Nowicka-Sauer","doi":"10.15557/pipk.2020.0034","DOIUrl":"https://doi.org/10.15557/pipk.2020.0034","url":null,"abstract":"The paper presents the issue of anxiety disorders among children and adolescents with particular focus on generalised anxiety disorder and the use of cognitive behavioural psychotherapy in the affected patients. Data on epidemiology, aetiology and diagnosis of anxiety disorders, as well as the specificity and limitations of cognitive behavioural therapy among younger patients are presented. Statistical data show that the group of affected children and adolescents is growing. The diagnosis of generalised anxiety in children and adolescents may take time. It requires careful collection of history and observation of behaviours in different functioning domains. It may happen that symptoms indicative of generalised anxiety disorders are underestimated or missed by adults from the child’s immediate environment. Early and accurate diagnosis is important also because the disorder may give rise to depressive disorders or contribute to suicide attempts. Research shows that cognitive behavioural psychotherapy, often aided by pharmacotherapy, is one of the most effective and popular forms of therapy in children and adolescents with generalised anxiety disorders. The paper describes the theoretical models of the disorder, as well as its implications for psychotherapy and current research trends. Cognitive behavioural therapeutic programs intended for children and adolescents with anxiety disorders are also presented. The literature review shows that studies and protocols devoted exclusively to the treatment of generalised anxiety in children and adolescents are still missing. Studies show that psychotherapists are vulnerable to the effects of distress which, if left unattended, lead to burnout and serious professional impairments. Even though it has been emphasised that self-care of a psychotherapist is more of an imperative than an option, all too often professionals ignore their needs. Among the most common self-care myths cognitive behavioural therapists believe in, one may find the assumptions that self-care is optional, the knowledge how to look after oneself is equivalent to managing it, and coping is identical to being a therapist. Responsibility for oneself is usually an optional topic during education and supervision processes. Even though articles on that subject are widely available, it is important to analyse the problem of self-care, and to propose ways for creating a healthy work environment for cognitive behavioural therapists. In the article, the core ideas concerning the cognitive behavioural model of therapy are presented, and the cognitive behavioural model of burnout is described. In addition, the role of self-care in professional activity among cognitive behavioural psychotherapists is examined. The data on the role of awareness, balance, flexibility, and health in increasing self-care practices among psychotherapists is presented, and the importance of a proactive approach to self-care is highlighted. {"title":"Assessment of risky drinking, harmful drinking and alcohol addiction among lawyers","authors":"Katarzyna Chrobak-Kasprzyk, J. Studies conducted around the world have so far shown that this professional group is at a greater risk of alcohol addiction than the total population or other professional groups. Aim: The aim of this study was to determine the model of alcohol consumption among lawyers, as well as the factors influencing this phenomenon. Materials and methods: The study was conducted among 206 lawyers, including 102 women and 104 men, aged between 29 and 84 years. The AUDIT test, developed by World Health Organization, was used. Results: In the study group of lawyers, 22.33% drink in a risky and harmful manner, including nearly 3% whose score suggests alcohol addiction. It was shown that the sex criterion differentiated the alcohol consumption model among lawyers, with statistically significant higher average values obtained for men (p = 0.002). The study showed that the drinking model is influenced both by the marital status of respondents and whether they have children or not (p = 0.000033). There was no influence of weekly working hours on the alcohol use pattern (p = 0.169, r = 0.10). An analysis using the Pearson’s r correlation coefficient showed a statistically significant relationship (p = 0.021) between age and AUDIT score. This was a low-strength negative correlation (r = −0.16), i.e. AUDIT scores decreased with the age of respondents. Conclusions: This study is the first medical assessment of the prevalence of alcohol use among Polish lawyers. Lawyers are a professional group reluctant to undergo evaluation, who are at a high risk of problematic alcohol use. After 7 days of hospitalisation, the patient was discharged at her own request. On the same day, after a few hours, she returned to the emergency room, reporting severe anxiety and auditory hallucinations. Risperidone was replaced by paliperidone. During the treatment, a decrease in leucocyte count after paliperidone administration was observed. During further hospital stay, the treatment was switched to olanzapine in depot form. Correct