This paper reports its conceptual genesis and gradual diffusion, the dynamics of practitioners/healthcare insurer agreements in France, and medical liability. Irrespective of any incentive agreement, consideration of the advisability of de-prescribing has become mandatory in situations where iatrogenic risks are well established – particularly in the case of multi-medication treatments: as knowledge grows, failure to deprescribe (or to consider the advisability of doing so) could be considered a fault. However, de-prescribing is not a quantitative approach, due to its clinical purpose and consequences: it requires gathering scattered data, prioritizing treatments, planning, and sometimes arbitrating between therapeutic intentions. The last major reform of the European Union's pharmacovigilance system dates back to 2010. In France, the transposition of European texts took place against a backdrop of health scandals and a loss of patient trust in the healthcare system. More than ten years on, a review of pharmacovigilance activities highlights three areas of improvement during this period of time. Elhagary , Nashwa Ahmed , Emad Ahmed Abdelmooty","doi":"10.1016/j.meddro.2024.10.001","DOIUrl":"10.1016/j.meddro.2024.10.001","url":null,"abstract":"<div><h3>Background</h3><div>Medical errors (MEs) refers to the professional negligence or wrongdoing by healthcare providers that result in harm to patients, it can vary significantly across different specialities due to variations in patient populations, diagnostic challenges, treatment complexity, and procedural risks.</div></div><div><h3>Methods</h3><div>We conducted an electronic search of PubMed, Scopus, Web of Science, and Embase databases to identify all relevant published records till 11th August 2023. The analyses were carried out in R studio using the meta package. The mean difference (MD) was used to pool continuous outcomes, while the odds ratio (OR) was used to pool dichotomous outcomes, both with 95% confidence intervals (CI). We used the New Castle Ottawa (NOS) Scale for the observational studies.</div></div><div><h3>Results</h3><div>We included 63 studies encompassing a diverse range of specialities and involving 376,574 participants. The highest rates of MEs were found in neurosurgery, orthopaedic surgery, and cardiology. The lowest rates were seen in dramatology, vascular surgery, gastroenterology, endocrinology, and rheumatology. There is a huge gap in research investigations of MEs in Middle Eastern countries.</div></div><div><h3>Conclusion</h3><div>Medical malpractice is a complicated issue that varies by speciality within the healthcare profession. Two markers, namely the frequency of suicide occurrence and the level of use of toxic substances, allow us to present a comparison between mainland France and French Guiana. This territory is one of the five French overseas departments and presents many particularities (90% covered in primary forest, presence of original peoples, etc.). Thanks to the annual activity report of the Centre des dons du corps (CDC) des Alpes Maritimes, it is possible to obtain a snapshot of its activity in terms of data relating to registrations and admissions carried out over the year 2023. Analysis of the information collected in the donation consent form has made it possible to extract donors’ preferences with regard to the fate of their body after completion of the anatomical work.
