdown.</p><p><strong>Methods: </strong>This study was designed as a case-control study. Assessments including the Nordic Musculoskeletal Questionnaire (NMQ), the International Physical Activity Questionnaire-Short Form, and the Short Health Anxiety Inventory were performed through an online questionnaire using Google forms during the COVID-19 lockdown. We reached 494 volunteers, and 348 were eliminated by the exclusion criteria. One hundred and fifty-six participants were classified as the spinal pain group (n=70) and the asymptomatic group (n=86) based on the NMQ.</p><p><strong>Results: </strong>The total amount of physical activity was less in the spinal pain group than the asymptomatic group (p<0.05). The spi-nal pain group had higher levels of health anxiety than the asymptomatic group (p<0.05). Further, the percentage of people who experienced spinal pain in the past year was higher in the spinal pain group (p<0.05).</p><p><strong>Conclusion: </strong>People experiencing spinal pain during the COVID-19 lockdown were physically less active and more concerned about their health. We present a patient with acute painful paraplegia and symptoms resolved within a few hours. We identified 10 patients in literature search. Five of them were male, the mean age of patients was 53.8. Paraparesis/pleji was present in all. Only two patients did not have radicular pain. Vascular risk factors were hypertension in five patients, smoking in five and diabetes mel-litus in one. TIA period ranged from 1 min to 24 h. The most common etiology was aortic dissection (n=6). Four patients had aortic thrombosis. Six patients were treated with medical and surgical methods; other four were treated with only medically. This study aims to investigate the attitude changes of pain physicians in Turkey in the treatment of chronic pain patients and the compliance of these changes with the published guidelines.</p><p><strong>Methods: </strong>A total of 113 pain physicians were sent an online questionnaire forms to be completed voluntarily.</p><p><strong>Results: </strong>The questionnaire was completed by 61% (n=69) of the total physicians to whom it was sent to. The rate of physicians who did not request the COVID-19 polymerase chain reaction test from their patients before any interventional procedure was 48% (n=33). The rate of physicians who ignored the immunosuppressive effect and while prescribing opioids and did not reduce the opioid dose was 42% (n=29). The rate of physicians who did not reduce the corticosteroid dose they used in their interventional procedures was 61% (n=42). It was determined that 49.1% (n=28) of physicians who applied facet joint medial branch radiofrequency denervation (RFD) during the pandemic period decreased the number of diagnostic blocks they ap-plied compared to the pre-pandemic period. Data were collected using the data collection form, examining nurses' demographic characteristics (10 items) and the questionnaire (14 items).</p><p><strong>Results: </strong>In this study, the interclass correlation coefficient was calculated as 0.80. Items' suitability was assessed using the experts' opinions, and the internal consistency was examined considering the Cronbach's alpha value. Cronbach's alpha value ranged from 0.78 to 0.81 for the items in the scale. This value was 0.81 for all 14 items on the scale, and the reliability was found to be high. The aim of this retrospective study was to examine the characteristics of patients diagnosed with NP.</p><p><strong>Methods: </strong>A total of 25 female patients, aged 26-64 years (mean+-10.67 years; median 47.17 years), were studied to determine the spinal or peripheral origin of NP.</p><p><strong>Results: </strong>On radiological examination, discopathy was observed in 24% of the patients, arthrosis in 32%, and degeneration in 56%. There was no significant relationship found between dermatological symptoms and magnetic resonance imaging posi-tivity (p=0.188). The patients' treatment programs were based on either physical therapy (44%), local injection (28%), exercise therapy (16%), or physical therapy and kinesiotherapy (12%). Subjective measures for pain were validated with an average visual analog scale (VAS), and the 'Douleur Neuropathique 4' (DN4) questionnaire was used to screen for neuropathic pain. The VAS score of the patients before treatment was 7, and after treatment was 2. The mean DN4 score was 7 before treatment and 2 after treatment.</p><p><strong>Conclusion: </strong>NP is a rare syndrome of unknown etiology.
