Standard wound dressing required prolonged period, repeated debridement, trauma to granulation tissue and had poor patient compliance. Vacuum-assisted closure provides an extremely efficacious method for treating difficult wounds. This research aimed to study whether VAC gives better outcome in open fractures after primary intervention, whether it leads to faster and more effective wound healing and whether it leads to shorter length of stay in hospital. An interventional study was conducted, studying 30 cases of open tibia fractures who were fitting in the inclusion criteria. It followed Gustillo- Anderson classification to classify cases and then mapped out various conclusions on the aim of study whilst also charting the effect of the treatment in different circumstances, demographics and like. Amongst the 30 cases studied,all patients were evaluated clinically after the primary fixation and following VAC application, for an average period of follow up of 12 months. Majority of patients required 4-5 VAC dressings. The mean decrease in wound size was 9.97 cm [21.22%]. Out of 30 patients, 4 patients had excellent, 16 patients had good, 8 patients had fair and 2 patients had poor result.In this study, 30 patients were included with open fractures of tibia after primary internal fixation with VAC application. The greatest advantage of VAC was found to facilitate rapid formation of granulation tissue on wounds with exposed tissue and implants hence shorten healing time and minimize secondary soft tissue defect coverage procedures. It occurs more commonly in the third decade of life and is slightly more common in females. The knee joint i.e. the distal femur as well as proximal tibia account for most number cases followed by the distal end of radius. The occurrence in foot is rare and usually occurs in the younger age group. The progression in foot is usually accelerated and is masked by vague symptoms and boring type of foot pain. Malignant transformation is rare. In our case the 38 years female patient had GCT of the first metatarsal which later progressed to a secondary malignancy involving the ipsilateral inguinal and femoral lymph nodes. This was later managed by performing a Syme’s amputation along with the involved lymph node dissection. Surgical treatment of these fractures poses a challenge to Orthopaedic surgeons. The aim of this study was to evaluate the efficacy, safety and outcomes of the fixation technique by using IRBP for acetabular fractures with quadrilateral plate involvement via single modified Stoppa approach. A total of 30 patients with acetabular quadrilateral plate fracture, selected between January 2016 to June 2019, were managed by IRBP via modified Stoppa approach. This was a prospective study and all patients were followed up to a minimum of 2 years. The primary outcome measures were reduction quality and functional outcome which were evaluated according to Matta’s radiological criteria and modified Merle d’Aubigné and Postel score respectively. Secondary outcomes were intra-operative conditions such as operative time and blood loss and post-operative complications.Anatomic reduction was obtained in 22 (73.3%), Imperfect in 6 (20%), and Poor in 2 (6.7%) cases. The modified Merle d’Aubigné and Postel score outcomes were Excellent to Good in 25 (83.3%) patients, Fair in 3 (10%), and Poor in 2 (6.7%) with the mean score being 16.07 ± 2.68. 21 (70%) patients did not have any complications. Tears of rotator cuff arebecoming increasingly prevalent in today’s population. Shoulderarthroscopy has become a reliable and reproducible method of treatingrotator cuff tears. Arthroscopic surgery allows for a shorter recovery timeand predictably less pain in first few days following procedure than doesany open surgery. : A prospective and retrospective review of themedical records of 37 cases of rotator cuff tear who underwentarthroscopic rotator cuff repair between January 2017 to May 2020 atDayanand Medical college Hospital, Ludhiana was performed.In our study, we had measured UCLA shoulder score over 6months period in 37 patients. Our study showed improved functionaloutcome from UCLA score at presentation- 10.27 to UCLA at 6 months-30.41 with a p value <0.001 which is graded as a good result. There wasno significant gender difference in terms of incidence and postoperativefunctional outcome. Maximum incidence was seen in age group- 51-60years with etiological inclination towards precedent trauma. No statisticaldifference between functional outcome of single and double row techniquewas noted.Arthroscopic repair of rotator cuff tear offers excellent functional outcome; with minimal morbidity. Once ignored, late reduction is difficult and requires extensive dissection. Lisfranc joint injuries are known to result in unctional loss and chronic pain due to residual ligamentous instability, deformity, and/or arthritis; osteoporosis may also occur due to antalgic gait without weight bearing.This recognition