Unfortunately, these visual field defects are difficult for the patient to detect until more advanced stages and, as a result, early glaucoma is usually asymptomatic.2 \u0000This paper presents a case that is consistent with population studies that suggest that as many as half of people with glaucoma are unaware that they have the disease.3

Clinical features, an extended minimally symptomatic course and a temporal relationship to amiodarone use implicated amiodarone-associated optic neuropathy. Serial ancillary testing analyses provided insight into this entity’s natural course. This patient developed the greatest retinal nerve fiber layer thinning in the inferior quadrant; this may correlate with anatomically larger-diameter axons, supporting a previous publication which suggested that larger-diameter optic nerve axons are more susceptible to amiodarone-induced lipidosis. While rare, amiodarone-associated optic neuropathy may develop and cause permanent loss of visual function.

Hippocrates wrote of a blindness in the elderly that he called “glaykoseis.” Much later, the English ophthalmologist Richard Banister first discovered a correlation with high intraocular pressure (IOP). The invention of the ophthalmoscope by Hermann von Helmholtz in the mid-1800’s allowed the in vivo visualization of glaucomatous changes to the optic nerve for the first time, and in 1862 Franciscus Donders coined the term “Glaukoma simplex” to describe blindness resulting from elevated IOP. Shortly thereafter, the invention of the tonometer, the development of perimetry, and the use of cocaine as an anesthetic all further advanced the diagnosis of glaucoma.

Canada’s statistics are likely to mirror those of the U.S., where up to 40% of the population is affected by ocular allergies. The symptoms and signs of ocular allergies can greatly affect productivity and have a dramatic effect on overall quality of life (QoL). Over the years, many effective treatments have been developed for the management of ocular allergies. For allergic conjunctivitis, topical ophthalmic agents include antihistamines, mast-cell stabilizers, dual-activity agents, steroids, nonsteroidal anti-inflammatory drugs, and other immune-modulating drugs. Oral antihistamines are commonly chosen by patients for all forms of allergy, including allergic conjunctivitis. This review provides a summary of the forms of ocular allergy, with a particular focus on the symptoms and signs, diagnosis, current treatment options, and impact on QoL. More importantly, through multidisciplinary collaboration, a simplified treatment algorithm is proposed for Canadian clinical practice. This algorithm provides practitioners the best possible management strategies based on an individual patient presentation, thereby maximizing treatment efficacy and minimizing the effects on tasks of daily living and QoL.
