We report a case of very late-onset RTAD occurring 9 years after hybrid aortic arch repair. An 81-year-old man with a history of TEVAR with cervical debranching for subacute thrombosed RTAD, which later developed an ulcer-like projection, presented with dizziness and back pain. Acute RTAD with cardiac tamponade was diagnosed, and emergent ascending aortic replacement with resection of the entry tear-located at the proximal edge of the stent on the lesser curvature-was performed. Postoperatively, the patient required prolonged ventilatory support for respiratory failure and developed pyothorax secondary to aspiration pneumonia. He was ultimately discharged home on postoperative day 188 after rehabilitation. This case underscores the importance of a distinct pathophysiology in late RTAD, in which progressive aortic wall degeneration and biomechanical fatigue over time-rather than procedural factors-may contribute to dissection. We report abdominal organ injury cases not attributed to high-energy blunt trauma but seemingly minor blunt trauma with focal impact.Case 1:A 6-year-old patient was running in a park and slipped one day before. The FAST examination was positive, revealing hepatic injury. Computed tomography demonstrated a massive subcapsular hemorrhage of the liver.Case 2:A 6-year-old patient fell in a room the day before. She experienced trauma to the epigastrium after hitting a water bottle during the fall. The FAST examination was positive, revealing pancreatic injury. Computed tomography was performed and showed a hematoma anterior to the pancreas.Case 3:A 7-year-old girl was practicing bar pullovers one day before. FAST examination was negative;however, targeted ultrasonography revealed a hyperechoic lesion, and computed tomography revealed a hematoma in the duodenal wall.These cases had abdominal organ injury caused by seemingly minor blunt trauma with focal impact. {"title":"The prognostic significance of pharmacological therapy after cardiac resynchronization therapy.","authors":"Takamasa Sato, Sadahiro Murota, Minoru Nodera, Shinya Yamada, Takashi Kaneshiro, Takafumi Ishida, Yasuchika Takeishi","doi":"10.5387/fms.25-00040","DOIUrl":"https://doi.org/10.5387/fms.25-00040","url":null,"abstract":"<p><strong>Background: </strong>Pharmacological therapy is essential in heart failure with reduced ejection fraction (HFrEF), but its prognostic role at the time of cardiac resynchronization therapy (CRT) remains uncertain.</p><p><strong>Methods: </strong>We retrospectively studied 272 patients (mean age 66 years;72% male) who underwent CRT between 2005 and 2023. Pharmacological therapy was assessed only at baseline. Medication included β-blockers (97.4%), renin-angiotensin system inhibitors or angiotensin receptor-neprilysin inhibitors (RAS inhibitors/ARNIs, 81.3%), mineralocorticoid receptor antagonists (64.7%), and loop diuretics (76.8%). The primary outcome was cardiac death or left ventricular assist device (LVAD) implantation. Cox models evaluated associations between baseline medication use, medication doses, and outcomes.</p><p><strong>Results: </strong>During a median follow-up of 4.2 years, 126 patients (46.3%) experienced the primary outcome. Baseline use and higher β-blocker dose were independently associated with a lower risk of the primary outcome. RAS inhibitors/ARNIs use was also associated with a lower risk of cardiac death or LVAD implantation. By leveraging these interactions, researchers can create innovative solid forms with customized physical and chemical properties. This versatile approach is especially valuable for tackling the solubility and bioavailability issues commonly associated with poorly soluble drugs. In this study, we focused on synthesizing cocrystals of itraconazole (ITZ) and proline via the kneading technique, an eco-friendly crystal engineering technique. Our work included comprehensive solid-state characterization, stability assessments, and in vitro efficacy evaluations. We specifically examined how these cocrystals performed in terms of dissolution, solubility, and antifungal activity. Our solid-state characterization results highlighted the formation of significant intermolecular interactions within the cocrystals, particularly hydrogen bonds between ITZ and the functional groups of proline. Notably, the ITZ-proline cocrystals demonstrated superior solubility and faster dissolution rates compared to pure ITZ, alongside increased crystallinity in the newly formed solid state. Additionally, the cocrystals exhibited enhanced antifungal activity and stability. Few reports have described extremely low birth weight (ELBW) infants with CdLS. We report an ELBW infant diagnosed with CdLS. The infant was delivered by emergency cesarean section at 34 weeks and 1 day of gestation, weighing 994 g (-3.7 SD score). A diagnosis of CdLS was suspected based on dysmorphic features, including forehead hirsutism, cleft palate, long philtrum, wide nostrils, syndactyly of toes, femoral hirsutism, and hypospadias. Chromosomal analysis revealed a normal male karyotype (46,XY). Bilateral sensorineural hearing loss was also confirmed. Additional findings included atrial and ventricular septal defects. During hospitalization, the infant developed urinary tract infection and gastric volvulus but survived without severe complications. However, interpreting early serum human chorionic gonadotropin (hCG) levels remains challenging, especially in the absence of endogenous luteal function. This case report describes a 30-year-old woman who underwent FET with a high-quality blastocyst under HRC. At 4+1 weeks, her serum hCG level was 6.6 mIU/mL ; hormone support was discontinued, assuming a biochemical pregnancy loss. No follow-up hCG testing or transvaginal ultrasound was done. At 8+1 weeks, she presented with acute abdominal pain and was diagnosed with a ruptured tubal ectopic pregnancy with massive hemoperitoneum. Emergency laparoscopy revealed 1,500 mL of intra-abdominal bleeding ; right salpingectomy was performed and histopathological examination confirmed a tubal pregnancy. This case highlights the diagnostic limitations of relying on a single low hCG measurement during FET cycles. Even low or borderline hCG levels are compatible with ongoing or ectopic pregnancies. Clinicians should be cautious before discontinuing luteal support and should consider continued follow-up when hCG is detectable. With HPV DNA testing, the oral rinse of RRP patients may be a useful liquid biopsy, as previously shown in patients with oropharyngeal cancer.</p><p><strong>Methods: </strong>Oral rinse, along with palatine and pharyngeal tonsil swabs, were collected from seven patients with persistent RRP. HPV DNA detection was performed using polymerase chain reaction, followed by genotype identification.</p><p><strong>Results: </strong>HPV DNA was detected in five of seven oral rinse samples, but not in any palatine or pharyngeal tonsil swabs. HPV6 was identified in four of the five HPV-positive oral rinses, which was consistent with the RRP tissues.</p><p><strong>Conclusion: </strong>HPV DNA can be detected in oral rinses from patients with RRP, suggesting the utility of the oral rinse as a liquid biopsy. {"title":"Positive outcomes of disease awareness information conveyed by high school students to their guardians.","authors":"Hiroyuki Terawaki, Masaru Isoyama, Yoshikazu Nemoto, Megumi Nakamura, Lihua Wang, Takafumi Ito","doi":"10.5387/fms.25-00020","DOIUrl":"10.5387/fms.25-00020","url":null,"abstract":"<p><strong>Background: </strong>This questionnaire-based study aimed to evaluate whether health information conveyed by high school students can encourage their guardians to undergo annual health checks, regardless of their guardians' health literacy (HL) level.</p><p><strong>Methods: </strong>After a lecture to students by an internist on lifestyle-related diseases and chronic kidney disease (CKD), we asked their guardians (n = 1,027) if the students had shared information about the lecture with them and their willingness to participate in an annual health check. We also measured guardian HL using a self-administered questionnaire from which an HL score (minimum=1;maximum=5) could be derived.</p><p><strong>Results: </strong>The median HL score (with interquartile range) of guardians was 3.8 (3.2-4.0).
