Thus, the smoking behavior and dietary habits of 99 patients treated against pulmonary, cardiological and gastroenterological diseases in specialist outpatient clinics in Rzeszów were analyzed. The survey questionnaire examined the relation between tobacco smoking and dietary preferences (quantity and regularity of food intake, and frequency of consumption of several foodstuffs) as well as health effects of smoking. Among the respondents, 48% were currently smoking (smokers), 16% declared themselves as formerly smoking ex-smokers) and 35% did not smoke (non-smokers). The cardiovascular and respiratory health problems were more common in smokers group, but the differences were not significant (p>0.05). Smokers’ diet was low in healthy kind of food (i.e. vegetables, fruit, dairy product, antioxidative spices) and simultaneously was abundant in meat, strong coffee and alcohol (p<0.05) compared to non-smokers. For the ex-smokers greater attention to proper diet composition was observed. {"title":"A psychological analysis of factors affecting acute pain in postoperative patients","authors":"L. Suchocka, Kazimierz Popielski, M. The postoperative situation consists of three stages: the preoperative stage, the surgical phase, and the postoperative stage. Each of the stages is equally important, and it is crucial that medical staff should minimize the stress and discomfort related to hospitalization. Specialists suggest that the preparation to surgery should correspond to the patient’s style of responding to stress. The level of individually experienced pain depends not only on the type of surgery, but also on psychological factors and the patient’s personality traits. Aim. The aim of the study was to analyze the factors that affect the experience of acute pain in postoperative patients. Material and methods. The study was conducted in Lublin, Poland, and comprised 100 patients of the local surgical wards. After incomplete tests were excluded, the group of 68 patients (37 women and 31 men, aged 20-73) was selected. The following test methods were used: The McGill Pain Questionnaire (MPQ) by R. Melzack, Test Noo-dynamiki [The Test of Noo-Dynamics] (T.N-D) by K. Popielski, Kwestionariusz Poczucia Odpowiedzialności [The Sense of Responsibility Questionnaire] (KPO) by L. Suchocka, The IPAT Anxiety Scale Questionnaire (Self Analysis Form) by R.B. Cattell. Results. The study results show that the evaluation of pain is affected, at the statistically significant level, by the patients’ subjective experience of feeling ill, their surgery-related discomfort, and the intensity of pain. The patients who are not oriented towards future goals and tasks, closing upon themselves, evaluate the postoperative situation as difficult and distressing. The orientation towards new goals motivates the patients to fast recovery. Conclusion. The test results confirmed the research hypotheses. {"title":"Glycemic index and glycemic load in preventive cardiology – the state of knowledge, selected controversies","authors":"M. Skrzypek, Agnieszka Momora, R. Krzyszycha","doi":"10.2478/pjph-2019-0005","DOIUrl":"https://doi.org/10.2478/pjph-2019-0005","url":null,"abstract":"Abstract The current epidemiological situation in Poland is characterized by prevalence of cardiovascular diseases (CVD) as the main cause of mortality. In the paper the current state of knowledge on the importance of glycemic index (GI) and glycemic load (GL) in CVD prevention is presented, taking into consideration the latest recommendations on CVD prevention. In a nutrition strategy based on consumption of low GI/GL foods, the target of the intervention is the profile of dietary carbohydrates which is treated as a modulator of glycemic response. In the light of the current state of research, there is no reason to treat GI/GL values as markers of pro-health qualities of the dietary plan recommended in CVD prevention to individuals with normal glucose homeostasis. However, the preventive potential of diets characterized by low GI/GL deserves a more extensive application in patients with glucose homeostasis disturbances, including those with prediabetes and insulin resistance syndrome. Taking into account the relations between abnormal carbohydrate metabolism and a risk of developing diabetes, followed by CVD, the benefits of low-glycemic diets in individuals with impaired glucose homeostasis can indirectly result in lowering the risk of CVD. {"title":"Acute radiation-induced oral mucositis in patients subjected to radiotherapy due to head and neck cancer","authors":"A. Severe mucositis is followed by symptoms, such as extreme pain, mucosal ulceration and consequent limitations in swallowing and achieving adequate nutritional intake. Mucositis may also increase the risk of local and systemic infection and significantly affect quality of life and cost of care. Severe oral mucositis can lead to the need to interrupt or discontinue cancer therapy and thus may have an impact on cure of the primary disease. In spite of all the advances made in understanding the pathophysiology of oral mucositis, there is still no prophylactic therapy with proven efficacy and known risk factors. The rate of aging and the possible coexistence of diet-related diseases are largely determined by the nutrition and lifestyle of the elderly. Therefore, it is important to assess the degree of coverage of organism demand for such nutrients as calcium and vitamin D in the elderly population. Aim. The aim of the study was to determine the impact of age on calcium and vitamin D supply in daily alimentary rations of the elderly as well as their nutritional behaviour in terms of consumption of products which are the source of these nutrients. Material and methods. The study involved 42 subjects aged from 60 to 85 years. The vitamin D and calcium content in daily food rations was assessed with the method of a 24-hour dietary interview carried out in triplicate. Results. The nutrition of the elderly turned out to be inappropriate in terms of supplying the organism with calcium and vitamin D, and this was not age dependent. The average calcium and vitamin D supply in the daily food rations was estimated at 455.53 mg/day and 2.31 μg/day, respectively. The demand for calcium was covered in the range of 31-60% of RDA in over 60% of the elderly