Although OKC has been reported as a manifestation of nevoid basal cell carcinoma syndrome (Gorlin syndrome) in many pediatric patients, there have been few reports of OKC occurring in isolation. Herein, we report the case of a relatively large OKC in the mandibular body of a 7‐year‐old girl who was treated conservatively using cystectomy and open wound surgery, and the permanent teeth were unaffected. Case presentation A 20‐year‐old man with a history of pharyngeal flap construction for a cleft palate was planned for orthognathic surgery. Preoperative nasopharyngoscopy showed that the space lateral to the flap was too narrow for nasotracheal intubation; then submental intubation was performed. No intra‐ or postoperative complications were observed. Methods This prospective study included 120 patients with temporomandibular disorders (TMD), including 60 patients with TMJ‐OA and 60 without TMJ‐OA, and 60 without TMD patients. All patients underwent magnetic resonance imaging of the TMJs at our hospital between November 2011 and January 2017. The regions of interest included the right and left lateral pterygoid muscles, medial pterygoid muscles, and masseter muscles on axial slices. Magnetic resonance images from the TMJ‐OA and control groups were independently evaluated by two oral and maxillofacial radiologists. ADC values were compared between the two groups using Mann–Whitney U tests. Results The respective mean ADC values of the lateral pterygoid muscles in patients with TMJ‐OA and controls were 1.18 × 10 −3 and 1.33 × 10 −3 mm 2 /s, respectively. Those for the medial pterygoid muscles were 1.14 × 10 −3 and 1.25 × 10 −3 mm 2 /s, while those for the masseter muscles were 1.20 × 10 −3 and 1.28 × 10 −3 mm 2 /s, respectively. The mean ADC values of the normal lateral pterygoid, normal medial pterygoid, and normal masseter muscles were 1.29 × 10 −3 , 1.09 × 10 −3 , and 1.08 × 10 −3 mm 2 /s, respectively. Both lateral cephalometric and CBCT images of the patients were used for angular and linear measurements. Angular measurements included vestibular concavity and lingual and tooth inclination angles. Vestibular and lingual cortical bone and total bone thickness measurements were obtained for the comparison of linear measurements. Three radiologists performed the measurements on 200 images. Comparison of the measurements and the imaging methods were done using paired‐ and Wilcoxon t‐tests. Inter‐observer reliability was evaluated with the intra‐class correlation coefficient (ICC).A significant difference was observed between measurements of two imaging methods for tooth inclination and vestibular concavity angles; however, no difference was found between LC and CBCT measurements for lingual inclination angle (p > 0.05). We sometimes extract the teeth in contact with the lesion to prevent recurrence.A 14‐year‐old male was referred to our hospital by his family dentist for the evaluation of a posterior left mandible lesion. We extracted the mandibular left second and third molar and performed enucleation and curettage of the OKC. He had GSD1a. Despite perioperative blood glucose control, lactic acidosis worsened. No abnormal bleeding or infections were observed.
