It accounts for almost 4% of the injuries seen in the emergency department. Despite its high incidence and the substantial possible implications of suboptimal management, no high-level evidence regarding the best treatment method yet exists. The study aimed to compare the functional outcomes of extra articular distal radius fractures managed operatively with those managed conservatively. In this prospective cohort study conducted between November 2018 to September 2020, 18 to 65 years old patients with displaced extraarticular distal radial fracture were treated surgically or conservatively. DASH and Modified Mayo Wrist Score were calculated after a follow up of one year.: At the end of 12 months the patients were evaluated, and it was noticed that patients who were treated operatively had significantly better functional and clinical outcomes, as indicated by significantly higher Mayo scores than patients treated conservatively by casting (all p values < 0.05). The DASH scores were lower in the patients who had undergone operative management. Patients treated with volar plate fixation were able to resume activities of daily living few weeks earlier compared to those managed with K-wire fixation and conservative group. Patients are more likely to be fully aware and averse of their underlying health condition. This makes it paramount to have quality and easily comprehensible information available to general public. The goal of this study is to evaluate the readability and quality of information available on the Internet relating to Lisfranc complex injuries.To identify potential web sites, five most common internet search engines, Google, Bing, Yahoo, Yandex and Duck Duck go were used and a search for \"Lisfranc complex injuries\" and “Lisfranc fractures” was performed on each. 98 web sites were reviewed. Quality of information was assessed by DISCERN tool, Journal of American Medical Association benchmark criteria, JAMA and Health on the Net code (HON) while readability was analysed with Flesch Reading Ease score (FRES), Flesch Kincaid Grade level (FKG) and Gunning Fog Index (GFI) scores.After removing duplicates, 98 web sites were identified and analysed. Only 17% of the websites were Health on the Net Code Certified. HON-code certified websites demonstrated higher quality information than those without the code. They also achieved significantly greater JAMA scores. 97% of the websites were above the recommended 6 grade readability level. Flesch readability ease score of more than 80 websites (81%) was below 60 which makes them fairly difficult to read by public.Like other orthopaedic problemsonline information on Lisfranc injuries is of dubious quality. Most of the available information is either set at an academic level standard that is too difficult to comprehend by an average layman person or its too deficient. Attempts at realignment via manipulation under anaesthesia (MUA) were unsuccessful, so the patient underwent an open reduction and the application of an external fixator to stabilise the knee. Knee dislocations are characterised by a high rate of neurovascular injuries, therefore special emphasis should be made on the prompt diagnosis and treatment of such injuries. They are the third most common fractures in the elderly population after hip and distal radius fractures. Regarding the treatment of proximal humerus fractures, controversies still exist whether to do conservative or operative management. Various operative procedures are carried out. A recent trend in internal fixation has moved on to locking plates. The present study is undertaken to evaluate the functional outcome and complication of proximal humerus fractures treated by Open Reduction Internal fixation using proximal humerus Interlocking plate.A prospective study involving thirty patients who were above18yrs of age with proximal humerus fractures admitted to our institution were treated by open reduction, and internal fixation with locking plate were evaluated. In this study, the majority of the patients were males, elderly aged, withslip and fall being the most frequent mode of Injury, resulting in 2 part and 3 partfractures of the proximal humerus. Accuracy of fracture reduction is the strongest predictor of clinical outcome in acetabular fixation surgery.Twenty four patients with posteriorly based acetabular fracture were treated with open reduction and internal fixation using Kocher- Langenbeck incision. In our series, there were 11 posterior wall, 3 posterior column, 5 transverse, 2 posterior column and posterior wall and 3 transverse and posterior wall fractures. All the patients were in the age group of 30 to 45 years (mean age 38.5 years). Males dominated our series (n=18 i.e. 75%) and right side was more commonly involved in both the sexes (n=14 i.e. 58.33%). Road traffic accident was the leading cause (n=19 i.e. 79.16%). All the patients were operated achieving adequate stability and with utmost soft tissue care. Follow up radiograph were graded according to criteria developed by Matta J et al. A minimum follow up was two years. Patient with accurate reconstruction (n =21 i.e. 87.50%) had good or excellent functional outcome while two patients (08.33%) with inaccurate reduction and one patient (04.16%) with poor reduction had fair and poor outcome respectively. It constitutes about 60% of all the fractures about the Elbow in children. The aim of the present study is to evaluate the results of percutaneous pinning of unstable or irreducible Type II and III Supracondylar Humeral fractures in children. A prospective study was conducted on 30 children, taking into consideration age, gender, affected side, mode of injury, type of fracture, Gartland classification of fractures, time duration between injury and surgery, postoperative complications and radiographic and clinic variables.: The median