Relationship Between Decremental Response on Repetitive Nerve Stimulation and Amyotrophic Lateral Sclerosis Functional Rating Scale 肌萎缩侧索硬化症功能评定量表与重复性神经刺激减值反应的关系

{"title":"Relationship Between Decremental Response on Repetitive Nerve Stimulation and Amyotrophic Lateral Sclerosis Functional Rating Scale","authors":"","doi":"10.36283/pjmd12-3/009","DOIUrl":"https://doi.org/10.36283/pjmd12-3/009","url":null,"abstract":"Background: Amyotrophic Lateral Sclerosis (ALS), is a progressive degenerative disease of the motor neuronal cells, and its clinical presentation includes upper motor neuron symptoms, lower motor neuron symptoms, bulbar symptoms, or a combination of them all. This study aimed to measure the association between decrements in Repetitive Nerve Stimulation (RNS) and the overall functional status of ALS patients.\u0000\u0000 Methods: In this cross-sectional study, routine RNS was performed, and its association was measured with the demographic data, disease onset, and ALS functional rating scales (ALSFRS) in 20 patients having ALS. RNS was performed and patients were divided into RNS-positive and RNS-negative groups. ALSFRS was used to assess disease severity, statistical analysis was done using SPSS, and the student t-test was used for comparison among groups.\u0000\u0000 Results: Twenty ALS patients (mean age: 55.85±11.18 years; 65% males) were enrolled. The patients belonged to the following professions: laborers (20%), shopkeepers (20%), homemakers (10%), retired from work (10%), and teachers (5%). The mean duration of illness was 14.00±12.98 months. No significant difference was observed in RNS decremental responses or ALSFRS scores between patients with limb and bulbar onset. There was also no statistically significant association between RNS decrements and ALSFRS scores (p=0.975).\u0000\u0000Conclusion: The association between decremental response to RNS with the clinical correlation of patients with ALS was statistically not significant. The objective of this study was to determine the practice of lab staff before and after carrying out an intervention in the form of health education intervention based on WHO protocols for infection prevention.\u0000\u0000Methods: This is a Quasi-experimental study carried out in three hospitals in Rawalpindi. A total of n=38 technical and non-technical lab staff participated in the study. Laboratory practices were observed by the researchers themselves for a week. Then an education session was conducted for the staff regarding the World Health Organization (WHO) standard protocols for infection control. The post-intervention data were collected after about two weeks of health education sessions. Data were analyzed using SPSS version 22. Paired t-test was applied to compare the mean scores of pre- and post-interventional data.\u0000\u0000Results: The laboratory staff participated in the study mostly 19(50%) belonging to the age(years) range of 20-27. The percentage of respondents having good practice of infection prevention protocols before intervention was 7.5%, after the intervention this increased to 57.9%. Paired t-test showed that the difference in practice mean score of pre- and post-interventional data was statistically significant (CI=21.55-15.33 p-value=0.001).\u0000\u0000Conclusion: Pre-intervention data showed that the practice of the staff was not completely in accordance with WHO infection prevention.
