Tucker","doi":"10.15353/CJO.80.259","DOIUrl":"https://doi.org/10.15353/CJO.80.259","url":null,"abstract":"Polypoidal choroidal vasculopathy (PCV) is a disease of the choroidal vasculature that may result in sub-retinal hemorrhage and serous detachment of the retinal pigmented epithelium (RPE), leading to sub-retinal brosis and, sometimes, permanent vision loss. This report describes a case of PCV in an African-American female over the course of 1 year and demonstrates the progression of PCV, from being relatively asymptomatic to the development of a visually significant sub-retinal hemorrhage. Les statistiques du Canada sont probablement analogues à celles des États-Unis, pays où jusqu’à 40 % de la population souffre d’allergies oculaires. Les symptômes et les signes de ces allergies peuvent avoir un effet considérable sur la productivité et la qualité de vie en général. Au fil des ans, on a mis au point de nombreux traitements ef caces pour la prise en charge des allergies oculaires. Dans le cas de la conjonctivite allergique, les agents ophtalmiques topiques comprennent les antihistaminiques, les stabilisateurs mastocytaires, les agents à double activité, les stéroïdes, les anti-inflammatoires non stéroïdiens et d’autres médicaments à modulation immunitaire. Les patients choisissent souvent des antihistaminiques oraux pour toutes les formes d’allergie, y compris la conjonctivite allergique. Nous récapitulerons ici les formes d’allergies oculaires en mettant particulièrement l’accent sur les symptômes et les signes, le diagnostic, les options de traitement actuelles et l’incidence sur la qualité de vie. Plus important encore, nous proposerons, grâce à la collaboration multidisciplinaire, un algorithme de traitement simplifié pour la pratique clinique au Canada. Hippocrate a évoqué la c´écité des personnes âgées qu’il a appelée « glaykoseis ». Bien plus tard, un ophtalmologue anglais, Richard Banister,a été le premier à constater un rapport de corrélation avec une pression intraoculaire (PIO) élevée. L’invention de l’ophtalmoscope par Hermann von Helmholtz au milieu des années 1800 a permis une première visualisation in vivo des changements glaucomateux du nerf optique et, en 1862, Franciscus Donders a inventé le terme « Glaukoma simplex » pour décrire la cécité résultant d’une PIO élevée. On sait néanmoins que l’utilisation des écrans électroniques est aujourd’hui plus courante chez les enfants (à la maison comme à l’école), qu’elle commence plus tôt et s’effectue sur de plus longues durées. \u0000Les constatations sur la prévalence des symptômes liés aux écrans électroniques chez l’adulte et les lignes directrices correspondantes ne peuvent s’appliquer automatiquement aux enfants. En effet, les systèmes oculaire et physique de l’enfant diffèrent de ceux de l’adulte et sont en cours de développement. De plus, les enfants utilisent les écrans différemment et pour des tâches diverses. La présente question de principe examine la littérature disponible sur les symptômes oculaires et visuels liés à l’utilisation d’écrans électroniques chez l’enfant et propose des recommandations fondées sur des données probantes pour une utilisation sans risque.
