The year 2025 will mark 10 years since the adoption of the WHO Global Action Plan against AMR [2], and despite the progress made to date, there is still a need to initiate concrete and effective actions to eradicate this phenomenon, which is still too little widespread in the collective consciousness: it is no coincidence that some experts label it with the name \"silent tsunami\" [3], while others speak of a \"neglected pandemic\" [4], and already this is a message that should not be overlooked, and a more than valid reason to engage in the fight against AMR. In this regard, how to raise awareness, and what contribution can be expected from civic organizations and patients' advocacy groups (PAGs) in support of initiatives – both institutional and non-institutional – to counter AMR? This editorial dilemma is form of bureaucracy that editors of journals make in front of science publishing. The editors will ask to change the article many times, until the article is not the same idea that the author submitted in the 1st time. This dilemma as well characterized by delaying of articles publishing which will take 6 months to 1 year to publish one article and maybe even more time. The formatting of medical journals are different and each journal ask for different styles of formatting and citation. Most of these journals request publishing fees, open source fees, archiving the article “indexed” on PubMed fees, etc. These obstacles put forward by the editor are restrictions against science which had led to form the research centers publishing groups which is “a formal malpractice”. This practice where endless list of authors, list their names on the paper since publishing needs a team effort and funding for publishing of only one paper. The editor can jump in the peer-reviewing process to become a reviewer then reject the paper or ask for changes which lead to create “preprint online depositories” to avoid the editor dilemma. The editors shape what type of science should be published or not. They determine what science is? And what is not! The editorial dilemma is even chasing the online preprint depositories by forcing them to remove papers from their websites. {"title":"Imaging of Mitral Valve Area by Cardiac Magnetic Resonance in Patients with Rheumatic Mitral Stenosis","authors":"A. bedier, A. Rheumatic mitral stenosis (MS) is the commonest delayed valvular affection as a consequence of rheumatic fever. The assessment of MS severity by measuring mitral valve area (MVA) is very essential for patient management. Different imaging modalities are available for MVA assessment including echocardiography and cardiovascular magnetic resonance (CMR).Objectives: The aim of this study is to compare the planimetric MVA between CMR and two dimensional echocardiography in MS patients.Patients and methods: A forty adult patients with symptomatic mitral stenosis were included in the study. Other significant valvular lesions, atrial fibrillation, poor echocardiographic window, contraindications to CMR, and NYHA IV were excluded from the study. All patients were assessed by 2D echocardiography and CMR for MVA measurement.Results: the mean 2D TTE MVA was 1.2±0.26 cm2, while the mean CMR MVA 1.2±0.28 cm2. No significant statistical difference was found between both methods (P value 0.842) with a very strong correlation between both methods (r = 0.93 and p-value < 0.0001).
