Sultana, Atiar Rahman","doi":"10.9734/ajpr/2024/v14i4336","DOIUrl":"https://doi.org/10.9734/ajpr/2024/v14i4336","url":null,"abstract":"Objectives: The aim of the study was to evaluate the demographic information, clinical features, surgical methods, postoperative complications, and outcomes at 6 months for patients with choledochal cysts who were treated at the Sheikh Russel National Gastroliver Institute and Hospital. The goal was to assess the outcomes of radical cystectomy versus partial hepatectomy in the treatment of choledochal cysts. Methods: This was a retrospective analysis of 36 patients who had choledochal cysts and were treated with either a total cystectomy (n=30) or partial hepatectomy (n=6). Results: The groups that underwent radical cystectomy and partial hepatectomy had similar age ranges, sex distributions, and types of cysts. The majority of the cysts were type I (83.3% of the total cystectomy group) and type V (100% of the partial hepatectomy group). Other associations included: gallstones (26.7% vs. 83.3%) and Abnormal pancreatic biliary duct junction (APBDJ) (36.7% vs. 16.7%). An increased frequency of post-operative pain, jaundice, upper abdominal swelling, and fever was noted in the group that had a radical cystectomy. Minimal post-operative complications were seen in both groups. At 6 months, the rates of complications were similar, with hypertrophic scars being the most common (30.7% for radical cystectomy and 25% for partial hepatectomy). Conclusion: The groups that had radical cystectomy and partial hepatectomy were of similar demographics, cyst types, abnormalities, clinical traits, and complication rates post surgery and at 6 months. Both procedures are still good ways to treat choledochal cysts, and this small group doesn't clearly show which is better. However, they are contributors to childhood morbidity and could lead to mortality in severe cases. \u0000Objectives: This study was undertaken to determine the prevalence, pattern and outcome of domestic injuries among children admitted into the Children’s Emergency Room (CHER) in Rivers State University Teaching Hospital (RSUTH). \u0000Methods: This was a 5-year retrospective study of 179 children with domestic injuries admitted into the children’s emergency unit from January 2017 to December 2021. Data on socio-demographic characteristics, cause, type and outcome of domestic injuries were collected from their medical records and analyzed with SPSS version 23. \u0000Results: A total of 3987 children aged < 18 years were admitted into CHER. Out of these, 179 (4.48%) children had injuries from domestic accidents. Majority of them 89 (46.9%) were under-fives and males 108 (60.3%). Three commonest causes of home accidents were falls 52 (29.1%), burns 44 (24.6%) and mechanical trauma 32 (17.8%). Common domestic injuries observed in the study were burns (24.6%), head injuries (20.7%), ingestion of poisonous substances (13.4%) and lacerations (11.7%). Burns injuries were significantly more prevalent among females and poisoning among males. Incidents of poisoning was significantly more prevalent among under-fives (21.4%) and animal bites (13.7%) among older children, (p value < 0.05). The mean duration of admission in the emergency room was 1.27 ± 1.90 days. The present study investigated infections in childcare facility and The samples were taken by sterile cotton swabs from 39 different places, to know the different types of bacteria that in the nursery. the results revealed that, 6 different types of dangerous bacteria which are (Staphylococcus aureus, Klebsiella pneumonia, pseudomonas Aeruginosa, streptococcus viridians, Corynebacterium, Diphtheria, bacillus, and Enterobacteriaceae Serratia.). All of the isolate’s bacteria were found resistant to at least two antibiotics. Thirteen antibiotics were used in the sensitivity test which are: Cefotaxime, Azithromycin, Tetracycline, Vancomycin, Cefixime, ceftolozane-tazobactam test Doxycycline, Ciprofloxacin, Ampicillin, Cefotazime, Amikacin, and Oxacillin. The examination includes assessment of the central and peripheral nervous system and observation of posture, muscle tone, strength, reflexes and symmetry. Abnormal assessments may indicate neurological problems. As a rule, neurological maturity varies according to gestational age. Systematic physical examinations usually detect neurological defects. Early detection is crucial as plasticity is greater in early life. Despite developments in neuroimaging and neurophysiological technologies, clinical neurological examinations still have their importance. Performing a timely neurological assessment can speed up the diagnostic process and enable the swift administration of advantageous therapies. This is specifically applicable in situations that require therapeutic hypothermia to be initiated within six hours or sooner, as evaluation of neurologic status plays a crucial part in identifying the necessity for intervention. Neurophysiological technologies can be difficult due to immaturity and reduced myelination in neonates. It is due to premature welding of the coronal sutures. \u0000Presently described are cases of three infants diagnosed with Apert syndrome based on symptomatic association with the help of medical imaging. \u0000Alpert syndrome requires treatment by a multidisciplinary team. The priority of treatment is to combat brain compression in children and to manage cardiorespiratory problems. Acute respiratory distress syndrome (ARDS) is a significant cause of morbidity and mortality, necessitating various forms of respiratory support in pediatric intensive care units (PICUs). Prone positioning has emerged as a strategy to improve oxygenation in ARDS, but its effectiveness and correlation with non-invasive monitoring parameters remain underexplored. \u0000Subjects and Methods: This prospective observational study, conducted in the PICU of Dr. B.C Roy PGIPS, Kolkata, aimed to assess the impact of prone ventilation on oxygenation in children diagnosed with ARDS. The study, spanning 18 months, included 40 patients aged 3 months to 12 years requiring mechanical ventilation. Parameters such as oxygenation index (OI), oxygen saturation index (OSI), PF ratio, and SF ratio were monitored at different time points during supine and prone ventilation. Prone positioning's effectiveness was evaluated based on improvements in these parameters. \u0000Results: The study revealed a significant improvement in oxygenation status after four hours of prone ventilation compared to supine ventilation. Oxygenation index, OSI, SF ratio, and PF ratio showed statistically significant changes favoring prone positioning. A strong positive correlation between SF ratio and PF ratio was observed at various time points, emphasizing the potential of SF ratio as a non-invasive alternative. Responders and non-responders to prone positioning were identified based on predefined criteria, highlighting individual variability in treatment response. \u0000Conclusion: Prone ventilation demonstrated significant improvements in oxygenation parameters in children with ARDS. The study supports the use of non-invasive SF ratio as a reliable substitute for PF ratio, simplifying monitoring without invasive arterial sampling.
