We report a case of a 75-year-old woman with de novo AVH temporally related to hemp oil use, in the general context of longstanding hearing impairment. Symptoms persisted and progressively worsened over 12 months despite discontinuation and antipsychotic treatment, resulting in marked distress and functional impairment. Adjunctive repetitive transcranial magnetic stimulation (rTMS) targeting the left temporoparietal cortex was administered in 2 separate treatment courses. Each course was associated with a marked reduction in AVH severity and enabled substantial tapering of antipsychotic medication. In both instances, symptom relapse occurred within 2-3 months after rTMS discontinuation. This case study illustrates the importance of a comprehensive, interdisciplinary assessment when the clinical presentation is complex. Johanne (fictional name), a woman in her forties with no prior psychiatric history, became a high user of emergency services following acute psychosocial stressors. For several years, the diagnoses retained were BPD with dependent traits. However, outpatient clinical observations raised doubts about the accuracy of these diagnoses. A thorough reassessment, including a functional occupational therapy evaluation and a neuropsychological assessment, revealed significant cognitive limitations consistent with borderline intellectual functioning. These findings suggested that her crises stemmed less from abandonment fears typical of BPD and more from situations exceeding her cognitive and adaptive capacities. Adjusting the interventions (substantial simplification of Dialectical Behavior Therapy [DBT], emphasis on concrete problem-solving, and clarification of her crisis plan) led to notable improvements, including a marked reduction in emergency visits and a sustained return to employment. In this context, Return-to-Work Coordinators (RTW-C) play a central role by coordinating the various stakeholders involved and supporting the implementation of tailored interventions. Context RTW unfolds through a three-phase sequence, from the initial sick-leave notification to the consolidation of full reintegration. The use of standardized tools, such as the Return-to-work obstacles and self-efficacy scale (ROSES), provides essential support for guiding both clinical and organizational interventions carried out by RTW-C. Case Description This clinical and organizational case study examines the support process implemented for a nurse on sick leave for an adjustment disorder. Two RTW-Cs describe their approach: assessing obstacles, identifying priority issues, and implementing targeted strategies. These include psychoeducation, developing a relapse-prevention plan, preparing meetings with the supervisor, and continuously adjusting interventions throughout the graded RTW process. Practice Implications This case report highlights the crucial role of RTW-C in coordinating RTW for employees with CMD. In this educational organizational case study, we present and highlight a recent partnership between a clinic specializing in the treatment of personality disorders (affiliated with the public health care system) and an employability organization (affiliated with the community sector) as part of the implementation of the program called Borderline Intervention for Work Integration (BIWI), an intervention designed to support work reintegration of people living with borderline personality disorder. The main challenges encountered were related to integrating the intervention into the service pathways and coordinating between the various services. These challenges were overcome through curiosity, flexibility, and dialogue. The collaborative experience was appreciated by both practitioners and service users. Positive outcomes were observed with respect to increased confidence and sense of competence; enthusiasm for collaboration remains strong. In parallel, the use of metabolic interventions is gaining credibility in the management of psychiatric disorders such as bipolar disorder. We present here the case of a 34-year-old woman with a known diagnosis of bipolar disorder type II, mixed personality disorder, and alcohol and cannabis use disorder, who exhibits euthymia with persistent residual anxio-depressive symptoms and impaired overall functioning. As part of a therapeutic approach complementary to psychopharmacology, the patient undertook a ketogenic nutritional intervention lasting 3 months. At the end of the intervention, a clinically significant improvement was observed, both in anthropometric parameters and in psychiatric symptoms. The patient also reported a marked improvement in her level of daily functioning and quality of life. Reassurance is generally indicated, but there is no validated treatment for patients with frequent and disabling symptoms. We report the case of a man who developed EHS after discontinuing chronic cannabis use. Daily symptoms and the associated distress led to a relapse into use. In parallel, the patient learned Schultz autogenic training through an artificial intelligence tool. This training was undertaken with the aim of treating occasional sleep terrors, during which he would scream and awaken his wife at the beginning of the night (while he himself was amnestic of the events). The patient describes a specific evening during which the combination of autogenic training and high-THC cannabis appeared to abort the onset of an EHS episode. Specifically, he reports that a heightened trance state facilitated the generation of a specific formula that terminated the episode. Since then, the patient has continued to practice autogenic training daily and has had no recurrence of EHS. He ultimately succeeded in completely discontinuing cannabis. This case suggests that self-hypnosis could represent a promising non-pharmacological approach for patients with severe forms of EHS. It is caused by the use of one or multiple serotoninergic medication(s), which results in excessive activation of serotoninergic receptors. Here, we present a case of probable SS following the addition of trazodone to treat insomnia. Besides trazodone, the patient was prescribed valproic acid, lurasidone, estradiol, dexlansoprazole and spironolactone. A few hours after taking trazodone 100 mg, the patient developed several physical symptoms, including chills, nausea, sweating, dizziness, generalized muscle stiffness and tremor. The symptoms were associated with significant functional impairment and gradually subsided over the next 48 hours. No physical examination or laboratory test were performed when the patient was symptomatic. However, the patient meets the diagnostic criteria for SS using the Sternbach criteria. Although most cases of SS associated with trazodone involve selective serotonin reuptake inhibitor antidepressants, this case report suggests that the combination of trazodone, lurasidone and valproic acid can lead to excessive serotoninergic activity via serotonin reuptake inhibition, 5-HT1A partial agonism and increased central serotoninergic neurotransmission. The originality of this case lies in the progressive emergence of atypical obsessive-compulsive phenomena during phases of structured psychosocial reintegration. These phenomena intensify in contexts of multiple relational and institutional demands and attenuate when the patient withdraws from certain commitments or imposed frameworks. Predominant symptoms include persistent intrusive ruminations, ritualized somatic self-soothing behaviors, and repetitive actions that are poorly modifiable through reasoning. The differential diagnosis, therapeutic interventions, and clinical evolution are discussed in light of contemporary models of obsessive-compulsive disorders, complex post-traumatic stress disorder, and reintegration trajectories among justice-involved patients with psychotic disorders.
