{"title":"Deaths of despair – causes and possible cures","authors":"H. Burns","doi":"10.2307/j.ctv177tgp0.10","DOIUrl":"https://doi.org/10.2307/j.ctv177tgp0.10","url":null,"abstract":"This chapter discusses the decline in life expectancy in the United States (US), United Kingdom (UK), and other European countries since 2014. This demographic phenomenon appears to be related to underlying social and economic factors, leading to 'deaths of despair'. Social isolation and poor labour market opportunities negatively impact on physical and mental health. There are particular concerns regarding increasing levels of premature deaths in younger age groups. In many cases, deaths are due to drug overdoses, suicide, alcohol-related problems and 'external causes', such as violence and accidents. These social determinants are, to a large extent, influenced by local and national politics. Following the devolution of statutory powers to Scotland and Wales, there are quite distinct health and social care policies emerging across UK regions. {"title":"Devolution and the health of Scottish housing policy","authors":"I. Home and housing represent a key sphere in which local authorities can contribute to the health and wellbeing of their residents, including the most marginalised groups in society. Prior to devolution, Scotland had some policy autonomy and Scots Law has long been distinct from the rest of the United Kingdom (UK). Nonetheless, devolution represented a step change in the capacity of the Scottish policy community to shape its own destiny. The chapter reviews key developments in housing policy under the devolved administrations led first by the Labour/Liberal Democrat coalition and then by the Scottish National Party. Implementation of housing policy remains heavily dependent on local authorities and their partners. The impact of austerity measures imposed by central government amounted to an effective reduction in Wigan Council's budget of 40 per cent phased in over 10 years, and a loss of around a fifth of its workforce. Wigan experienced the fifth highest cut in spending out of all local authorities. The response, the creation of the 'Wigan Deal', is a major transformation programme which is still ongoing at the time of writing. It considers how the Municipal Corporations Act (1835) led to the establishment of elected town councils. In the mid- to late 20th century, municipalisation gave way to centralised government, which subverted the autonomy of local authorities. Currently, social care is provided and funded by local authorities and private funders. The main objective of social care is to help people to live well and happily, and live as long as they can. This person-centred approach is in contrast to the systems that have been developed to support the health care needs of people. The authors set out some of the changes they have seen: the integration of health and social care, and the creation of local care systems; the bringing together of health and care commissioning; and the crucial role of partnerships across health, care, the voluntary and community sector, housing, police, fire, and others. One of the arguments for local authority leadership on devolution is that good health is about more than the National Health Service (NHS) — it is about good food, warm homes, decent jobs, good education, close friends, and open green and safe public spaces (to name just a few). Devolution is about connecting all these agendas through local leadership and local action. Devolution is complex, as are the relationships between Greater Manchester and local democratic leadership in each of the ten boroughs. The Youth United Foundation (YUF) supports the development of well-established community-based organisations including the Scouts, Guides, and Boys' Brigade, joined recently by the creation of new uniformed youth organisations, including Fire and Police Cadets, to help significantly increase opportunities for young people from the most disadvantaged communities. Building on the place-based policies of the London Borough of Sutton, Sutton Community Dance (SCD) is an example of reimagining the local high street and prioritising shared places as an important context for building intergenerational bridges. Such a model of reimagination and creative agility will be critical in helping already challenged town centres to develop new possibilities for reform post the COVID-19 pandemic.
