A prospective review of the medical records of 32 cases who underwent surgery at Dayanand Medical college & Hospital, Ludhiana was performed. Patients were selected with proximal femur fracture having cardiac diseases after fulfilling the inclusion and exclusion criteria. After taking detailed history and examination, the patients were followed up after 4-6 weeks and 3-4 months. A detailed structured performa was filled and functional assessment was done using HHS. We had final follow up of all 32 patients which were included in the study managed by PFN. Modified HHS was accessed at 4-6 weeks and 3-4 months and found to be 41.19 (SD =12.89) & 64.66 (SD =13.35) respectively. No mortality was observed during the follow up. Presence of cardiac comorbidities increased the chances of hip fracture and poor HHS was observed among the patients in the study. Although no significant difference on HHS was observed among the subtypes of cardiac diseases. Most common cause of delay in surgery was the use of antiplatelet agents and delay in surgery affected the HHS significantly in a way that more the delay, poor was the HHS. Presence of comorbidities like hypertension, diabetes mellitus decreased the mean HHS. Occurrence of complications also affected the HHS. More the complications, poor was the HHS. Patients with hip fracture with a diagnosis of CVD, have greater risk of hip fracture with longer length of hospital stay and hence higher cost per hospital stay. Presence of more comorbidities along with CVD shows a significant association in the functional outcome.","PeriodicalId":407871,"journal":{"name":"Indian Journal of Orthopaedics Surgery","volume":"323 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124570140","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We present a case report of a 7-year-old male who presented with a solitary lateral ankle mass associated with occasional pain and altered gait pattern for a duration of 18 months after being evaluated in other hospitals, without much success. The patient was successfully treated with open surgical excision, using an anterolateral approach to. Pathologic evaluation reported a benign osteochondroma of talus, and the patient subsequently had routine healing of the postoperative incision site and return to full function without pain or disability at 6-week follow-up. This case study adds to the current understanding, incidence, occurrence, and treatment of rare osteochondromas occurring in the talus causing gait disturbance which has to be identified and treated with surgical excision which in the literature has the lowest recurrence rate of less than 2% following the treatment.","PeriodicalId":407871,"journal":{"name":"Indian Journal of Orthopaedics Surgery","volume":"21 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114609035","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Out of 140 participants, 78 males and 62 females with mean age 52.22 (±11.97) years; 78 underwent PLF and 62 underwent TLIF. Assessment was done using VAS score and ODI score before surgery and post surgery at 3, 6, and 18 weeks and thereafter at 18, 24 and 36 months. Radiologically sagittal and coronal angles were measured both preoperatively and postoperatively and evaluation of correction in sagittal and coronal angle was calculated.We observed highly significant reduction in the post-operative VAS score, ODI Score compared to pre-operative scores in both TLIF & PLF group but radiologically TLIF gives better correction in sagittal balance and rotational alignment compared to PLF. We conclude that in the short term duration of our study, both the procedures done with proper technique in duly indicated patients shows satisfactory clinical outcome . However, radiologically TLIF patients had better outcome. We expect better outcome in long term with TLIF compared to PLF. In presence of insignificant blood loss, surgical duration and better 360° fusion TLIF is preferred over PLF.","PeriodicalId":407871,"journal":{"name":"Indian Journal of Orthopaedics Surgery","volume":"25 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115105627","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Because one-third of the tibia is subcutaneous throughout most of its length and its location, it is more prone to open fractures.Distal tibia fractures (mostly due to RTA, sports injuries) include extra-articular fracture of the metaphysis and intra-articular pilon fractures. Treatment depends on the closeness to the tibial plafond, displacement of the fracture, comminution, and injury to the soft tissue envelope.The goal of treatment is to obtain a healed, well-aligned fracture; pain-free weight-bearing; and functional range of motion of the knee and ankle. 17 patients of the 18–60-year age group with open distal tibia fractures (without vascular injury) of less than 3 weeks old of trauma were included in the prospective study of 1 year period (1 June 2019 to 31 May 2020). All cases were treated with the Hybrid external fixator (HEF).The mean duration of surgery was 67.6 minutes. All fractures united with a mean time of 16.5 weeks. The mean AOFAS score at 6 months was 84.59. A good-excellent functional outcome was seen in 88.25% of the cases. There was 1 case of valgus deformity and 4 cases of pin-tract infections. HEF is effective in the treatment of open distal tibia fractures as it provides stable fracture fixation, early joint motion, and weight-bearing with minimal complications without jeopardizing the status of soft tissue condition.","PeriodicalId":407871,"journal":{"name":"Indian Journal of Orthopaedics Surgery","volume":"16 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116883860","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Obesity has been linked to neck pain, but the prevalence of neck discomfort in MetS patients has not been explored. The goal of this study was to look at the link between MetS and neck discomfort.A total of 1,000 middle-aged Indian individuals were included in the study. A total of 450 men and 