Resistance to antibiotics is one of the most important challenges today, with serious implications for human health, due to self-medication and lack of regulation of their sale. Article 226 of the General Health Law establishes that no medicine can be sold in semi-fixed or mobile stalls, because it can contribute to public health problems such as antibiotic resistance.Purpose: To explore whether there is informal sale of antibiotics in street markets in an urban area of Mexico.Methods: A qualitative investigation with a descriptive and exploratory scope was carried out in eight markets in the city through the mystery shopper technique and to detect and evaluate whether there is informal sale of antibiotics in the markets.Results: The indiscriminate and non-prescription sale of medications was confirmed in the area's street markets. Hypoglycemic agents, antipyretics, analgesics, and various types of antibiotics were prominent in five of the eight markets studied. The European Happy Patient project aims to optimize antimicrobial use through audits and training of healthcare professionals, including community pharmacists. Objectives: To evaluate the impact of an educational intervention on the quality of antibiotic dispensing in Spanish community pharmacies and to identify areas for improvement following the training.Materials and methods: A five-hour multifactorial intervention was conducted at IDIAP Jordi Gol (Barcelona) in January 2023, including feedback on results, communication workshops, and educational materials. Subse-quently, a second audit (February-April 2023) was carried out using the Audit Project Odense® method in 16 community pharmacies. A total of 450 dispensations of oral antibiotics for acute infections were analyzed.Results: Sixty percent of patients were women, and 90% were aware of the indication for the antibiotic. Relevant increases were observed in safety checks: drug interactions (+26%), contraindications (+23%), and allergies (+20%), with all three being performed in 60% of cases. Personalized information provided to patients increased regarding treatment duration (66%à80%), antimicrobial resistance (30%à48%), and medicine return/disposal (6%à28%). Contact with the prescriber increased from 0.9% to 9.3%.Conclusions: The educational intervention significantly improved the quality of antibiotic dispensing by increasing patient counseling and safety checks. Having been detected decades ago, there are now new health strategies that can optimise its control in this group. Community pharmacists should be trained in this approach to improve their intervention and contribute to better health outcomes. Material and methods: Non-systematic review of literature in English and/or Spanish, open access, published between 1 January 2023 and 31 December 2024 in PubMed and Google Scholar. The aim is to contribute to the training of community pharmacists on this disease and its impact on the ageing population, considering preventive and therapeutic tools for its control. Titles, keywords and abstracts were analysed before accessing the articles. Consensus documents and relevant bibliographic references were also reviewed. Results: 63 articles and one consensus document on community pharmacist involvement in the RSV approach were identified. Conclusions: This narrative review synthesizes the available evidence on the burden of RSV in older adults and the potential role of community pharmacies in addressing it. It identifies a clear need for detection, referral, and prevention strategies from the pharmacy setting, as well as the integration of community pharmacists into public health programs. This work constitutes a useful starting point for future systematic reviews and applied research. Community pharmacy (CP) has proven to be a key resource in the detection and management of various infections, facilitating access to screening services and health counseling. Objectives: To design and assess the impact of a pharmacist-led professional service for the detection of suspected UTI in the CP setting, exploring its implementation in routine practice, patient follow-up, and referral to primary care when required. Methodology: A type 1 hybrid study was conducted involving 32 CP in Gipuzkoa. The CP offered the service to any person who entered the pharmacy with symptoms compatible with UTI, where a questionnaire and a urine test strip were administered. If the result of the strip was positive, the person was referred to the health care. In all cases, and depending on the pharmacist's criteria, non-pharmacological treatment was indicated and/or sanitary and hygiene measures were recommended and the patient was contacted after 3-7 days to follow up the problem. Results: A total of 272 individuals with UTI symptoms were recruited. 67.6% (n=184) of the patients had a positive urine test strip result and were referred to the health center, while 32.4% (n=88) had a negative result and received follow-up. Overall, the health problem was resolved in 60.6% (n=117) of the referred cases, and 58.2% (n=46) of those with a negative strip improved with pharmacist recommendations. Conclusions: The service enabled the identification and referral of patients with suspected UTI and supported the management of a relevant proportion of cases without medical referral. Community pharmacists (CPs) are well-positioned to manage these conditions, reducing unnecessary physician visits and healthcare costs. However, their role in Libya remains undocumented, with no formal guidelines or standardized practices. Therefore, this study assessed Libyan CPs' practices, the prevalence of minor ailments encountered, and the challenges faced in delivering these services. An exploratory, cross-sectional study was conducted using a validated questionnaire. Researchers visited 200 community pharmacies in Tripoli, Libya, between October 2024 and January 2025, including only one pharmacist per pharmacy. Descriptive statistics were used for analysis. Most CPs were female (61.5%) and aged 21-30 (58%). A significant qualification gap was identified; only 54% held the mandatory qualification for independent practice, while 44% were assistant pharmacists with technical diplomas. High-prevalence conditions included headache (89%), fever (85%), and the common cold (73%). Pharmacists primarily relied on patient-reported symptoms (90%) and spent ≤5 minutes per consultation (50.5%). Key challenges included managing pregnant (67%) and geriatric patients (63.5%), workflow interruptions (25.5%), and a lack of formal training (only 51% received pre-graduation education). Libyan CPs actively manage minor ailments but face systemic barriers, including inadequate consultation time, a lack of guidelines, and difficulties with complex cases.
