Yue, Shiu-Feng Huang","doi":"10.6501/CJM.1203.002","DOIUrl":"https://doi.org/10.6501/CJM.1203.002","url":null,"abstract":"Intranodal palisaded myofibroblastoma (IPM) is a very rare primary mesenchymal tumor arising from the lymph nodes. Only around 70 tumors have been reported in the English literatures. Most tumors were found in adult patients and were located in inguinal lymph nodes. IPM is a benign tumor; no malignant changes nor metastases have been reported and surgical resection is sufficient for treatment. However, this tumor is easily misdiagnosed as a metastatic malignant tumor, such as Kaposi’s sarcoma, melanoma, or leiomyosarcoma. Here we report the first incidence of IPM in Taiwan. A 40-year-old male patient presented with a painless, elastic nodule in the right inguinal region that had persisted for a long period of time. Excision biopsy was performed to investigate a suspected lymphadenopathy. Microscopically, the tumor was composed of interlacing bundles of spindle cells with nuclear palisading, focal hemorrhage, hemosiderin deposition, and the presence of amianthoid fibers. The latter is characteristic of IPM. A rim of remnant lymphoid tissue was observed at the tumor border. The tumor cells were positive for smooth muscle actin and cyclin D1, but negative for desmin, CD34, S-100 and CD117 by immunohistochemical staining . The patient had no tumor recurrence 3 years after resection. Positron emission tomography with 2-[^(18)F]fluoro-2-Deoxy-D-glucose integrated with computed tomography (^(18)F-FDG PET/CT) was performed to assist therapeutic planning. A high FDG uptake was noted at the primary site without skull base involvement or distal metastasis. Common adverse events of 5-FU include diarrhea, nausea, mucositis, and bone marrow suppression. Cardiotoxicity is not common. Sinus bradycardia induced by 5-FU is a rare complication but can be fatal if not managed well. We present a 79-year-old woman with colon cancer who developed bradycardia while receiving 5-FU infusion. The arrhythmia returned to sinus rhythm after cessation of 5-FU infusion. The mechanism of 5-FU-induced cardiotoxicity is unclear and there are no prophylactic medications that can prevent this side effect. If symptoms and signs of cardiovascular anomalies develop, infusion should be stopped immediately and a work-up performed. {"title":"Cerebral Infarction due to Cardiogenic Emboli Originating from Atrial Myxoma: A Case Report","authors":"S. A 51-year-old woman presented to a hospital elsewhere with sudden onset of vision loss, loss of consciousness and subsequent circulatory collapse. Computed tomography and magnetic resonance imaging of brain revealed a rostral brainstem infarction, cerebral angiography showed evidence of occlusion of the left posterior cerebral artery, and echocardiographic examination revealed the presence of a mass in the left atrium. The patient received palliative care and was discharged. Four months later, she presented with muscle weakness in both legs and was unable to walk. Ultrasonography revealed atherosclerotic plaques in the left and right carotid arteries and multiple segmental emboli in the lower extremities. In addition, physical examination revealed bilateral blepharoptosis, poor pupillary reaction to light, deficient upward and downward eye movements and limited adduction. Anticoagulants and blood-lipid lowering drugs were administered. Auscultation of the precordium revealed systolic murmurs when the patient was in a sitting position. Echocardiographic examination and computed tomography of the chest disclosed a mass in the left atrium. The patient underwent surgical resection of the atrial mass. Histopathological examination of the resected tumor revealed a glandular cardiac myxoma. At four-month follow-up, there was some improvement in neurological symptoms. A cardiac origin of cerebral infarction should be suspected in middleaged patients who present with cardiac myxoma. But there was no similar study to analyze the conventional risk factors of coronary artery disease with the patients of CSFP in our review. We retrospective analyzed patients presented with severe myocardial ischemia which documented by strongly positive Treadmill test, also accepted coronary angiography, and tried to delineate differences in the cardiac risk profile among the patients of significant coronary heart disease (SCHD), CSFP and normal coronary artery groups. Methods: We retrospectively recruited 139 patients presented with severe myocardial ischemia which documented by strongly positive Treadmill test. All patients received coronary angiography at Changhua Christian Hospital Taiwan, from November, 2008 to August, 2010. There were classified into three groups; SCHD (97 patients); CSFP (32 patients); and normal coronary angiography results (10 patients). All cardiac risk factors were recorded and analyzed. Results: The average age in CSFP group is younger than the SCHD group's (56.53 years: 62.57 years, p<0.025) and normal group's (56.53 years: 64.80 years, p<0.05). Although the difference of sex between these three groups was not statistically significant, but the percentage of women in the CSFP group is higher than the SCHD group's (37.50%: 28.87%, p=0.19). The most difference of the CAD risk factors between the CSFP and SCHD group is in DM (9.38%: 37.11%, p<0.025) and hypertension (43.75%: 69.07%, p<0.025). Many meshrelated complications have been continually published, but the clinical evidence is limited in regarding to repair large vaginal defect after surgically resect contracted mesh and fibrotic vaginal tissue. We report a 60-year-old woman who experienced vaginal spotting and dyspareunia for one and half years after hysterectomy with anterior vaginal wall inlaid synthetic mesh. Mesh shrinkage and vaginal stricture were the first impressions. Because she was continually treated with conservative methods for more than 6 months, but the symptoms did not relief. Surgical operation was arranged thereafter in our urogynecologic department. After the fibrotic tissue and contracted mesh were resected, then an appropriate size of Surgisis® (Cook Surgical Biodesign, Bloomington, Indiana) was overlaid the large defect without tension and sutured to the edges of healthy epithelium. Phenotypic information is being used to increase the accuracy of radiation treatment planning, by fusing CT planning images, which has resulted in better tumor control and fewer treatment related toxicities. Herein, we describe a teenager who presented with symptoms of acute retroviral syndrome and a very low CD4 count (7×10^7 cells/L). Serum specimens also tested positive on the HIV-1/2 Ag/Ab Combo assay for HIV infection.
