Various molecular studies and prognostic biomarkers are helpful to assess the outcome of urinary bladder carcinoma. High expression of p53 and Ki-67 represents the aggressiveness of the disease and indicates poor prognosis.\u0000\u0000Methods: This cross-sectional study was conducted at the Department of Pathology, Sylhet MAG Osmani Medical College from September 2018 to July 2020 to observe the immunoexpression of p53 and Ki-67 in urinary bladder carcinoma and their association with tumor grade and muscle-invasiveness. Total 57 transurethral resection (TURBT) samples of radiologically and clinically suspected bladder tumor cases were diagnosed histopathologically included in the study.\u0000\u0000Results: Age of the study patients ranged from 35 to 80 years with a mean of 60.60 ± 11.14 and male to female ratio was 4.7:1. There were 31 (54.4%) high grade and 26 (45.6%) low grade urinary bladder carcinomas on histopathological examination. Thirty (52.6%) cases were histopathologically muscle invasive bladder carcinoma (MIBC) and 27 (47.4%) cases were non-muscle invasive bladder carcinoma (NMIBC). Positive p53 expression was observed in 24 (77.4%) cases of high grade carcinoma while only in 6 (23.1%) cases of low grade urinary bladder carcinoma. Positive p53 expression was seen in 22 (73.3%) cases of MIBC and 8 (29.6%) cases of NMIBC. Ki-67 expression was found positive in 27 (87.1%) cases of high grade carcinoma while only in 7 (26.9%) cases of low grade urinary bladder carcinoma. Positive Ki-67 expression was seen in 25 (83.3%) cases of MIBC and 9 (33.3%) cases of NMIBC. There is a growing concern about stress among medical students and its effects. Although each year of medical students is characterized by unique stress, the students in their first year are more prone to stressful situations. The reasons include academic pressure, unrealistic expectations and unfamiliarity in a new environment. These stresses are badly affecting their physical and mental health, which also affects their academic performance.\u0000\u0000Methods: This was a cross-sectional study over four weeks in the year 2022, conducted at different medical colleges in Sylhet. The sample size was 286 first-year MBBS students in different medical colleges. The sampling technique was a non-probability convenient type. Data were collected by using the pretested Medical Students Stressor Questionnaire (MSSQ) and examined using SPSS, version 22.\u0000\u0000Results: A total of 286 medical students were recruited, with age range of 19-21years; among them, 90 were male and 196 were female. The maximum number of students found to have moderate stress (185, 64.7%) followed by much stress (80, 28.0%).There were few students with no stress (0.3%), extreme stress (1.0%), and mild stress (5.9%) found in this study. The unpaired t-test showed significantly higher (p = <0.001) stress among females (3.82±0.49) than males (3.52±0.49).\u0000\u0000Conclusion: It can be concluded that the vast majority of students experienced moderate stress, and analysis highlighted a greater association with academic factors. Renal magnesium wasting can be made worse by insulin resistance and hyperglycemia increases the osmotic urinary excretion of magnesium. High incidence of hypomagnesaemia in Type 2 DM has been reported in different studies.\u0000\u0000Aims and Objectives: To assess status of serum magnesium level in patients with Type 2 diabetes mellitus.\u0000\u0000Materials and Methods: This cross-sectional analytical study was conducted in the Department of Biochemistry, Sylhet MAG Osmani Medical College, Sylhet between January 2021 and December 2021. Fifty-five type-2 diabetics and age and sex matched 55 healthy volunteers were included by applying non probability convenient sampling technique. Diabetics with acute complications, diarrhoea or other malabsorptive states, those taking alcohol or drugs such as amino glycosides, thiazide diuretics and pregnant women were excluded. All underwent fasting plasma glucose, serum magnesium and HbA1c levels.\u0000\u0000Results: The mean serum magnesium level was 1.67 ± 0.41 mg/dl and 2.02 ± 0.40 mg/dl in diabetics and non-diabetics respectively; difference was statistically significant (p=0.001). {"title":"Effectiveness of Consecutive Versus Premixed Administration of Fentanyl and Bupivacaine in Subarachnoid Block for Total Abdominal Hysterectomy","authors":"Chhanda CS, Rashid MSU, Mondal SK, Begum M, Shimu AA, Boby N, Ali MS","doi":"10.47648/jswmc2024v14-01-86","DOIUrl":"https://doi.org/10.47648/jswmc2024v14-01-86","url":null,"abstract":"Background: Subarachnoid block (SAB) was a common regional anaesthetic technique, with opioids as adjuvants to enhance effectiveness and reduce hypotension.\u0000\u0000Objective: Comparison of the effectiveness of consecutive versus premixed administration of fentanyl and hyperbaric bupivacaine in subarachnoid block for TAH.\u0000\u0000Methods: This Quasi experimental study was carried out in 70 patients divided into two groups of 35 each: Group A received 0.5% hyperbaric bupivacaine 15mg (3ml) premixed with fentanyl 25µg (0.5ml) in a same syringe and Group Breceived fentanyl 25µg (0.5ml) in one syringe followed by 0.5% hyperbaric bupivacaine 15mg (3ml) in a separate syringe. The study compared the onset of sensory and motor block, reaching maximum sensory level, and complete motor block, as well as the time required for analgesia and ephedrine. Statistical significance was determined at a P-value < 0.05.\u0000\u0000Results: The study found that group B had significantly less time for onset of sensory block(3.45±0.41 vs 2.65±0.32min), motor block (4.51±0.53 vs 3.31±0.46min), and total ephedrine consumption (15.29±6.52 vs 11.86±5.95mg)compared to group A. Data were collected through face-to-face interview using a pre-tested semi-structured questionnaire with validated and reliable tools.