is important, as most of the injuries are either misdiagnosed or overlooked, such as in patients suffering from polytraumatic injuries, possibly becoming a permanent source of pain after the major fractures have healed.A prospective study to be conducted at the Department of Orthopaedic Surgery, Vijayanagar Institute of Medical Sciences, Ballari from 2018 to 2020. This study consists of 30 cases of Lisfranc’s Fracture Dislocation treated operatively by Cannulated Cancellous Screws and K wires (Kirschner Wires). The cases were selected according to inclusion and exclusion criteria. The functional outcome was evaluated using AOFAS score.In our study, we achieved Excellent in 10% of the cases. Good outcome in 73.33% of cases, Fair outcome in 13.3% of the cases and no patient had poor outcome in our study. It has been a gruelling job for the orthopaedic surgeons to deal with the delayed union after definitive fixation due to the lack of patient compliance for reoperation like classical bone grafting technique for delayed union and non-union, unfavourable fracture site morphology for external compression, post-operative surgical wound issues etc., The percutaneous autologous bone marrow injection is a reasonable choice in this scenario for the treatment of Delayed union of long bone fractures after definitive fixation. The study was performed in the department of orthopaedics Assam Medical College and Hospital, Dibrugarh, Assam, India for a period of 2 years from July 2019 to June 2021. A total of 23 patients were taken with delayed union of long bone fractures after definitive fixation. These 23 patients were treated with percutaneous autologous bone marrow injection and followed up for 6 months. The results were evaluated on the basis of clinico-radiological criteria of union, and found to be excellent in 60.86% (14/23), good in 17.39% (4/23) and poor in 21.73% (5/23) cases. The percutaneous autologous bone marrow injection provides cellular stimulation and promotes fracture healing without the necessity for opening the fracture site. So In our study, we have put forth an algorithm to categorise the patients for suitable operative management. This was prospective study of 30 patients. Patients with age 50 and above and ones who had no relief/worsening of symptoms after 6 months of conservative management were included. Patients were categorised into 3 groups. Patients were either treated with Focal Decompression / Decompression and long instrumented fusion with correction of deformity /Decompression only. MODI and SF36 scores were compared at the end of 2 years.11 patients were categorised into group A, 15 into group B and, 4 patients who had significant co-morbidities and had very high risk for surgery were categorised into group 3. There was significant (p <.001) improvement in average MODI scores in both group A and group B at the end of 2 years. Improvement was seen in average MODI scores of patients in group 3, but it was not statistically significant. SF-36 score showed significant improvement in Group A and group B at the end of 2 years (p <.001). Patients from group 3 also showed improvement in average scores, but that was not significant.Not every case needs long stabilization, focal decompression only also gives satisfactory outcome in properly selected patients. The purpose of the present study is the analysis of functional outcome of the muscle pedicle bone graft porcedure in ununited fracture neck of femur in children and adolescents.: Sixteen patients with ununited fracture neck of femur (presenting late) in children and adolescents were treated with quadratus femoris based muscle pedicle bone graft along with open reduction and internal fixation. Mean age of the patient was 14.25 year (range 12-16 years). Boys dominated our series (n= 10 i.e.62.50%). Road traffic accident was the leading cause. Average injury –surgery interval was 5.88 week (range 3-9 week). A minimum follow up was two years. There were 2 Transepiphyseal, 11 Transcervical and 3 Cervicotrochanteric fractures. Most of the cases showed union in 16 to 24 weeks. In one case there was implant failure. : The result was analysed using Harris Hip Score. We had 11 (68.75%) excellent, 2 (12.50%) Good, 2(12.50%) Fair and one (06.25%) Poor results in our series.Quadratus femoris based Muscle pedicle bone with internal fixation is a very effective method to address the difficult problem of non-union fracture neck of femur. They account for around 1-2% of dislocations. We present to you, a few cases of subtalar joint dislocation managed initially by closed reduction, followed by open reduction under anesthesia, with excellent functional outcome. Close to fifty percent of patients suffering from complicated injury, are frequently at risk of developing complications. Avascular necrosis of the talus is the most common complication, following dislocation. Other long-term sequelae include osteochondral fracture and subtalar joint instability.
