Young females (median age 38) are the most commonly affected population (70%). CVT is a result of clot formation within a major cerebral vein. These veins include dural sinuses, cortical veins, vein of Galen and jugular veins [1]. Cases of CVT have a good prognosis with anticoagulation. Therefore, the aim of the study was to identify the deviation in walking pattern of 10-15 year old obese children from their non-obese counterparts. Methods A cross sectional comparative study was done at the Lady Ridgeway Hospital, Colombo involving 50 obese and 50 non-obese children aged 10-15 years. A simple random sampling method was used, and the participants were asked to walk along a 12m walkway with a 6m center recording zone, at a self-selected walking speed. The step length, stride length and cadence were measured by foot print analysis. A frontal stationary camera and a lateral running camera were used for two-dimensional walking analysis. Single limb and double limb support time was assessed by a Motion view 8.0 video analyzer using captured videos. Data was analyzed using the independent sample t test on SPSS 25. Ethics clearance was obtained from the Ethics Review Committee, Faculty of Medicine, University of Colombo. Results Non-obese children had a significantly higher mean step length, right stride length, left stride length and cadence than obese children (p 0.05). {"title":"Mixed connective tissue disease with secondary immune thrombocytopenic purpura -- a case report","authors":"S. Rupasinghe, W. Kularatne","doi":"10.4038/jpgim.8294","DOIUrl":"https://doi.org/10.4038/jpgim.8294","url":null,"abstract":"Mixed connective tissue disease [MCTD] is a generalized connective tissue disease which is confirmed by the presence of anti-U1 ribonucleoprotein (RNP) antibodies. The etiology of MCTD remains unclear. We present a case of a young female presenting with menorrhagia, symptomatic anaemia, small joint pain and raynaud’s phenomenon. She was found to have thrombocytopenia with positive anti-U1RNP antibodies. She was diagnosed as having MCTD with pulmonary hypertension and secondary immune thrombocytopenic purpura [ITP]. She was treated with corticosteroids and immunosuppressants which led to complete remission. Even though secondary ITP is associated with autoimmune diseases, cases of MCTD with secondary ITP are rare in literature. Cases of MCTD presenting with bleeding manifestations are rare as well. This should be considered as a differential diagnosis in a patient presenting with bleeding manifestations along with symptoms suggestive of an underlying connective tissue disorder. It is classified into two subtypes as unicentric CD and multicentric CD depending on the number of lymph node regions involved. Here we report a case of an elderly malepresenting with bilateral lower limb swelling, generalized lymphadenopathy and hepatomegaly. Left side cervical lymph node biospy was consistent with plasma cell type of Castleman disease. He was diagnosed as having‘not otherwise specified’ idiopathic multicentric Castleman disease. He was promptly treated with chemotherapy and steroids which led to improvement of symptoms. Accurate histological diagnosis is important since CD can clinically mimic lymphoma, which has a different prognosis. Fernando","doi":"10.4038/jpgim.8274","DOIUrl":"https://doi.org/10.4038/jpgim.8274","url":null,"abstract":"Background Ischaemic Heart Disease (IHD) is a major constituent of cardiovascular disease in the world at present and has become the leading cause of death in Sri Lanka. Poor adherence to medication in IHD is related to increased re-hospitalisation and mortality. Therefore, continuous assessment of medication adherence is extremely important among these patients. The Morisky Green Levine Medication Adherence Scale (MGLS) is a widely used instrument to evaluate medication adherence in the clinical setting. The study aimed to translate the MGLS into Sinhala and cross-culturally adapt it to the Sri Lankan setting. Methods A Delphi process was conducted with a panel of experts where content and consensual validity of the scale was assessed after translation, back translation and pre-testing of the original scale. The ratings received from the first round of the Delphi process were noted and the amended translation re-sent for a second round. The re-ratings were evaluated for the degree of consensus and Items for the MGLS Sinhala version were chosen based on the following criteria: (i) the item was reworded or removed if 70% or more of the re-ratings were in category 0–3. If reworded, the Delphi Process was repeated for that item, (ii) the item was kept with no change if 70% or more of the re-ratings were in categories of 4–6 and 7–9. Results No items were removed from the original scale. Some items were reworded according to the experts’ suggestions to retain the conceptual meaning when translated into Sinhala. The Sinhala version of the MGLS exhibited adequate content and consensual validity. {"title":"Pattern of utilization of facilities in the intensive care unit by elective surgical patients in a tertiary care institution: Can the services be provided by high dependency units in surgical wards?","authors":"Seneetha De Silva, A. It is a costly and time limited resource utilized by those with reversible pathologies and reasonable likelihood of survival. Patients awaiting elective surgery with preoperative intensive care unit (ICU) bookings need to compete with emergency and unanticipated admissions leading to cancellation and postponement of elective surgery, a disadvantage to both the patient and the institution. Hence this study is aimed at evaluating the extent of ICU care utilized by elective surgical patients and to analyse the indications vs. utilization of ICU care by them. Methods This was a retrospective cross-sectional study carried out in the surgical ICU of Colombo South Teaching Hospital, enrolling 47 elective surgical patients admitted to the ICU over a period of 3 months. Data was collected using a pre structured data collection form. Results Majority of the patients were middle aged, belonged to the American Society of Anaesthesiologists Physical Status 2 (68.1%) and underwent colorectal (26%), upper gastrointestinal (GI) (15%) or hepatobiliary (13%) surgeries. Main comorbidities amongst the study population were diabetes and hypertension. Mean duration of ICU stay was 2 days. Most patients were monitored non-invasively within the ICU (68%). Organ support was required by only 17% of patients. Most frequent indication for preoperative ICU booking was the surgical/anaesthetic complexity determined by British United Provident Association (BUPA) schedule of procedures (83%). However, ICU resources were mainly utilized for provision of analgesia particularly in the form of epidural infusions (52%).
