There are several types of muscular dystrophies classified according to the distribution of predominant muscle weakness including Duchenne and Becker, Emery-Dreifuss, facioscapulohumeral, oculopharyngeal, and limb-girdle type. Clinical manifestations of PMD are clumsy, unsteady gait, pneumonia, heart failure, pulmonary edema, hydropericardium, hydrothorax, aspiration, syncopal attacks, and sudden cardiac death. The deceased was a 34-year-old man, and the onset of the first clinical symptom, gait disturbance, was in his late teens. His elder brother had the same disease and experienced brain death after a head trauma and died after mechanical ventilation was discontinued. After an autopsy, we found contracture of the joints, pseudohypertrophy of the calf, wasting and fat replacement of the thigh muscle, pericardial effusion (80 mL), fibrosis and fat replacement of the cardiac ventricular wall, pulmonary edema, and froth in the bronchus. The cause of death was heart failure and dyspnea due to muscular dystrophy. We recently encountered two autopsy cases of accidental choking on small octopus and webfoot octopus. Case 1 involved a 58-year-old fisherman who ingested two live webfoot octopuses in his fishing boat and collapsed. He was immediately taken to the hospital but died. During autopsy, one of the webfoot octopuses was found between his pharynx and esophagus; it was obstructing the epiglottis and upper esophagus. His blood alcohol concentration was 0.140%. Case 2 involved a 55-year-old man who ingested an intact body part of a small octopus and was found dead in his house. He had a history of cerebral infarction and angina pectoris. Fatal pulmonary thromboembolism (PTE) is a cause of sudden natural death, and it is important to evaluate the underlying cause of PTE at postmortem examination. Various disorders or risk factors of PTE have been well known, such as malignancy, pregnancy, trauma, surgery and so on, or inherited thrombophilia [1]. While we are performing postmortem examination, such various disorders or risk factors should be concerned and examined in every organ and in deep veins in the lower extremities. Venous aneurysms (VAs) are rare [2, 4]. They are asymptomatic or used to be presented as various localized lesions [2-4]. However, they also may be presented as PTE when thrombi formation occurs in the aneurysm [2-4]. It suggests that it is necessary to examine the venous structures of the body as well to check for deep vein thrombosis in the lower extremities. Herein, we present a case of a woman who suddenly died of fatal PTE. The deceased had no various disorders or risk factors related to PTE, and chronic venous disease such as deep vein thrombosis. Furthermore, long-term drug use can gradually induce harmful effects in tissues and organs and can exacerbate some pre-existing diseases. Chronic methamphetamine use can induce various cardiovascular pathologies such as arteriosclerosis and morphological alterations of the heart, resulting finally in death. The incidence of methamphetamine abuse is growing, especially in younger age groups, and intracerebral hemorrhage in young people secondary to methamphetamine use has been increasingly reported [2]. In Korea, 1,319 cases of cardiac transplantation have been performed from 2000 to 2016, and more than 100 patients a year are currently operated [1,2]. With the development of surgical technique and immunosuppressant regimens, the prognosis of the recipients also has improved continuously. In the 2000s, the 5-year and 10-year survival rates were approximately 75% and 60% respectively in the western countries [3]. The long-term survival data in Korea also show compatible outcomes [4]. For the improvement of survival rate and quality of life, every cardiac recipient needs regular longterm follow-up with monitoring of acute and chronic complications such as graft rejection and side effects of drugs. Huh, Eui Yong Kim","doi":"10.7580/KJLM.2018.42.2.33","DOIUrl":"https://doi.org/10.7580/KJLM.2018.42.2.33","url":null,"abstract":"겨울이 시작되면 노인들이 목욕 중 갑작스럽게 사망했다는 언론 보도를 심심치 않게 접하게 된다. 정확한 빈도는 알 수 없지만, 전통적으로 대중목욕탕을 자주 이용하는 우리나라의 노인들의 목욕 문화에 비추어 볼 때 앞으로도 적지 않은 사례 가 지속적으로 발생할 것으로 생각된다. 노인들이 목욕탕의 온탕(hot bath tub) 목욕 중 사망한 경우, 특별히 이러한 사 망을 지칭하는 정해진 것은 없으나 다양한 용어(온욕사: hot bath-related death, 溫浴死, 입욕사, 욕조 내 사망, dead in hot bath tub, sudden death phenomenon while bathing, sudden bath-related death, bath-related death 등으로 표 현) 중 온욕사가 비교적 적절한 것으로 본 논문에 사용하고자 한다. 우리나라에서 노인 온욕사의 빈도는 사인통계를 통해 간접적으로 추정해볼 수밖에 없다. 2016년도 사인 별 사망통 계를 보면 70세 이상에서 79세까지 및 80세 이상 남자에서 불의의 익사 및 익수에 의한 사망은 10만 명 당 각각 6.4명, 6.6명이고, 같은 연령대의 여자에서도 각각 4.2명, 4.1명으로 전 연령 평균의 3배에 이를 정도로 높은 편에 속하고 있다[1]. 물론 이 통계에서 의미하는 불의의 익사 및 익수에 의한 사망 이 온욕사만을 의미하는 것은 아니지만 상당수의 온욕사 사 례가 포함되어 있을 가능성이 높다.