550 women took part in the competition. Males and females were both 47 years old on average. Clinical and biochemical tests were performed. A standard questionnaire was completed by the participants. The 12-item General Health Questionnaire was used to measure psychological discomfort (GHQ-12). Neck discomfort was described as neck pain that was experienced on a daily basis. The National Cholesterol Education Program (NCEP) criteria were used to define MetS. A bootstrap-type t-test or the Chi-Square test were used to make statistical comparisons between the groups. Generalized linear models using age, smoking, alcohol consumption, exercise, and GHQ-12 score as factors were used to construct risk ratios for neck discomfort.MetS was found in 455 men and 550 women in the study. Neck discomfort was linked to BMI and waist circumference in males but not in females. Antihypertensive medicine was used more frequently by neck pain patients than by non-neck pain patients. There were no significant changes in smoking, physical activity, blood pressure, lipids, glucose, or CRP levels. Neck pain was reported by 8.7% (95 percent CI, 4.8 percent to 13%) of male patients without MetS and 17%.","PeriodicalId":407871,"journal":{"name":"Indian Journal of Orthopaedics Surgery","volume":"6 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128771450","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Several factors determine the outcome of intramedullary nail including working length of the nail. Addition of subtrochanteric locking screw to a reconstruction nail will reduce the working length providing biomechanical advantage.177 adult patients treated with reconstruction nail with a subtrochanteric locking screw for intertrochanteric fracture of the femur in our institution from January 2011 to December 2020 were prospectively analyzed. Final outcome is assessed by radiological union after a followup period of minimum 6 months postoperatively. 3 patients had varus malreduction, 4 patients had screw backout and one patient had screw migration. We didn’t encounter any wound complications in our study. Complication rates were much lesser when compared with studies done on standard proximal femoral nail.Reconstruction nail with a subtrochanteric locking screw reduces the working length of the nail and improves the outcome in intertrochanteric fractures.","PeriodicalId":407871,"journal":{"name":"Indian Journal of Orthopaedics Surgery","volume":"16 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"129943558","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Surgical methods to the knee joint and its surrounding structures vary, but they are all designed to provide the best access to a pathological area while protecting vital surrounding tissues. A perfectly aligned knee is required for installing a rotating platform device, which MPA can easily create. The subvastus method, on the other hand, allows for a perfect lateral exposure and a completely positioned knee.Currently, the medial patellar approach is favoured for Total Knee Replacement, however our theory suggests that the lateral corner can be better viewed with the subvastus approach, making it a superior option for Rotating Platform Device insertion in varus, valgus, and stiff knee situations. To determine the outcome of subvastus approach in all types of knee pathologies using mobile bearing rotating platform implant design. We conducted prospective analysis of patients with limited preoperative ROM (flexion ≤90°) of the knee who underwent TKA using subvastus approach and presenting the 6 months results. There were a total 60 patients (60 knees) with mean age 63 (range 49–79 years) years. The mean preoperative flexion was 44°. Postoperatively knee flexion improved by mean 95° (P < 0.05) which was significant as assed by paired t-test. The mean knee society score improved from 34 (range 20–60) preoperative to 78 (range 70–90) postoperatively (P < 0.05). Our findings show that employing the subvastus method, gratifying TKA results can be achieved in varus, valgus, and stiff knee cases with restricted preoperative ROM while maintaining the benefits of early mobilisation, reduced post-operative discomfort, and patient satisfaction.","PeriodicalId":407871,"journal":{"name":"Indian Journal of Orthopaedics Surgery","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130500904","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Our objectives are to investigate the short-term functional outcomes of an anatomic single-bundle reconstruction using the modified I.D.E.A.L technique of femoral tunnel placement, with 1-year follow-up.A retrospective observational study of 120 patients who developed ACL rupture and underwent anatomic single-bundle reconstruction in TUY MALIK private hospital/ Sulaymaniyah city/Iraq from March 2020 till March 2022. The functional outcomes of the procedure were assessed using the subjective knee evaluation scores (IKDC, Lysholm, and Tegner scores) preoperative and 1-year postoperatively. The mean preoperative IKDC score was 50.96 (40.22- 60.91), and 1-year postoperative score was 83.81 (72.41-95.40), with a (p-value 0.000), whereas the mean Lysholm score was 51.17 (40-67) and 86.19 (75-95) preoperative and postoperative, respectively, with a (p-value 0.000). The mean Tegner score was 7.59 preinjury, while 3.63 before the operation and 7.18 after the operation with (p-value 0.000), representing a statistically significant enhancement in functional outcomes between preoperative and final clinical follow-up assessment.Reconstructing ACL anatomically by placing the graft in the modified I.D.E.A.L position within the native femoral footprint can reestablish near-normal knee biomechanics, obtain unrestricted knee range of motion, and successful return to preinjury activity level.","PeriodicalId":407871,"journal":{"name":"Indian Journal of Orthopaedics Surgery","volume":"2006 319","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114111860","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
