{"title":"Prevalence and impact of obstructive sleep apnea in type 2 diabetes mellitus: A descriptive cross-sectional study","authors":"Puneet Saxena , Dharmendra Singh , Yashpal Singh","doi":"10.1016/j.mjafi.2022.07.008","DOIUrl":"10.1016/j.mjafi.2022.07.008","url":null,"abstract":"<div><h3>Background</h3><div>Obstructive sleep apnea<span> (OSA) has been reported to have a high prevalence in patients with type 2 diabetes mellitus. There is scarcity of literature on relationship between OSA and diabetes in Indian population.</span></div></div><div><h3>Methods</h3><div><span>A cross-sectional observational study was conducted at a tertiary care hospital and 80 consecutive and consenting patients with diabetes were enrolled over 24 months from 01 Sep 2014 to 31 Aug 2016. After a detailed history and clinical examination, all patients were subjected to a level I polysomnography (PSG), and their blood sample was drawn for the assessment of diabetes control, insulin resistance, and </span>microvascular complications.</div></div><div><h3>Results</h3><div>Out of 80 patients with diabetes, 30 (37.5%) patients had a high-risk score on Berlin questionnaire and 59 (73.8%) patients had evidence of OSA by PSG. The prevalence of OSA in diabetics with normal body mass index<span><span>, waist circumference, and neck circumference was 65.5%, 64.2%, and 67.2%, respectively. Patients with OSA had a significantly higher mean glycosylated hemoglobin (correlation coefficient 0.53) and higher insulin resistance (correlation coefficient 0.78). Patients with microvascular complications had a higher prevalence of OSA than those without. This included </span>neuropathy<span> (100% versus 62.5%), retinopathy<span> (100% versus 69.6%) and diabetic kidney disease (DKD) (90.9% versus 52.8%). Patients with any microvascular complication were significantly more likely to have OSA (odds ratio 13.66, 95% confidence interval 3.94–47.37, p < 0.001).</span></span></span></div></div><div><h3>Conclusion</h3><div><span>Indian patients with diabetes have a high prevalence of OSA, regardless of obesity. With a far-reaching health, social, economic impact, preventive strategies form the cornerstone of its management. Knowledge about vaccination status and protection rendered thereof would aid in more wholesome management among highrisk population groups like healthcare workers. The aim of this study was to evaluate the </span>Hepatitis B vaccination status among Healthcare workers (HCW) and the level of protection post-vaccination.</span></div></div><div><h3>Methods</h3><div><span>During the study period a total of 420 HCWs were included in the study. The details of vaccination status was ascertained by an online questionnaire. Serum samples were evaluated for </span>HBsAg<span> status and Anti HBs<span> titres to assess the level of protection against HBV infection. Those who were HBsAg negative, anti-HBc total was assessed.</span></span></div></div><div><h3>Results</h3><div>Our study revealed a high vaccination status among HCWs, n-384 (93.2%). However, the percentage of HCWs who were completely vaccinated was low (56.3%). Most of the study population, n-395 (95.6%) had protective titres of anti-HBs which showed a declining trend with increasing duration since the last dose of the vaccine.</div></div><div><h3>Conclusion</h3><div>Hepatitis B infection continues to be an important health hazard with significant morbidity and mortality. Healthcare workers are at higher risk of contracting the infection. Self-directed learning (SDL) is one of the approaches for making undergraduate students health/ profession students lifelong learners. Moreover, the SDL approach allows learners to work in a team and gain social skills. The aim of the study was to develop and validate the facilitator and peer score sheet as an assessment tool for self-directed learning in the department of physiology, in the Indian context.</div></div><div><h3>Method</h3><div>The facilitator and peer score sheet was developed and validated by the Delphi technique. The score sheet was implemented as a pilot test for one of the SDL modules in the department of physiology for undergraduate students.</div></div><div><h3>Result</h3><div>Two Delphi rounds were conducted till the content validity index (CVI) for the Facilitator Score Sheet (FSS), for 10 items attained was 0.977 and a CVI of 0.90 for seven items was for the Peer Score Sheet (PSS). The Coefficient of Variance (CV) in FSS for items 6, 10, and 11 and in PSS for item 6 was more than 30, which is not acceptable, so the item needed to be modified or removed from the list.</div></div><div><h3>Conclusion</h3><div>Varied assessment strategies are required for the SDL module to attain academic gain as per cognitive and affective attributional gains.
