The use of digital tools can be advantageous to enhance communication between the dental team and patients. Moreover, a full digital approach would allow to check each single clinical step according to the treatment plan with a backward approach. The aim of this case report is to describe a multidisciplinary treatment program of a young patient with severe open bite and amelogenesis imperfecta, whose restorative rehabilitation was planned in a full-digital workflow, from the initial communication phase up to the final prosthodontic rehabilitation. \u0000Case report An 8-year old female patient was diagnosed with a Class II division 1 malocclusion, severe open bite and AI. The treatment plan included 3 phases: the elimination of the tongue interposition habit and the anterior open bite, the orthodontic correction of dental deviation and leveling of both dental arches and a prosthetic rehabilitation. The extraction of the four first molars corrected the canine and molar Angle Class II relationships, deviation of the midline and repositioning of the maxillary incisors on the sagittal plane. Feldspathic ceramics was used in anterior sites to enhance the esthetic outcome. Of those, 30 were Tapered Screw Vent (TSV) implants with an immediate loading protocol (TSVi), 30 TSV with delayed loading (TSVd), and 30 were one-piece implants with an immediate loading protocol (OP). Crestal marginal bone loss in the coronal area of dental implants was evaluated radiographically at three months and one year post-implant insertion.  \u0000Results Marginal bone loss was significantly higher after one year post-surgery compared to three months post-surgery in all the study groups. The mean values of marginal bone loss obtained by TSV implants were higher than those obtained with OP implants at both follow-up points. TSVd implants experienced the higher crestal marginal bone loss among all the study groups at both three months and one year.  \u0000Conclusions Crestal marginal bone loss in the most coronal part of one-piece implants  is significantly less than the marginal bone loss observed in tapered screw vent  implants with either immediate or delayed prosthetic loading protocols with single implant crown rehabilitations. Patwa","doi":"10.23805/JO.2020.13.01.1","DOIUrl":"https://doi.org/10.23805/JO.2020.13.01.1","url":null,"abstract":"Aim The present study aimed to evaluate the feasibility and efficacy of neoadjuvant chemoradiotherapy (NACRT) followed by definitive surgery and free fibular transfer in advanced stage oral squamous cell carcinoma patients. \u0000Methods Patients diagnosed with resectable oral squamous cell carcinoma, Stage IVa were enrolled for the study. The patients underwent a planned protocol of neoadjuvant radiotherapy and concurrent chemotherapy, insertions of dental implants in fibula followed by curative and reconstructive surgery. Objective response rate, overall survival, disease free survival and patient quality of life assessments were carried out. \u0000Results A total of eight patients with Stage IVa oral  squamous cell carcinoma were enrolled for the study and treated according to the planned protocol. All patients completed neoadjuvant chemoradiotherapy and surgery; 6 (75%) patients achieved complete response while 2 exhibited partial response (downstaging) on post surgical pathological analysis. After a median follow-up time of 24.6 months and maximal follow up of 34 months, the overall survival was 75% and disease free survival was 87%. Two patients suffered from trismus and are currently undergoing oral physiotherapy. Six patients had orocutaneous fistula which healed. One had extrusion of fibular graft and two patients died. \u0000Conclusion The present study elucidates a treatment strategy for patients with locally advanced squamous cell carcinoma validated for better functional and aesthetic outcome with improved quality of life. In particular, GPA is characterized by inflammation of small and medium-sized blood vessels that may affect different organs, such as kidneys, lungs and, most commonly, upper respiratory tract. Lesions of the oral cavity, are represented by a group of non-specific lesions that include deep ulcers of tongue, cheek and palate with the presence of oro-antral fistulae, osteonecrosis of hard palate and labial mucosal nodules. However, the most pathognomonic GPA oral lesion is the so called “strawberry” gingival hyperplasia, a gingival swelling of reddish colour with scattered darker petechiae. Generally, such lesions are present in about 6-13 % of GPA patients; however, the recognition of oral lesions can help in establishing early diagnosis and treat the disease avoiding severe systemic involvement. \u0000Methods A literature review consisting of a thorough literature search was performed. Granulomatosis with polyangiitis (GPA) and oral lesions publications were selected from PubMed/Medline. The following keywords and MeSH headings were used for the search strategy: Granulomatosis with Polyangiitis, Wegener granulomatosis, antineutrophil cytoplasmic antibody associated vasculitis, mouth lesions, and strawberry gingivitis. \u0000Results Clinical and radiological features are important tools in early diagnosis, but mucosal biopsies of the upper respiratory tract are still the  “gold standard” of GPA. Push out bond strength test was perfomed on  two types of cement: Kuraray Panavia (Japan) and Gradia Core (GC Tokyo Japan) in combination with two types of fiber posts: Edelweiss