They are of uncertain histogenesis, characterized by STK11 genomic alterations, with a subset associated with Peutz-Jeghers syndrome. The spectrum of morphologic appearances is wide. Most commonly, cords and trabeculae in myxoid stroma are seen in addition to other components. They exhibit polyphenotypic immunoprofile with frequent expression of epithelial, mesothelial, sex cord, and smooth muscle marker desmin as well as hormone receptors, and loss of cytoplasmic STK11 expression.</p>","PeriodicalId":39485,"journal":{"name":"Surgical Pathology Clinics","volume":"19 4","pages":"689-701"},"PeriodicalIF":0.0,"publicationDate":"2026-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149289168","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Homologous recombination deficiency (HRD) is a biomarker reflecting predisposing germline genetic changes as well as somatically acquired changes. Tumors with HRD may be eligible for poly-ADP(ribose) polymerase inhibitor therapy in the maintenance or recurrent setting. Expression of HER2 (ERBB2 gene product) or folate receptor alpha (FOLR1 gene product), assessed using immunohistochemical assays, is a biomarker for therapy with antibody-drug conjugates. Although the biomarker landscape in ovarian cancer remains relatively limited, these markers provide meaningful diagnostic and therapeutic guidance.</p>","PeriodicalId":39485,"journal":{"name":"Surgical Pathology Clinics","volume":"19 4","pages":"529-539"},"PeriodicalIF":0.0,"publicationDate":"2026-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149289054","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Despite these improvements, significant information gaps remain, and assigning an accurate FIGO stage may be challenging in several situations faced by pathologists.
