Hence the aim of this study was to estimate the stillbirth rate for Bhutan. \u0000Methods: A retrospective study was carried out for live and stillbirths recorded in delivery registers of all 253 health facilities across the country for a period of one year commencing 1st January till 31st December 2015. \u0000Results: There were a total of 11,126 live births and 108 stillbirths documented in delivery registers. The stillbirth rate from this data set was 10 per 1000 live births. \u0000 Conclusions: The stillbirth rate for Bhutan from this study is 10 per 1000 live births lower than 16 per 1000 live births estimated in Lancet Series 2015. In order to find the true burden of stillbirths in the country, a surveillance may be instituted which can facilitate the prevention efforts while at the same time enable to strengthen information system.

Measurement of HRQoL is an important medical outcome study and its study among older adults in Bhutan is limited. \u0000Methods: This is a cross-sectional study aimed to assess HRQoL and its sociodemographic and health correlates, among older adults in Bhutan. Data for this study was collected from the four major towns of Thimphu, Phuntsholing, Gelephu, and Samdrupjongkhar, Bhutan, from November 2014 - February 2015, using structured questionnaire with face-to-face interview. A total of 337 Bhutanese older adults participated in this study. Statistical analysis was performed using statistical package for social science version 21.0. \u0000Results: The overall mean score for the HRQoL among older adults in this study was 0.67 (SD: 0.13) significantly different between the gender (p-value<0.001). A significantly low scores inthe areas of role limitations (p<0.05), pain (p<0.01), mental health (p<0.001), and vitality (p<0.05) of the HRQoL was observed for the female gender. Frequent back pain (67.1%), memory decline (60.5%), depression (46.0%) mobility impairment (45.4%), insomnia (42.1%), and problem affecting breathing (31.8%) were common health problems and were significantly higher among the female gender. Better health conditions was positively related with better HRQoL (p-value<0.001). \u0000Conclusions: Low HRQoL was reported higher among female gender and was linked to multiple and cumulative health morbidities. Members of the family, community and healthcare providers could incorporate holistic approach to foster positive health outcomes and HRQoL of the older adults.

Providing free health service and education has the top priority from the beginning to this day. With humble starting from almost bare grounds, the situation of maternal health has improved. Until the start of the Safe Motherhood program in 1994, health programs were not streamlined. The maternal mortality ratio (MMR) was 380 per 100000 livebirths in 1994. A lot of efforts with different programs have been implemented by the reproductive health program of Ministry of Health to improve maternal health. Main challenge has been with health human resource from the very beginning and it still stands to this day as number one challenge. With the inputs of various programs to improve maternal health and development in non-health sectors, the MMR has come down to 89 per 100000 livebirths in 2017.

There were 190 deaths due to harmful use of alcohol in 2016 and alcohol liver disease (ALD) incidence was 46 per 10,000 people. Community action to reduce harmful use of alcohol in the communities is one of the strategies being implemented in Bhutan. However, the impact of such community action is not assessed in the country. Therefore, this pilot study was conducted in two gewogs (sub-district) of Trashiyangtse dzongkhag to assess the short-term effects of this community action to reduce alcohol use. \u0000Methods: This pilot study involved one intervention and one control community. The interventions were developed and implemented in consultation with the community by empowering them for monitoring to ensure effectiveness of the intervention. The data were collected pre and post-intervention in both the gewogs through repeated cross-sectional surveys. \u0000 Results: The practice of offering tshogchang decreased from about 99% to about 50% after the intervention. Practice of children drinking alcohol decreased by 6.83%. The average monthly spending on alcohol reduced from about Nu. 418 to about Nu. 97, and the amount of grains used for brewing decreased from about 34 Kg to 18 Kg, after the intervention. \u0000Conclusion: This study found that the community action is effective in bringing positive impacts in reducing harmful use of alcohol in community. Therefore, expansion of such community action in other communities in Bhutan with similiar context is strongly recommended..

Nursing documentation is one of the principal sources of information about patient care and an important tool for communication. This descriptive study assessed both quantitative completeness and quality of nursing documentation by major in-patient units of Jigme Dorji Wangchuck National Referral Hospital.\u0000Methods: This cross-sectional study used D-catch tool. Data of randomly selected 317 patient records from six major in-patient units were entered into EpiData file. Using STATA version IC/14, descriptive statistics and multi variable analysis were carried out.\u0000Results: Overall quantitative completeness (M-3.4, SD-.59) of the nursing documentation was higher compared to the quality of the documents maintained (M-2.8, SD-.79). The basic and less time-consuming information such as admission data and vital signs charting are documented better compared to the more time consuming and complex documentation such as nursing care process.\u0000Conclusion: Systems should not only be in place to enhance documentation quantitatively but also consider uplifting the quality of the documents maintained. Initiating centralized admission system in the hospital may reduce nurses’ burden of clerical documentation, which will allow them to focus on quality nursing documentation and overall nursing care of patients.