\u0000\u0000Results: The mean (±SD) age of the respondents was 25.75 (3.97) with predominantly (53.6%) higher secondary school certificate qualified. Majority (60.3%) had <1 child and >90% availed the antenatal checkup. Among the 209 respondents, 39.2% and 36.4% had perinatal anxiety and depression respectively. There are different reasons of satisfaction and dissatisfaction in different age group, gender and types of specialties. Aim of this study was to evaluate the satisfaction and dissatisfaction of the senior specialists on their own specialties.\u0000\u0000Methodology: It was a questionnaire based survey. Sample size was 45. Specialists have more than 5-year experience were included in this study. Reasons of selecting the specialty, causes of satisfaction or dissatisfaction were included in the questionnaire. Level of satisfaction was documented by Satisfaction with Life Scale (SWLS) guideline. Non-judgmental or neutral participants were excluded from the study. Chi-square test was done for the significance tests for categorical data.\u0000\u0000Result: Satisfaction level was very high (93%) and was equally distributed in both the genders. Among the participants 29 (65%) were male and 16 (35%) were female. Key reasons of selecting the specialty were scope to teach (44.44%), scope to treat patients (33.33%) and duration of the course (26.67%). Main causes of satisfaction were less physical / mental stress (21.92%), student / patient satisfaction (17.81%) and scope of job / family time balance (16.44%). The most important fator for specialist dissatisfaction was less time to work-family imbalance (26.09%).\u0000\u0000Conclusion: Selecting the specialty is an important decision of life. The study* was done from July 2013 to December 2013. The study included 100 neonates who experienced seizure episodes. A detailed history was taken and thorough physical examinations were performed after acquiring written consent. A complete blood count, urine R/M/E (routine microscopic examination), blood glucose, total serum calcium, and serum electrolytes (sodium, potassium, and chloride) levels were done in all 100 neonates.\u0000\u0000Results: 100 hospitalized newborns were evaluated, of which 43% were born to primi mothers and 57% to multipara mothers, 37% of newborns were delivered at home while 63% were born in a hospital. The majority (60%) of births were through normal vaginal delivery (NVD) and 73% of the mothers reported being in good health in the antenatal period. After death, bones often survive decay and provide evidence of a person's sex. As the strongest bone in the facial skeleton, the mandible often resists post-mortem damage and provides important information about sexual dimorphism.\u0000\u0000Objective: Assessing the morphometric changes in the dry human mandible to ascertain sex is the study's goal.\u0000\u0000Methods: From January to December 2017, a descriptive study took place at the Department of Anatomy, Sylhet MAG Osmani Medical College, Sylhet. By using a convenient sampling method 50 adults, dry, complete, undamaged human mandibles were selected.\u0000\u0000Results: A variety of measurements were made, recorded as Mean ± SD, and examined using SPSS Statistics 21's unpaired t-test. {"title":"Pattern of Thalassemia and other Hemoglobinopathies in Sylhet, Bangladesh","authors":"Chowdhury MZR, Benzamin M, Chowdhury T, Jahan A, Dey RK, Sharker MH, Tamal TB, Chowdhury A","doi":"10.47648/jswmc2024v14-01-90","DOIUrl":"https://doi.org/10.47648/jswmc2024v14-01-90","url":null,"abstract":"Background: Thalassemias and hemoglobinopathies are the most common inheritant hemolytic congenital disorders in Bangladesh.There is no well precise validated data available about the prevalence of Thalassaemia and related hemoglobin disorders in Bangladesh.\u0000\u0000Method: This retrospective, cross-sectional observational study was carried out in Dr.Benzamin’s Pediatric Liver Research Centre and Nutrition Clinic, Sylhet, Bangladesh. We reviewed the data record software of the hematology section of the Popular Diagnostic Centre and Mount Adora Hospital, Sylhet, from March, 2022 to July, 2023 and collected all the Hemoglobin electrophoresis reports.A total of 783patients data were evaluated, and the data were entered into Microsoft Excel and analyzed by Statistical Package for Social Sciences (SPSS) software version 22. The aim of the study was to identify the pattern of Thalassemia and other Hemoglobinopathies in Sylhet, Bangladesh.\u0000\u0000Result: Out of 783 patients, 291(37.2%) were male, 492 (62.8%) female, pediatric population that is under 18 years 387 (44.4%) and 18 years or more 396(50.6%). Among these, 262 (33.46%) reports showed Hemoglobinopathies. Before 6 months, 10 patients were advised for Hb electrophoresis. Overall, in study population, most common hemoglobinopathies were Beta thalassemia trait (118, 15.1%), followed by Hb E trait (45, 11.6%), Hb E Beta thalassemia (34, 4.43%), Beta thalassemia major (8, 0.9%). Same trend followed in pediatric age group, Beta thalassemia trait (53, 13.7%), Hb E tarit (84, 10.7%), Hb E Beta thalassemia (28,7.24%), Beta thalassemia major (8, 1.8 %). In adult age group, Beta thalassemia trait (65, 16.4%), Hb E tarit (39, 9.8%), Hb E Beta thalassemia (6, 1.5%%).\u0000\u0000Conclusion: Hemoglobinopathies among the anaemic population of Sylhet is very common. Beta thalassaemia trait, Hb E tarit, Hb E Beta thalassemia, Beta thalassemia major are common variants. This article reports a case of a young male farmer who presented to a tertiary-level hospital in north-eastern Bangladesh with severe fatigue, fever, abdominal pain and yellow discoloration of the whole body. Meticulous history taking revealed that he worked bare-footed in an animal farm and had a cut injury in his right foot 15 days back. On the basis of this piece of information, serological tests for leptospirosis were done that showed positive results. There was significant clinical and biochemical improvement after treatment.
