SARS-CoV-2 is a virus which is more aggressive in aged patients with associated chronic age-related diseases. Although it is known that clinical picture of COVID-19 is already multifaceted, very little is still known about the infectious syndrome in aged patients. We present a case of an aged patient hospitalized for fall affected from COVID-19 pneumonia, initially misinterpreted for an aspiration pneu-monia. Case presentation. We describe the case of a 92 years patient with chronic heart diseases, hospitalized for fall complicated by head trau-ma. She was diagnosed with an aspiration pneumonia due to the fall so that she was put under antibiotics treatment. Despite four days of antibiotics treatment, the patient presented high fever and dry cough motivating in an epidemic period the nasopharyngeal PCR test for the research of SARS-CoV-2 which resulted positive. Discussion and conclusions. Aspiration pneumonia resulting from a fall is a common finding in geriatric patients, that's why the diagnosis of COVID-19 was delayed. The geriatric population often have an altered clinical presentation of diseases, in particular regarding infectious diseases. The correct interpretation of some suspicious findings such as regular blood tests would have led more quickly to a diagnosis of non-aspiration pneumonia. Inflammatory markers and white blood cell count resulted within the normal range. Moreover, lymphopenia, considered as one of the biological1. The presence of confusion as a sign of persisting infection in an older patient should not be under-estimated, expression of a possible involvement of nervous system in SARS-CoV-2 infection2. The common diagnosis of aspiration pneumonia in the geriatric field, the aspecific signs and symptoms of COVID-19 disease, the complex picture of our patient, prove that in geriatrics an apparently simple diagnosis could instead be insidious and complex. Little is known about the relationship between sudden loss, social isolation, and onset of major depressive disorder (MDD) with psychotic features. We report two cases of MDD with psychotic features that developed in older women after the sudden loss of a spouse in the setting of social isolation in the COVID-19 pandemic. Case reports. Both women were brought to stay with family after their symptoms were discovered. Both experienced remission of symptoms on the combination of an antidepressant and a relatively low dose of risperidone (1.5-2 mg daily). Neuropsychological assessments were typical of normal ageing. Conclusions. These cases suggest that unexpected loss and social isolation may precipitate psychotic depression, and providers may want to recommend social engagement for older adults experiencing bereavement. High risk medications use is common at hospitals and poses a major risk for frail older adults. The study aims to determine the association between Potentially Inappropriate Prescrip-tions (PIPs) and hospital outcome among geriatric patients. Methods . A retrospective cohort study including 152 older adults (age ≥ 60 years). These patients were admitted to the intensive care unit (ICU) at Geriatrics hospital from August 1 st , 2021 to January 1 st , 2022. Age, sex, clinical data and medications used throughout admission were extracted for each patient. The target outcome was in-hospital mortality. PIPs were identified in accordance with Screening Tool of Older Per-sons’ potentially inappropriate Prescriptions version 2 (STOPP v. 2). Logistic regression analysis was done to test the association between use of PIPs and in-hospital mortality. Descriptive statistics was performed for PIPs and medications use near the end of life. Results . PIPs occurred in 67.8% (103 patients) of participants. Multi-variate regression analysis revealed independent predictors of mortality including older age (OR = 1.075; 95% CI: 1.020-1.134; P .007), delirium/ altered mental status on admission (OR = 2.688; 95% CI: 1.086-6.651; P .032), and utilizing of ≥ 3 PIPs (OR = 4.049; 95% CI: 1.320-12.421; P .014). Use of anticholinergics in patients with delirium or dementia was the most frequently reported PIPs among participants. Conclu-sions. PIPs are common and significantly associated with mortality among hospitalized older adults. To validate a survey tool in Italian, useful for assessing the perceptions and beliefs of healthcare professionals with respect to the person with dementia, with particular attention to the levels of dignity and personality (Personhood) that operators address to residents of facilities for the elderly. Methods . Data collection took place in two distinct periods, the first (T1) from May 2021 to July 2021; the second starting from December 2021 to February 2022 (T2). Results . The test-retest stability in the two measurements performed was confirmed by the value of the correlation coefficient Rho, very high and statistically significant (p < .05) in the comparison of each item at T1 and T2, respectively. Conclusions . In his medical history, the patient reported several comorbidities, among which also sarcopenia. De-lirium was already diagnosed by the geriatrician on hospital admission. The patient underwent hip endoprosthesis surgery after 24 hours without any intra-operative complications. However, in the post-operative period delirium persisted, causing a prolonged hospital stay, a delayed physio-therapy rehabilitation with poor functional recovery, and subsequent institutionalization. The prevalence of delirium in older people with hip fracture is extremely high and it is associated with several negative outcomes. De-lirium is considered a multifactorial disorder, and, in particular, sarcopenia appears directly linked to the development of delirium.
