{"title":"A rare cause of hemobilia: Metastatic melanoma of the gallbladder","authors":"Meera L Chandrananth, M. Patients usually present with symptoms resembling acute cholecystitis, however here we present a case of metastatic melanoma of the gallbladder presenting with hemobilia.\u0000 Case Report: An 82-year-old lady with a history of skin cancer excisions, including melanoma, many years ago, presented with anemia of unknown origin. A computed tomography (CT) scan was performed, demonstrating a gallbladder mass and hemobilia. She successfully underwent open cholecystectomy, partial liver resection, and lymph node dissection, which on histopathology, demonstrated metastatic melanoma. She remains disease free eight months later.\u0000 Conclusion: Although melanoma of the gallbladder is rare, in patients with a past history of melanoma, a high index of suspicion should be maintained if there are concerning signs, such as anemia, without a clear cause. {"title":"Serum negative immunoglobulin G4-associated cholangiopathy mimicking hilar cholangiocarcinoma: A case report and review","authors":"Nwaduru Chinedu, C. Thomas, P. Immunoglobulin G4-associated cholangiopathy often has a robust clinical response to steroid therapy, however making a diagnosis can be difficult as the cholangiographic features may resemble that of cholangiocarcinoma with varying immunological profiles. In the absence of features suggestive of malignancy, a high index of suspicion for the disease should be maintained, even in the presence of normal serum levels of IgG4. Case Report: In this case report, the diagnosis of IAC was made following cholangiographic imaging, along with the presence of retroperitoneal fibrosis and thickened urothelia, although serum IgG4 levels were within normal limits. The resolution of biliary strictures following a trial of steroid therapy further confirmed the diagnosis. Conclusion: Immunoglobulin G4associated cholangiopathy requires a combination of clinical, serological, histopathological, and radiological features in order to make a clear diagnosis. A trial of steroid therapy in the event of an unclear clinical presentation further helps in differentiating IAC from hilar cholangiocarcinoma. Bile duct injuries are associated with increased morbidity and mortality, as well as substantial health care costs, and malpractice litigation claims. This study aims to determine the quality of life (QoL) of Filipino patients after repair of BDIs incurred from their previous laparoscopic or open cholecystectomies. Methods: A total of 22 patients post-repair of BDIs and 22 ageand sex-matched patients who had unremarkable cholecystectomies from 1997 to 2017 were recruited for this study. The patients were requested to answer a survey on QoL using RAND 36-Item Health Survey 1.0, a validated survey which includes a multi-item scale that assesses eight health concepts. Results: Patients with BDIs had lower scores in all domains compared to non-BDI group. Physical functioning and role limitations due to physical health are statistically significant (p values 0.0473 and 0.0025). Conclusion: The effect of BDI is considerable and bears an impact on a patient’s health and well-being. {"title":"Clinicopathological difference between invasive pancreatic duct cancer and distal bile duct cancer of the pancreas head after pancreaticoduodenectomy","authors":"M. Ikeguchi, K. However, the clinicopathological difference between IPDC and DBDC after PD has not been thoroughly discussed. In this study, we retrospectively analyzed the clinical and pathological difference between IPDC and DBDC in patients who underwent PD. Methods: Sixty-six patients who underwent curative PD were enrolled (IPDC, n = 35; DBDC, n = 31). Preoperative, intraoperative, and postoperative parameters and pathological factors (stages, lymph node metastasis, lymphatic invasion, vascular invasion, and perineural invasion) were compared. Results: Jaundice was frequently detected and preoperative biliary drainage was frequently performed in patients with DBDC (60.5% and 90.3%, respectively). Additionally, the preoperative serum total bilirubin concentration and C-reactive protein/albumin ratio were higher in patients with DBDC than IPDC. As a result, the occurrence of postoperative pancreatic fistula occurred more frequently in patients with DBDC. In contrast, lymph node metastasis, lymphatic invasion, and vascular invasion were detected more frequently in patients with IPDC. The overall 5-year survival rate of the 35 patients with IPDC (13.4%) was much worse than that of the 31 patients with DBDC (52.3%, p < 0.001). {"title":"Quantitative HBsAg titers in relation to disease progression and serum markers of iron metabolism among chronic hepatitis B patients","authors":"N. Yılmaz, H. Methods: A total of 99 treatment-naïve CHB patients [median (min/max) age: 39.0 (17–66) years, 61.6% were males] with HBsAg positivity for at least six months were included in this study. Data on patient demographics, quantitative HBsAg titers (IU/mL), liver enzymes, hepatitis B virus (HBV) DNA levels, fibrosis stage, necroinflammatory scores and serum iron parameters including serum Fe (μg/dL), total iron binding capacity (TIBC; μg/dL), transferrin saturation (%), and ferritin (ng/mL) were recorded and compared with respect to gender and age (median age <40 years vs. ≥40 years). Results: Serum Fe (78.2 ± 29.5 vs. 111.7 ± 36.8 μg/dL, p = 0.001), transferrin saturation [0.2 (0.1–0.5) vs. 0.3 (0.1–1.0) %, p < 0.001], and ferritin [27 (3.9–298) vs. 100 (31– 994) ng/mL, p < 0.001] levels were significantly lower in females than in males. Quantitative HBsAg titers were correlated with age negatively in males overall (r = –0.328, p < 0.001) and positively in females >40 years of age (r = 0.722, p < 0.001). In females, quantitative HBsAg titers were positively with fibrosis stage (r = 0.491, p = 0.002), necroinflammatory grade (r = 0.235, p = 0.049), and ferritin (r = 0.288, p = 0.011) regardless of the age.
