An exacerbations of chronic obstructive pulmonary disease (ECOPD) and Comminity-acquired pneumonia (CAP) are associated with high rates of hospitalizations, costs, and morbidity. Plasma levels of orosomucoid and Alpha-1 antitrypsin (AAT), also known as Alpha-1-acid glycoprotein (AGP), increase in response to inflammation and tissue necrosis. The purpose of this study is to evaluate the impact of markers in patients with ECOPD and COPD with pneumonia .\u0000Methods: To compare the levels of AAT, and AGP, between patients diagnosed with ECOPD only and patients diagnosed with COPD and pneumonia.\u0000Results: The study included 14 female and 22 male volunteers. The mean gender and age of groups 1 and 2 were similar. There was no statistical difference in laboratory values between the groups.\u0000Conclusion: Alpha-1-acid glycoprotein and AAT are acute-phase proteins elevated in various inflammatory conditions such as infections, trauma, and chronic diseases such as COPD. It is the first choice in the weaning process for patients with acute cardiogenic pulmonary edema, acute exacerbations of chronic obstructive pulmonary disease (COPD), and COPD patients with invasive mechanical ventilation. The choice of device, mode, and mask is important to the application and success of NIMV. In appropriate diagnosis and indication, NIMV should be the first choice in patients who are cooperative, able to protect their airways, clinically stable, and to whom the mask can be applied. Fifteen healthy individuals and 15 IPF patients. The demographic data and the measures from PFT, DLCO, laboratory tests of the patients included in the study were retrieved from IPF follow-up files. \u0000Results: The comparison of the IPF patient group with the group of healthy volunteers revealed significantly higher SAA values in IPF patients (p:0.005). A significant positive correlation was found between the patients' SAA and C-Reactive Protein (CRP) values. A negative significant correlation was found between the SAA values of the patients and the time to diagnosis (p<0.05). Despite the negative correlation between the SAA and FVC values of patients, no significant correlations were detected between these variables (p>0.05). This result suggests that SAA levels would be higher in newly or recently diagnosed . \u0000Conclusion: This study shows that SAA is significantly higher in IPF patients, suggesting that it will be a reliable biomarker feasible to predict the diagnosis. The study group consisted of people over the age of 18 who had been diagnosed with lung cancer and had not received any treatment for the tumor. The control group consisted of 45 people who had been diagnosed with any lung disease but did not have lung cancer, and 45 of them were completely healthy people. Those with benign lung disease apart from lung cancer were named as control group-1 and healthy control group was named as control-group-2. Plasma samples with 5 cc of EDTA were taken from all subjects and these samples were quickly frozen at – 80 degrees and stored in this way until laboratory analysis could be performed. Commercial kit based on the sandwich ELISA principle (ScheBo Biotech Ag, Tumor M2-PK ELISA kit, Germany) was used to determine plasma M2-PK levels. The M2-PK values of the samples were calculated on the basis of the standard graph.\u0000Results: M2-PK levels were measured and compared in the lung cancer group, control group-1 with non-lung cancer lung disease and healthy control group-2. M2-PK levels were found to be significantly higher in the lung cancer group than in the control group-1 (p<0.0001) and control-group-2 (p<0.0001). When M2-PK levels were compared in all three groups, they were statistically significant in the lung cancer group (p<0.001). In our study, the diagnostic cut off value was found to be 8.9 IU/ml using ROC curve. At this cut-off value, plasma m2-pk level was calculated as 100% sensitivity and 97.8% specificity in showing lung cancer. Spontaneous regression of thymoma is extremely rare. A 34-year-old female patient without any known disease, admitted by a myasthenic crisis, had a mass of 76 x 53 x 88 mm on thorax computed tomography (CT). The patient received intravenous immunoglobulin (IVIg) treatment for 5 days, pyridostigmine 60 mg/8 hours and prednisone 30 mg/day for the treatment of myasthenic crisis. Thorax CT on the 18th day showed that the mediastinal mass regressed to 63 x 30 x 63 mm. Patients with a history of active malignancy,\u0000hospitalization within the past 3 months, severe dementia precluding obtaining written consent, and patients under 65 years of age were excluded. Scores obtained from the Mini Nutritional Assessment-Short Form (MNA-SF) were recorded, with scores of 12 or higher considered as normal nutrition. Those with scores below 12 were reevaluated for malnutrition according to the Global Leadership Initiative on Malnutrition (GLIM) criteria, and those meeting any of the phenotypic and etiological criteria were considered malnourished. Malnutrition severity was reclassified according to the GLIM criteria. The 13-item OPQOLbrief questionnaire was used to evaluate their quality of life. The questionnaire was published in Turkish by Çalışkan et al. Each question was rated on a scale of 1-5, with “Strongly Disagree” scored as 1, “Disagree” as 2, “Undecided” as 3, “Agree” as 4, and “Strongly Agree” as 5.\u0000Results: The mean total MNA score was found to be statistically lower in patients classified as mild and severe malnutrition\u0000according to the GLIM classification compared to those without malnutrition (p<0.001). The rate of MNA scores between 12\u0000and 14 in patients without malnutrition was higher than that of patients with mild and severe malnutrition, while the rate\u0000of scores between 8 and 11 was higher in patients with severe malnutrition. The rate of scores between 0 and 7 was higher\u0000in patients with mild and severe malnutrition (p<0.001). The mean total MNA (p<0.001) and QOL (p=0.021) scores were\u0000statistically lower in patients with malnutrition compared to those without malnutrition. Ogan, Ceren Ilgar","doi":"10.51271/jopic-0016","DOIUrl":"https://doi.org/10.51271/jopic-0016","url":null,"abstract":"Chronic Thromboembolic Pulmonary Hypertension (CTEPH), defined as precapillary pulmonary hypertension (PH) with right heart catheterization and imaging consistent with chronic thromboembolism, is a long-term complication of Pulmonary Embolism (PE). CTEPH defines symptomatic patients with maladaptive perfusion disorders after at least 3 months of therapeutic anticoagulation. In this setting, pulmonary hypertension results from extensive Pulmonary Artery (PA) obstruction by only organized fibrotic clots and may also be present as micro vasculopathy. Echocardiographic (ECHO) examination to determine a pulmonary surgical operation if the patient complains of anticipation and/or the risk for CTEPH is widely demonstrated (prior PE episodes, existing imaging size as index event suggestive of CTEPH, key features such as hypothyroidism and cancer), It is the preferred recognition method because of its availability and the high data value and amount of data. Patients diagnosed with CTEPH or with a strong suspicion of CTEPH should be referred to a specialist pulmonary cardiovascular clinic for further multidisciplinary diagnosis and treatment, such as Pulmonary Endarterectomy (PEA), balloon Pulmonary Angioplasty (BPA), and/or Targeted pulmonary medical therapy. In recent years, significant progress has been made in evaluating patients with sequelae after a pulmonary embolism, confirming clinical, biochemical, and hemodynamic criteria.
