An alternative method for skin grafting and cutaneous substitute for bum victims and chronic non healing wounds. The new method of skin grafting is applied repetitively and non-invasively until healing is achieved. PATIENTS AND PROCEDURE: 39 patients including 33 burns victims and six with chronic wounds were treated between February and September 2012. One to four square centimeters of autologous skin graft were transformed with biodegradation of cellular peptic bonds with the goal of obtaining a rich solution of cutaneous cells (1.5 million skin cells/cm2). The process duration averaged 100 minutes. Part of the solution was sprayed immediately on the recipient skin zones. The remaining solution was divided up and kept in cryotubes inside a container of liquid nitrogen. Successive sprays were performed at each dressing.</p><p><strong>Results: </strong>Good esthetic results on the whole as well as functional for patients with deep 2nd degree burns; healing was achieved in 10-12 days. Third degree burns and chronic wounds showed encouraging results for small surfaces.</p><p><strong>Discussion: </strong>We present a new original technique without prior in vitro culture that gives autologous cellular grafts, including epidermal, junctional and stem cells that are necessary for skin regeneration. The contraindications to the technique are infected, oozing, hemorrhagic or granulomatous wounds.</p><p><strong>Conclusion: </strong>This is a technique that deserves to be practiced and should have its place in the therapeutic arsenal of burns and chronic wounds.</p>","PeriodicalId":430072,"journal":{"name":"Le Journal médical libanais. Learning objectives: LSVC is a rare anatomical venous variation. In Lebanon, carbapenem resistance was studied among gram negative pathogens collected from a university hospital from January to June of years 2011 and 2012. All isolates were subjected to phenotypic tests including antibiotic susceptibility, cloxacillin effect, modified Hodge test, and Etest for metallo-β-lactamase detection. They were also subjected to genotyping by PCR sequencing to characterize β-lactamases. Between January and June 2011, 48 carbapenem non-susceptible strains were collected. Of these, one Klebsiella pneumoniae harbored OXA-48 and insertion sequence IS 1999; four Acinetobacter baumanni harbored simultaneously OXA-23 and GES-11, and three Pseudomonas harbored VIM-2 carbapenemase. Between January and June 2012, 100 carbapenem non-susceptible strains were collected. Of these, one K. pneumoniae harbored simultaneously OXA-48, IS 1999, and an acquired AmpC of the ACC group; four Serratia marcescens harbored OXA-48, while among eight A. baumannii, one strain co-harbored OXA-23 and GES-11, six harbored OXA-23 and one OXA-24. Fifteen P, aeruginosa and two Pseudomonas species harbored VIM-2; two P. aeruginosa strains produced IMP-1 and two others IMP-2. {"title":"[Supravalvular aortic stenosis with and without coronary lesions in pediatrics. A Lebanese study].","authors":"Ghassan Chehab, Jessie Darido, Issam El-Rassi, Bernard Gerbaka, Tarek Smayra, Zakhia Saliba","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>This study aims to analyze several parameters concerning the supravalvular aortic stenosis (SVAS) in children such as age of diagnosis, place of residence, the existence of a metabolic disorder or dysmorphic syndrome and possible damage to the coronary ostia, the means of diagnosis and outcome of these patients.</p><p><strong>Materials and methods: </strong>A large group of patients (2868) with congenital heart disease enrolled between 1 May 1999 and 30 April 2010 at the National Register of Pediatric and Congenital Heart Disease, Lebanese Society of Cardiology.</p><p><strong>Results: </strong>SVAS were found in 14 patients (0.5%) aged 8.5 months to 15 years. The Williams Beuren syndrome was the most common etiology (6 cases) without ostial stenosis, asymptomatic and not treated, followed by forms without dysmorphic syndrome (5 cases) and without ostial stenosis, one patient was operated because of severe SVAS; finally, 3 cases of homozygous familial hypercholesterolemia treated differently: a patient had a successful liver transplantation at age of 4.5 years but has developed, despite the normalization of cholesterol level, a SVAS associated with severe ostial lesions 10 years after transplantation, another treated by coronary artery bypass graft surgery and the latter treated medically.</p><p><strong>Conclusion: </strong>SVAS is a very rare disease, but its discovery must lead to search for coronary lesions especially in presence of homozygous familial hypercholesterolemia. The ultrasound monitoring is mandatory and is designed to detect this anomaly and early coronary lesions. Primary care physicians should be familiar with this condition to avoid complications and improve quality of care.</p><p><strong>Methods: </strong>We present two cases of hereditary angioedema followed by a discussion based on a literature review of the recent guidelines and advances in this condition.</p><p><strong>Objectives: </strong>To highlight the clinical aspects, diagnosis and treatment of this condition and propose a practical local management based on the available medication.</p><p><strong>Conclusion: </strong>Hereditary angioedema management is still evolving. In this paper, we employ survey data collected in Greater Beirut in 2009 to explore associations between family relations and health. over the life course. We tested (1) the extent to which age and social relation characteristics predict health; and (2) whether the association between age and health is stronger for those who report: smaller social networks and poorer relationship quality. We employed self-rated health and self-reported chronic illness as the health outcome measures and:social network size, positive quality and negative quality with family members as the social relations measures. Our findings suggest that social relations are differentially important depending on the health status indicator examined. The single dimension that influenced both self-rated health and the probability of reporting a chronic illness was positive relationship quality with spouse. Further, social relations, particularly having a negative relationship quality with spouse and adult child, exert stronger effects on both self-rated health and chronic illness for older compared to younger adults. The findings of the present study are important for clinical practitioners who often consider the role and importance of available social resources as they address the health needs of older adults.</p>","PeriodicalId":430072,"journal":{"name":"Le Journal médical libanais. Using a sample of 490 older Lebanese (age 60 and over), this study examines whether access to and utilization of medical care buffer the impact of specific stressors on depressive symptoms. Findings show that access to medical care is associated with depressive symptoms for those who experienced recent death, serious accident and health-related stressors and that limited access increases depression for those exposed to recent violent stressors.