post (Edelweiss Dentistry Products GmbH, Wolfurt, Austria) and GC Fiber Post (GC Gradia Core Tokyo Japan). \u0000Results  According to the results of this study, the GC Post / Gradia Core group recorded push-out forces significantly higher than those of the groups in which the new post was used (p <0.05). \u0000Conclusion Within the limits of this in vitro study, it can be concluded that the highest values of adhesive strength in the push-out test were achieved by GC post/Gradia Core GC. The literature search was conducted using the following keywords: (“prosthesis” or “prosthetic”) AND “prognosis” AND “mandibulectomy”; (“prosthesis” or “prosthetic”) AND “prognosis” AND (“defect” or “reconstruction” or “resection”) and (“mandibular” or “mandible”). \u0000Results The selection process yielded 43 papers after the databases search, while 7 books and 3 congress proceedings were included after the manual search. \u0000Conclusion To enhance the prosthetic prognosis, the surgeons must preserve the tongue mobility and as many teeth as possible; vestibuloplasty could be needed, while intraseptal bone cuts and specific resection lines are recommended. Docimo","doi":"10.23805/JO.2020.12.03.4","DOIUrl":"https://doi.org/10.23805/JO.2020.12.03.4","url":null,"abstract":"Aim The aim of the present study was to evaluate in an adult population the correlation between different methods for the evaluation of halitosis and investigate the influence of prosthetic rehabilitation on halitosis level. \u0000Materials and methods A sample of 50 adult patients was selected at the Dentistry Unit of the University of Rome Tor Vergata, Italy, who were divided in Group 1 (absence of prosthesis), Group 2 (fixed prosthesis), and Group 3 (removable prosthesis). The assessment of oral malodor was carried out by organoleptic evaluation, measuring the concentration of H2S, CH3SH, and (CH3)2S with Oral ChromaTM and quantification of salivary β-galactosidases (Sβ-g) activity through the spectrophotometric method. Statistics: Anova, Postdoc LSD test, Pearson’s correlation coefficient (P) and Spearman’s correlation coefficient (Rho) were used; p values less than 0.05 were considered significant. \u0000Results A positive and significant correlation between  organoleptic evaluation, Sβ-g, levels of volatile sulfur compounds Oral ChromaTM measurements was found (p<0.05). By stratifying patients with and without a dental prosthesis, it was possible to show a significant increase of organoleptic scores (p<0.05), β-galactosidase (p<0.05), food stagnation  (p<0.01) and a higher scores of H2S (p<0.05) and CH3SH (p<0.05) in patients wearing a prosthesis. A 3D planning software was used to determine the correct placement, considering the bone volume, the functional and the aesthetic prosthetic result. Etiology of this phenomenon is unknown, but in some cases the presence of follicular cyst may be one of the causes. Canine transmigration is more prevalent in the mandible and women seem to be more affected than men. Mandibular canines play a crucial role in the oral cavity. Therefore, an early diagnosis is required, by the use of radiographs, especially orthopantomography or computed tomography. In case of transmigrated canine associated with follicular cyst, although several treatment options are described in literature, surgical extraction along with the enucleation of the cyst is the only treatment of choice. The first requires an analog workflow, the latter a full digital workflow. The aim of this study was to evaluate clinical performance of two lithium disilicate systems by means of the novel Functional Index for Teeth (FIT) after one year of clinical service. \u0000Materials and methods Partial adhesive crowns on posterior teeth were made on 60 patients, who were randomly divided into two groups: Group 1 Initial LiSi Press and Group 2 Initial LiSi Block (GC Co., Tokyo, Japan), which therefore followed full analog and digital workflows respectively. The restorations were followed-up for 1 year, and the FIT evaluation was performed at baseline and at last recall. The FIT includes 7 variables: Interproximal, Occlusion, Design, Mucosa, Bone, Biology and Margins, that are investigated using an intraoral radiograph and occlusal and buccal pictures and evaluated using a 0-1-2 score as follows. The presence or not of major, minor or no discrepancy (‘Interproximal’, ‘Occlusion’ and ‘Design’), presence or not of keratinized and attached gingiva (‘Mucosa’), presence of bone loss >1.5 mm, <1.5 mm or not detectable (‘Bone’), presence or not of Bleeding on Probing and or Plaque Index (‘Biology’), presence of detectable gap and marginal stain or not (‘Margins’). The Mann-Whitney ‘U’ test was used and a level of significance at p<0.05. Also, “success” of the crowns (restoration in place without any biological or technical complication) and “survival” (restoration still in place with biological or technical complication) were evaluated. \u0000Results All FIT parameters had high scores, between 1.85 and 2, and no statistically significant differences between the two groups were found (p>0.05). No statistically significant difference was found between baseline and recall scores. All FIT scores were compatible with the clinical success and not one restoration was replaced or repaired. The success rate at 1 year was 100%. \u0000Conclusions The FIT can be a viable standardized evaluation of the quality of prosthodontic therapy. The two materials showed similar results at 1 year.