symptomatic response to the drug used was observed. The leucocyte counts were gradually compensated, which confirms the drug-related cause of previously observed complete blood count abnormalities. This is due to a number of factors, including the fact that women tend to seek mental help relatively more often than men. Many developed countries have attempted to implement suicide prevention programmes and initiatives, but only a small proportion of these have targeted men directly. The aims of this paper are to highlight preventive measures focusing on mental problems experienced specifically by men, and provide a detailed description of these measures. Consequently, the paper has both theoretical and practical dimensions, outlining the challenges and presenting potential solutions. The study discusses selected examples of mental health actions targeting men which were carried out in several countries around the world including Japan (“Daddy, have you slept well?”), UK (“Heads Up” campaign), United States (“Real men. Real depression”), Canada (“HeadsUpGuys” campaign), and Australia (Movember), as well as social media campaigns (Instagram: #HereForYou). Men are often reluctant to reveal their mental problems (stress, low mood, insomnia, suicidal ideation), which can be attributed to the so-called macho culture existing in societies. The man is traditionally considered head of the family, a guardian and leader, a strong personality, and a person to rely on. This perception contributes to the fact that men tend not to admit their weaknesses in front of family or friends. It is recommended that preventive actions take into account such aspects as the transfer of knowledge, skills and competences in the areas of identification of mental problems, and abilities to cope with a crisis situation. However, men are about six times more likely to commit suicide than women. What could be the cause of this situation? The aim of the article was to analyse the potential correlation between mental health and suicide in men. Materials and methods: The review was based on the available scientific literature on male depression and suicide. We searched Pubmed, Scopus and Google Schoolar (from 1.11.2019 to 31.01.2020). Results: Polish statistics may differ from others due to the fact that current diagnostic criteria often fail to take into consideration the typical symptoms of mental disorders in men. Male suicide attempts are more commonly lethal. Men are also more likely to try to deal with mental problems on their own, often using alcohol, which, in many cases, leads to suicidal behaviour and addictions. This disparity between the numbers of suicides among men and women in recent years indicates an urgent need to promote mental health among men. Conclusions: Polish health policies should focus more on the mental health of men. It is very important to develop standards of promoting male mental health, which would meet the specific health needs of this group. However, excessive group identification could result in negative consequences for the identity of an individual. The sense of support in social relations is a significant personal resource of convicts. The social identity of convicted persons influences their behaviour, and thus the effect of penitentiary interactions. Hence, the aim of the study was to assess the sense of belonging in a group among convicts with body modifications. Methods: The research was conducted in penitentiary units at the District Inspectorate of the Prison Service in Gdańsk (Poland). Fifty-eight re-sentenced convicts participated in the study (36.2% did not have body modifications, and 63.8% had tattoo/tattoos). The age of the respondents ranged from 18 to 62 years (M = 35.12; standard deviation, SD = 10.98). The participants completed the Group Identification Scale (Mael and Ashforth, 1992), the Identity Fusion Scale (Gómez et al., 2011) and the measure of identity fusion (Swann et al., 2009). Results: The study showed that convicts declare a very weak sense of belonging with their cellmates (55.2%) and a very strong sense of belonging with their family (44.8%). A relationship was observed between younger age and identity fusion; however, it was on the border of a significant statistical trend. Conclusions: Interpersonal relationships used as a personal resource in the social rehabilitation process can be extremely important for the probability of recidivism in the future. Especially, non-standard methods may allow convicts to test themselves in a new role. Current ICD-10 and DSM-5 criteria for mental disorders obviously fail to exhaust the richness of psychopathologies and to fully describe patient’s experiences. Differentiation of psychotic experiences is of particular importance in pediatric psychiatry. A number of phenomena that can be classified as psychotic either occur as a variant of normal or, as reported in studies, have no clinical significance. We describe a case of an adolescent who, with a thoughtless use of criteria, could have been easily diagnosed with schizophrenia; however, a more thorough interview and observation allowed for identification of the phenomenology of psychotic symptoms in the course of conduct disorders.
