Antibodies cocktail has shown promising results in preventing progression to severe disease, but the real-world experience is still evolving. Herein, we present a retrospective analysis of 22 patients who were administered the antibodies cocktail between August 2021 and March 2022 at our tertiary care center.</div></div><div><h3>Methods</h3><div>We conducted an observational retrospective analysis of clinicoradiological, inflammatory parameters, progression of the disease, and outcome among 22 mild and moderate COVID-19 patients treated with antibodies cocktail.</div></div><div><h3>Results</h3><div>The mean age was 67.7 years (SD ± 18.3) and comprised of 13 males (59%), while 9 were females (40.9%). Nine (40.9%) patients were fully vaccinated with two doses, nine (40.9%) were partially vaccinated with one dose while four patients (18.2%) were unvaccinated, and the rest were unvaccinated. Diabetes and hypertension were the commonest comorbidities; hematological and solid organ malignancies were other comorbidities. Eight patients had radiological opacities consistent with COVID-19 pneumonia and had shown significant regression in four patients after the therapy. None of our patients required supplemental oxygen or progressed to severe acute respiratory distress syndrome. Simalti ,&nbsp;Akash Malik","doi":"10.1016/j.mjafi.2022.10.004","DOIUrl":"10.1016/j.mjafi.2022.10.004","url":null,"abstract":"<div><h3>Background</h3><div><span><span>The purpose of this paper is to compare the efficacy of dual-phase multidetector computed tomography angiography (CTA) with transthoracic </span>echocardiogram<span> (TTE) and cardiac catheterization<span><span> angiography (CCA) in evaluation of pulmonary arteries and collateral vessels, </span>major aortopulmonary collateral arteries (MAPCAs) in children with </span></span></span>cyanotic congenital heart diseases.</div></div><div><h3>Methods</h3><div>The study was a prospective observational study where 32 pediatric patients (18 males, 14 females and age range 2–116 months) with cyanotic congenital heart diseases (CCHD) were included. All patients underwent TTE, CTA, and CCA. The findings of CTA in evaluation of pulmonary arteries and MAPCAs were compared with TTE and correlated with CCA findings.</div></div><div><h3>Results</h3><div><span>All CTA studies were adequate except in one (3.1%) case in which main pulmonary artery and left pulmonary artery were not visualized on any of the three modalities. </span>Right pulmonary artery<span><span> anatomy was not clear or not demonstrated in four cases (12.6%) on CCA, whereas CTA was able to demonstrate pulmonary arteries in these cases. TTE was inadequate in 11 cases (34.3 %) in which one or more pulmonary artery was not clearly visualized. In cases with good pulmonary artery diameter (corresponding to Z score between 1 to 2) statistically significant (P &lt; 0.001) correlation was found between pulmonary artery diameters, McGoon ratio, Nakata index, and Z-scores calculated for pulmonary arteries on all three modalities. There was concordance between CTA and CCA in assessment of MAPCAs and </span>patent ductus arteriosus (PDA), whereas TTE failed to demonstrate MAPCAs in six cases (18.8%).</span></div></div><div><h3>Conclusion</h3><div>CTA was found to be superior to TTE and CCA for the assessment of pulmonary arteries and MAPCAs. After sleep loss, altered voice quality is noticed. The circadian rhythm controls the sleep cycle, and when it is askew, it results in fatigue, which is manifested in speech. Using </span>MATLAB<span> statistical techniques and the k-nearest neighbour (KNN) machine learning algorithm<span>, this study assessed the efficacy of the harmonic-to-noise ratio (HNR) as a speech biomarker in differentiating fatigued and normal voice after sleep deprivation of one night.</span></span></div></div><div><h3>Methods</h3><div><span>After one night of sleep deprivation, acoustic samples for sustained vowel/a/and </span>visual reaction time<span> were recorded from n = 32 healthy young Indian male volunteers (20–40 yrs). One-way ANOVA<span><span><span> established significant changes in voice characteristics with progressive sleep deprivation. The effectiveness of speech HNR as a biomarker for the detection of healthy and fatigued voice was researched, using the </span>KNN classifier in a </span>machine learning algorithm.</span></span></div></div><div><h3>Results</h3><div>The HNR voice feature was taken from an acoustic sample for three times: baseline (Time 1), 3 AM (Time 2), and 7 AM (Time 3) towards an incremental one-night sleep loss. At 3AM, the HNR changed significantly p&lt;0.05. Utilizing an iterative signal extrapolation approach, the KNN classifier divided the submitted voice signal sample into normal and fatigued categories.</div></div><div><h3>Conclusion</h3><div>The findings imply that the HNR can be used to link fatigue from sleep deprivation with vocal alterations by classifying voice samples in a KNN classifier. When it does occur, the clinical course<span><span> is aggressive. Certain radiological criteria may give a clue to diagnosis and help in deciding the appropriate course of action as well as prognostication. Surgical excision along with adjuvant radiotherapy is the treatment of choice; however, the overall prognosis is poor as complete excision is seldom possible. We present a rare case of </span>malignant transformation<span> of an intracranial EC without previous history of surgery. Subtotal excision of the lesion was possible, with post-operative histopathological examination revealing </span></span></span>squamous cell carcinoma. The patient was discharged from the hospital with no fresh deficits. She was, however, re-admitted on day 36 with features of raised </span></span>intracranial pressure<span>, for which she underwent cerebrospinal fluid diversion. Computed tomography<span> is an excellent modality for the initial evaluation of mediastinal lymph nodes<span> (LNs). Tissue diagnosis is of paramount importance to confirm the diagnosis of mediastinal lymphadenopathy. Of various modalities including CT-guided biopsy, mediastinoscopy is considered a gold standard for tissue acquisition, but it is associated with considerable morbidity. Endobronchial ultrasound-guided </span></span></span>transbronchial needle aspiration (EBUS-TBNA) is a </span></span>minimally invasive method<span> of sampling of mediastinal LNs and its role has been established in malignant cause of LN enlargement<span>. However, its role in diagnosing benign diseases has not been studied much.</span></span></div></div><div><h3>Methods</h3><div>In a cross-sectional observational study, we performed EBUS-TBNA of 116 patients, and the sample was evaluated by various pathological modalities.</div></div><div><h3>Results</h3><div><span>Most common LN sampled was subcarinal (68%). MTB gene Xpert was positive in 45 cases, and resistance was detected in 3 cases. Most common diagnosis was tuberculous lymphadenitis (67.9%).
