However, kidney complications developed after performing hematopoietic stem cell transplantation can affect the course and prognosis of the disease in patients with acute leukemia. This study is aimed at assessing the functional status of the kidneys in patients with acute leukemia who have undergone hematopoietic stem cell transplantation. The study has observed a group of patients with acute lymphoblastic leukemia and acute myeloid leukemia who have undergone hematopoietic stem cell transplantation. It has been discovered that β2 microglobulin is a sensitive method of analyzing renal function, with the β2 microglobulin threshold urine level not exceeding 0.3 mg/L. The complex diagnostics of kidney function in hematopoietic stem cell transplantation recipients has given the opportunity to identify the relationship between increased β2 microglobulin levels and decreased glomerular filtration rate. It has been determined that β2 microglobulin is a biomarker of renal disorders. The obtained data have showed that β2 microglobulin can be used as a diagnostic marker of reduced kidney function.","PeriodicalId":43715,"journal":{"name":"Italian Journal of Medicine","volume":" ","pages":""},"PeriodicalIF":0.4,"publicationDate":"2023-02-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"42263212","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

FADOI conducted a survey aimed at obtaining information on the management of HF in Italy, with particular reference to the Internal Medicine (IM) Units. The questionnaire, containing 32 questions, was sent to the Directors. Up to August 2022, it was possible to analyze data from140 centers. In 47.4±17.2% of cases with HF, the patients have preserved systolic function; 41% are in NYHA class III, 33% in class IV (33%), 21% and 5% in NYHA class II and I, respectively. The mean length of hospital stay is 9±2.5 days, with an inhospital mortality of 11.9%±9.5%. After discharge, the re-hospitalizations are 21.3±12.7%. In 44.4% of IM departments, there is a formal diagnostic-therapeutic care pathway. Also, through surveys, it is possible to help hospitals to and optimize assistance to patients affected by HF.","PeriodicalId":43715,"journal":{"name":"Italian Journal of Medicine","volume":" ","pages":""},"PeriodicalIF":0.4,"publicationDate":"2023-02-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"43258241","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objectives: The present study aimed to investigate the association between circulating IL-6 and minerals dysregulation in patients diagnosed with ESRF and on a continuous hemodialysis regimen. Methods: This cross-sectional study included 74 patients undergoing continuous hemodialysis. Serum samples were tested for IL-6 using an enzyme-linked immunosorbent assay. Mineral were analyzed using an electrolyte analyzer and biochemical tests. Parameter correlations were analyzed using the Pearson’s correlation test. Results: Among the studies group, the male: female ratio was 1:0.72. IL-6 mean value was 13.77 pg/ml ±9.79 SD. IL-6 was significantly negatively correlated with circulating iron and calcium levels (r= - 0.229, P= 0.049; r= -0.252, P= 0.03, respectively). IL-6 was significantly positively correlated with K+ levels (r= 0.269,P= 0.02). Conclusion: The present study highlighted the substantial role of IL-6 in mineral dysregulation in hemodialysis patients, highlighting this cytokine as a potential therapeutic target for minimizing and monitoring the clinical effects of mineral disturbances, including cardiovascular and neurological complications.","PeriodicalId":43715,"journal":{"name":"Italian Journal of Medicine","volume":" ","pages":""},"PeriodicalIF":0.4,"publicationDate":"2023-01-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"45999564","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We examined results of 205 COVID-19-recovered patients from 15 December 2021 to 1 March 2022 in two hospitals of Local Health Authority of Alessandria (Italy) during the fourth pandemic wave. 77% of patients were hospitalized for acute respiratory failure (ARF) with radiological pneumonia pattern (recovered for COVID), 23% for other causes with occasional positivity finding (recovered with COVID). 32% of patients were not vaccinated for Sars-COV-2, 37% had three doses, 25% two doses, 5% only one dose. All patients without vaccination were hospitalized for ARF and they had a 7 times higher risk of hospitalization than the vaccinated. 60% of all patients had >3 comorbidities, of these 50% was vaccinated with three doses. In the fourth pandemic wave compared to the other not all patients were hospitalized for ARF and pneumonia and the presence of comorbidities >3 is a risk factor for hospitalization regardless of vaccination status. This justifies the administration of the fourth dose to frail patients.","PeriodicalId":43715,"journal":{"name":"Italian Journal of Medicine","volume":" ","pages":""},"PeriodicalIF":0.4,"publicationDate":"2023-01-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"47447805","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In some patients the test is falsely negative, while other biological samples are positive. The aim of the study is to identify characteristics and prognostic factors for swab negativity in COVID-19 patients with BA-confirmed disease. \u0000Materials and methods : Multicentric retrospective case-control study of patients admitted for COVID-19 between March and November 2020 in two internal medicine units of the AOU Careggi and in the Internal Medicine of the Hospital of Varese. Were enrolled patients aged ≥18 years hospitalized for COVID-19 with viral RNA isolation on biological specimen, considering as cases the patients negative to swab but positive to BA. For each case, four controls with positive swab at admission were enrolled \u0000Results : the study included 95 patients, 19 cases and 76 controls. The mean time between symptoms onset and swab was 2.65±1.9 days in cases, with a statistically significant (p= 0.003) difference compared to controls (5.53±3.0 days). Patients with negative swab had a longer mean length of stay and more frequent adverse outcome than controls. \u0000Conclusions : Swabbing within a short time of symptoms onset is a predictor for false negative. The patient was feverish and a grade III/VI continuous murmur was noted in all precordium. Patient’s blood analysis showed elevation of inflammatory markers and bacteriaemia caused by Granulicatella adiacens. Transoesophageal echocardiogram diagnosed infective endocarditis of the aortic valve. The patient completed 3 weeks of antibiotic therapy and was submitted to aortic valve replacement and correction of the interventricular communication. Before hospital discharge, routine transthoracic echocardiogram was done and a pulmonary valve vegetation measuring 6x7 mm, not previously described, was noted, as well as de novo severe pulmonary valve insufficiency. The surgical risk was very high in this patient. Antibiotic therapy was extended and the patient was maintained under close follow-up in appointments. Intranasal corticosteroids are recommended as a part of the treatment plan for each of these conditions because they can help reduce symptoms by reducing inflammation. In this randomized, controlled trial, 30 individuals with rhinitis were enrolled to compare the efficacy of a new nano formulation of Fluticasone Propionate nano-nasal spray (FP-NNS) with the commercially available FP-NS nasal spray in the treatment of allergic rhinitis (15 to 60 years). Patients were administered 50 µg doses of FP-NNS in the morning and evening. This regimen was administered as a nasal spray during a 4-week phase evaluating its efficacy and safety. Each efficacy endpoint was analyzed using analysis of variance. More of our clinical studies have demonstrated that FP-NNS decreases inflammatory markers in both adults and children.","PeriodicalId":43715,"journal":{"name":"Italian Journal of Medicine","volume":" ","pages":""},"PeriodicalIF":0.4,"publicationDate":"2023-01-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"45606770","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
