The projection of dermal papillae which are interdigitate with epidermal evagination, is called epidermal ridges. By the 17thweek of intrauterine life, these ridges are permanent and remain unchanged throughout life, with the exception of a little increase in size that coincides with growth. Thus, it is quite helpful to predict various genetically acquired disorders. Diabetes mellitus is a condition of partial genetic background. The objective of this study was to determine whether type II diabetes mellitus in Bangladeshi people was associated with a certain finger ridge pattern. This observational cross sectional study was conducted in Sylhet MAG Osmani Medical College from July 2018 to June 2019. A total of 200 subjects were included; among them, 100 were diagnosed patients of type II DM and the rest were healthy individuals. The age and sex of the research participants were matched between the groups under investigation. The ink and paper method was used for making a print of the finger. A mounted hand lens was used to examine the prints. The distribution of finger ridge pattern between the diabetics and controls showed that the ulnar loops (p = 0.003) and whorls (p = 0.001) were significantly higher in diabetic group; whereas arches (p = <0.001) and radial loops (p = <0.001) were found to be significantly higher in control group. The study came to the conclusion that type II DM has the unique characteristics of the finger ridge distribution patterns in palmar dermatoglyphics, it can be employed as a reliable screening method for type II DM early diagnosis.

Several methods were studied by different authors both at home and abroad, but none were conclusive.\u0000\u0000Methods: This prospective study was done between January 2011 to December 2012, 15 patients of the open intercondylar fractures of Distal femur were treated by transosseous osteosynthesis technique by Ilizarov External Fixator in NITOR (National Institute of Traumatology and Orthopaedic rehabilitation), Dhaka. 2 of the patients were not available for final follow up after removal of the fixator frame. So ultimately the final result based on the results of 13 Cases. Minimum 6 months follow up was done from the date of injury. No patient below age 16, no type IIIB or IIIC fractures and no patients having less than 6 months follow up were included in the study. Patients with severe injuries (Head injury, Abdomen and chest injury) were excluded from the study. Patients with medical illness such as Diabetes Mellitus, Peripheral Vascular diseases were not included in the study.\u0000\u0000Results: 12 male and 01 female patient between 16 and 56 years were studied. Most common age group in this series were 16-30 year age group (53.85%) and average age of the patients was 31.15 years. Most of the patients were male and road traffic accident was the leading cause of injury (92.31%). Right side involved in (69.23%) most of the cases. All patients were C3 type of fractures and G-II and G-IIIA types were same (38.46%). Most of the patients (53.85%) were operated within 1-2 weeks of admission. Average duration of hospital stay was 22 days ranging from 10 day to 45 days. Most of the patient had soft tissue healing by granulation tissue formation. Others were treated by primary closure, Delayed primary closure or secondary closure.\u0000\u0000Various complications were observed among them pintract infection, wire breakage, wire loosening, stiffness of joints, angulations and rotations, deep infection of the tissue, leg length discrepancy was observed in this study. According to the criteria of Neer et al, 1967 final results of the treatment were analyzed. Acceptable result was found in 61.54% of cases (Excellent 7.69% and good 53.85%). Results of the rest of 38.46% cases were not acceptable (Fair 23.08% and poor 15.38%). The result is similar with others done for open fractures of distal femur by Ilizarov External Fixator or other methods of fixation.\u0000\u0000Conclusion: So it can be concluded that transosseous osteosynthesis technique is a satisfactory method for treating open intercondylar fracture of distal femur as a primary and definite mode of treatment.

The evidence demonstrates that mortality among ESRD patients is lower when sufficient hemodialysis treatments are provided. The achievement of adequate dialysis dose, measured by Kt/V, plays a pivotal role in improving patient outcomes.\u0000\u0000Method and Material: This was a center-based descriptive cross-sectional study carried out in Hemodialysis unit of NIKDU, Dhaka, Bangladesh from January 1, 2012, to June 30, 2013, seeks to investigate whether increasing the frequency of hemodialysis sessions to three times per week, totaling 12 hours per week, significantly improves the attainment of target Kt/V in hemodialysis patients when compared to those receiving the conventional regimen of 8 hours per week. Purposive sampling of 120 ESRD patients on maintenance hemodialysis (MHD) getting dialysis for at least one month through Arterio-Venous Fistula (AVF) and at least 2 dialysis session per week.\u0000\u0000Discussion: Out of 120 patients 72 (60.0%) were male and ratio was 1.5:1. The mean age of hemodialysis patients in this study was 51 years (range: 18-75 years). Most of the patients 62 (52%) were on 8 hours per week hemodialysis session. Our study showed mean Kt/V, URR, TACurea and nPCR of all study population was 1.21 ± 0.40 62 ± 12, 83 ± 26 and 1.29 ± 0.46 respectively. In 8 hours per week hemodialysis group achieved target Kt/V > 2 was only 3(5%) and on the other hand 12 hours per week hemodialysis group achieved target Kt/V > 1.2 was only 26(45%). Among the study population only 52 (43%) achieved URR > 65% , 13 (11%) patients TACurea was less than 52 mg/dl and 107 (89%) patients achieved nPCR > 1 g/kg/day. Mean values of URR was significantly higher of Kt/V of >1.2 and nPCR of >1 g/kg/day group. Most of our hemodialysis patients inadequately dialyzed. In 12 hours per week dialysis group significantly higher BMI, hemoglobin, nPCR and low phosphorus and TACurea. To achieve hemodialysis adequacy of KDOQI 2006 recommendation needs to increase frequency of HD that is 3 sessions per week (12 hours /week) and needs to give more attention to others factors which increase urea clearance.\u0000\u0000Conclusions: Furthermore, it underscores the need for continuous research to enhance our understanding of hemodialysis adequacy and improve the management of patients with end-stage kidney disease.
