It considers this question through the lens of public sector performance measurement and management (PSPMM), and how this has evolved towards increased complexity by moving from an output (activity) to an outcome (results) focus. The chapter then looks at the different aspects of complexity that arise when seeking to commission activity that creates positive outcomes for citizens and what a complexity-informed response requires. It also reflects on the way in which these requirements challenge existing public management arrangements, particularly in the field of PSPMM, as it applies to commissioning and performance management. For many years, the National Health Service (NHS) and local authorities have been attempting to deal with 'wicked issues'. Issues such as homelessness, disaffection of young people, and the ageing society that have complex multiple causes require joined-up approaches by the statutory and third sectors at national and local levels. In 2012, at the time when Public Health responsibilities were transferred from the NHS to local authorities, health and wellbeing boards (HWBs) were established in England. With few exceptions, HWBs punch below their weight and are not the powerful system leaders that were hoped for. These significant system changes provide a backdrop to current challenges in moving towards an integrated system of health and social care. The chapter discusses challenges related to NHS services being free at the point of use and their integration with means-tested social care. Working in Greater Manchester, the authors show how one of the first UK regions in which health budgets have been devolved to the local authority has allowed the development of governance arrangements and agreed strategic plans across the health and social care sector to promote relationship building. These developments have been underpinned by politically driven reorganisation. The Greater Manchester Partnership has increased the possibility of dealing with the complexity of 'wicked issues' (related to individual and community needs), supported by a Transformational Fund. It has been acknowledged and debated for a long time that the social care funding system is not fit for purpose, and in its current form is untenable both monetarily and politically. The focus of the funding debate continues to be on those individuals who do own their own home and how they make a valuable contribution to maintaining the current system. There is consideration that this system is unfair on individuals who are deemed to have provided for their future, but little consideration of the implications for individuals who, whether through choice or circumstance, have no assets to use towards the cost of their care. Despite one in four people over the age of 75 not owning their own home, they have been largely ignored as part of the deliberations. Disappointingly, the interpretation of what is fair seems based on what can be contributed in financial terms. The wider determinants of health — housing, environment, leisure, income, and education — are well recognised and are just as important as access to good health care. If we want to develop long-term strategies to enable social and cultural change with new models of care, more community-based responsibility for our neighbours and more freedoms and flexibilities to help us make better choices, then the role of place is key. place-based approaches, involving health and social care integration and outcome commissioning, have been developed with a view to a consideration of 'health' in all council planning. This reflects the place-based approach that is being promoted by the London Borough of Sutton as it delegates its statutory duties across the range of services it manages, collaborating with other statutory and third sector organisations within and beyond the two parliamentary constituencies of Sutton and Cheam, and Carshalton and Wallington. As a devolved service, health policy and the wider public policy legislative framework are the responsibility of the devolved Welsh Government tasked under the Government of Wales Act 2006 with developing and implementing policy, exercising executive functions, and making subordinate legislation. The chapter outlines the organisations involved and the context of the policy in Wales. It also explores the political drive for more collaboration between different organisations as a mechanism for the delivery of better services. The chapter then examines a unique piece of legislation in Wales, the Well-Being of Future Generations Act 2015 (WFG Act), and identifies how this may influence the work of local authorities and other organisations in relation to health and its social determinants.