loss of motion was 5 degrees and the median change in carrying angle was 4 degrees. All patients had satisfactory results in terms of Loss of Carrying angle, Limitation of Elbow flexion and Flynn’s grading. There were 19 patients that had excellent ratings, 9 patients had good ratings and lastly only 2 patients had fair Flynn’s ratings. There was only 01 patient suffering from neurological deficit and only 01 patient from pin track infection. In this study 10 patients of scapular fracture (neck, glenoid and body) were treated with locking plate at Dr. Hedgewar hospital, Aurangabad from January 2014 to December 2020 with age from 18 to 60 years. Functional outcome was evaluated by Constant-Murley scoring system. Total 10 patients were included in present study. Mean age of the patients was 44.9±11.06 years. Male patients (80%) were common than female patients (20%). Road traffic accident (80%) was most common mode of injury. In fracture type distribution, neck and body of scapula (90%) fracture was most common. Assessment of shoulder function was done using Constant and Murley scoring system. Good function was noted in 80% patients, followed by excellent function in 10% patients and fair function was noted in 10%. In patients with severe slipped capital femoral epiphysis, the modified Dunn method allows for the restoration of proximal femoral morphology and proper hip function (SCFE). The purpose of this article is to outline the treatment management of unstable bilateral slipped capital femoral epiphysis in an epileptic patient.A 9-year-old female, a known case of grandmal epilepsy, arrived in the outpatient department with a severe SCFE. The patient underwent modified Dunn osteotomy and fixation of the epiphysio-metaphysis junction with CC screw fixation. Since there is very little awareness regarding slipped capital femoral epiphysis in an epileptic patient, this case report is very rare.One should be vigilant about children suffering from uncontrolled epileptic attacks. Osteochondral fractures require an anterograde approach for treatment. Retrograde drilling and packing with a bone graft can treat lesions with intact cartilage. Because subchondral lesions involve the posterior aspect of the talar dome, these lesions can be difficult to localize with an image intensifier.We describe a method in which ankle arthroscopy is used to visualize a subchondral lesion, along with an ACL jig to target the lesion precisely.: We performed retrograde drilling in two patients: a 29-year-old man and a 34-year-old man. Each patient presented with a subchondral cystic lesion in the posteromedial aspect of the talus. We treated both patients by modifying the standard technique for retrograde drilling. Patients were evaluated before and after surgery using the AOFAS score and X-rays of the ankle joint. The X-rays taken at the 3-month follow-up showed a well-maintained contour of the talar dome with complete incorporation of the bone graft. The AOFAS scores for both patients were greater than 90, excellent per the Saxena and Eakin criteria. The purpose of this study is to quantify screw-plate angle. In this study, a total of 750 locking screws of different lengths were placed by six residents. The measured angles were compared with Repeated Measures ANOVA method. Significant differences were seen for 40mm, 50mm, 70mm and 80mm screw length (p < 0.05) to assess the deviation of angle in pre-training, post-training and final experiment. The placement of locking screws precisely can help avoid an easily preventable surgical risk factor for fixation failure. The evolution of Locking Compression Plate (LCP) technology has radically changed the practice of fracture fixation. This technology mandates meticulous detail in surgical technique in placing the locking screw. It demands perfect seating of the locking screw head in the corresponding recess in the plate hole. The purpose of this study is to quantify screw-plate angle i.e., off-axis screw trajectory and its change with an increase in the length of screw and strength of the surgeons.Six orthopaedic residents were selected. The surgeons were familiarized with the instrumentation. A total of 750 locking screws of different lengths were placed in a locking plate in osteoporotic saw bone models using a torque measuring screw driver. The deviation was assessed for 150 screws inserted in pre-training, post-training and the final test under the supervision of the senior author. Immediate feedback on the performance and objective proof of precision was given within an hour by measuring the angle of deviation on X-ray using InstaPACS.Collected data was entered in MS Excel sheet. Descriptive statistics were presented in mean ±SD for continuous variables and count with percentage for Categorical variables. Repeated Measures ANOVA was used to assess the difference in deviation of angle pre-training, post-training and final experiment with the length of the screw. IBM SPSS 25.0 software was used for data analysis. Surgeon grip strength and torque applied had no significant association with the angle of deviation. The surgeons exceeded 10 Newton meter torque in pretraining. This applied torque decreased after training in the post-training and final experiments. The length of the screw and angle of deviation were found to positively correlate in pre-training and final experiment. In this study, 40 out of 150 (26%) studied screw insertions were off-axis. Most of the deviations (29) were only one degree (19.33%). The remaining 11 were more than 2 degrees (6.67%). The maximum off-axis was 4.3 degrees. Significant differences were seen for 40mm, 50mm, 70mm and 80mm screw length (p < 0.05) in pre-training, post-training and the final experiment. : This study signifies that practice under supervision with immediate and objective feedback is a valuable learning tool.