The various topical treatment used are ineffective and the benefits are mainly due to its placebo effect. A study on a series of twelve patients was carried out in a tertiary hospital in Bhutan to see the effectiveness of lignocaine-dexamethasone periauricular injection on treating tinnitus. To remove the bias in recording tinnitus by subjective response, we used a single patient as both case and control for the first time in such studies. The day (for La-dza, Sog-dza and Shey-dza), lunar date (for La-ney) and time within the day of surgery (for La-ney), along with Wang-thang (field of power) and Lung-ta (Horse of fortune) are important astrological recommendations. However, there are no information about the patients’ knowledge, attitude and practice on astrological recommendations for timing of surgery in an allopathic setting in Bhutan. The study was conducted to assess the knowledge, attitude and practice of considering Buddhist astrological recommendations among patients undergoing elective surgeries.\u0000Methods: A cross-sectional study with convenient sampling was conducted at the National Referral Hospital, Thimphu from April to May 2018. Data were collected using a pretested interviewer-administered questionnaire. Ethical clearance was obtained from the Research Ethics Board of Health, Bhutan.\u0000Results: From 420 participants (response rate 99.5%), 188 (44.8%) were aware that there are astrologically determined good timings to undergo elective surgeries. Only four persons (1.0%) were aware about La-ney. Among those who believed in such astrological recommendations, the attitude were more favourable towards the areas in which they had higher knowledge.\u0000Conclusion: The patients had very poor knowledge about the astrological recommendations to choose the timing for surgery. A well designed study on the effect of astrological recommendations on the surgical outcomes is recommended for generating evidence for further action.

Little is known about physicians’ job satisfaction. The study was done to assess level of job satisfaction amongst physicians in Bhutan and identify factors affecting it, thereby be able to better understand factors affecting physician retention.\u0000Methods: A national, cross-sectional study on job satisfaction of all Bhutanese physicians was conducted in 2016. Physicians were defined as satisfied if they answered yes to >50% of general survey questions about job satisfaction and and associations between demographic or job characteristics (e.g., married vs. unmarried, clinical vs. non- clinical) and job satisfaction were examined. Physicians were also queried about specific elements of their jobs (e.g., pay, working conditions) and explored associations between demographic or job characteristics and job elements.\u0000Results: Of 147 physicians who completed the job survey, 94 (64%) were classified as satisfied. There were significant differences in job satisfaction between married and unmarried physicians (72% vs 49%, p=0·01), specialists and generalists (73% vs 55%, p=0·04), nonclinical and clinical physicians (89% vs 61%, p=0·02), and physicians in referral and district hospitals (75% vs 48%, p <0·01). Across all demographic and job characteristics, salary satisfaction was low (11%). In multivariable analysis, non-clinicians had significantly greater satisfaction than clinicians with salary, annual leave, and work-family balance. Physicians in referral hospitals had significantly greater satisfaction than physicians in district facilities with work hours and working conditions.\u0000Conclusion: Survey findings suggest that, although job satisfaction appeared high, improved physician retention may require increased pay, opportunities for promotion to desired settings and job categories, and improved staffing and work conditions in district healthcare facilities.

As per the WHO, exclusive breastfeeding is considered the best nourishment for children in first six months. However, majority of the mothers fail to practice exclusive breastfeeding due to inadequate or lack of knowledge which poses a challenge. Thus, this study intends to assess knowledge, attitude and practices among breastfeeding mothers in Trongsa. \u0000Methods: A cross-sectional study was used. Data was collected from 205 breast feeding mothers in Trongsa District using the pre-tested structured questionnaire.\u0000Results: Majority (98%) of the mothers had an adequate knowledge on exclusive breastfeeding starting from early initiation, the feeding of colostrum and providing complementary feeding. Health staff and the national television were the main source of information for the mothers on exclusive breastfeeding. However, in terms of practice, 20% of them did not exclusively breastfeed and had given additional feeds along with breast milk with the assumption of insufficient mother’s milk. Similarly, 13.2% of them had squeezed and thrown away the colostrum.\u0000Conclusions: Despite mothers having good knowledge on exclusive breastfeeding, it was not practiced and followed.