and the vitamin D demand was met in 30% of AI in over 80% of the subjects. Conclusions. Public health is a specific field, which deals with an improvement of population health. It is an interdisciplinary and intersectoral activity and requires participation of various workers. All current documents and acts in public health field in Poland emphasize on the performance of specific tasks, but leave aside personal status and competencies. Many aspects were not resolved till now, e.g. which domains should be included in public health workforce, the problem of workforce enumeration: how large this group should be and what competencies should have. Aim. The aim of the study was an attempt to indicate some professional identity components for public health workers. Material and methods. The study was performed applying the semi-structured interviews method using a hermeneutic approach. The informants were a purposeful sample of 12 people who obtained the master’s degree in public health and already took up professional work and graduated from two universities. The basic research question was: how do interviewees perceive the public health worker? Results. All respondents, regardless of the place of work, perceived themselves as public health workers and presumably as specialists. Attempts to describe a public health worker led to non-specific and generalized statements. There were no answers revealed to the responsibility issue for effects of work among interviews. Respondents tended to characterize public health worker competencies vertically, by domain categories, not functionally, by public health services, so the issue of key competencies importance for identity is controversial. Conclusions. There are no strong evidences that particular workplaces or core competencies are related to professional identity of public health workers. Currently, vegetable oils are used not only for consumption but also for medicinal and cosmetic purposes. They are especially eagerly used by women, although their knowledge in this field is usually insufficient. Aim. The aim of the study was to determine the dietary habits of women and to analyse the importance and position of vegetable fats in their diet and life. Another objective was to assess the level of knowledge and awareness of the health-enhancing properties of these products. Material and methods. The study involved women (n=253) aged 18-30, 31-40, and over 40 years living in Lubelskie Province, Poland. The modified questionnaire was composed of questions about their diet, regularity of meals, habits and preferences of fat intake, and motivation behind including fats in the diet or for cosmetic or medicinal applications. Results. Approximately 45% of the respondents sometimes used fats with their meals and 23% used them regularly. Fats were consumed by approximately 86% of the total number of respondents and were used for medicinal and cosmetic purposes by ca. 15.5 % and 6% of the respondents, respectively. Linseed oil (34%) and coconut oil (27%) were regarded as the most valuable fats. Butter and rapeseed oil were consumed most frequently (p<0.05). Approximately 47% of the respondents regarded refined oils as valuable products, and 40% of the respondents were aware of the harmful effects of trans fats. Conclusion. The provision of adequate health care for a successively growing group of geriatric patients requires taking into account many factors in order to preserve and/or improve their overall health and the related functional capacity. Its crucial aspects include the ability to safely change position and locomotion, as well as the severity of discomfort from the osteoarticular system. Aim. The objective of the present study was to assess the impact of comprehensive ambulatory rehabilitation including tailored endurance training, preceded by an ergospirometry test, on indicators showing the risk of falls and the severity of pain symptoms of patients aged 60 and over, receiving health services at the Outpatient Healthcare Home (DDOM). Material and methods. The study involved 60 seniors during their rehabilitation cycle implemented as part of the services provided to patients at DDOM of the W. Chodźko Institute of Rural Health in Lublin. The tests were carried out in the test-retest model on the first and last day of the kinesiotherapy cycle. The Tinetti scale of the risk of falls, balance and walk and Visual Analogue Scale of the severity of pain sensations (VAS) were utilized in the research. The patient rehabilitation program at the DDOM included adapted systemic kinesiotherapy (endurance training with a load determined according to individual exercise capacity, as determined on the basis of the ergospirometry test) and local kinesiotherapy as well as physical therapy adapted to the needs resulting from the condition of the musculoskeletal system. Results. After completing the rehabilitation cycle, the patients obtained higher scores, compared to the tests carried out before the beginning of the rehabilitation cycle, in the scales of Tinetti Questionnaire. There were also lower results of the respondents in VAS scale used to assess the severity of pain sensations. Conclusions. It is characterised by the MI provider’s empathy, understanding, mental and emotional attitudes towards the patient. This non-judgemental conversation is aimed at strengthening inner motivation and commitment to attain the goal and is based solely on the individual’s reasons for change. The initial application of MI in clinical psychology has been modified appropriately to allow for its use in health care, rehabilitation, public health, social work, dentistry, social rehabilitation, coaching and education. It is recommended for introducing new behaviours, lifestyles and therapeutic adherence. The application of MI contributes to the enhancement of patient-healthcare worker communication, the patient’s concordance and compliance. On the other hand, learning and adopting this method in counselling is useful for healthcare professionals. In treatment, greater patient awareness and obedience lead to more conscientious responsibility for treatment and health, which in turn, produces better therapy outcomes that serve as confirmation of the merit of the therapy prescribed. The application of MI contributes to health professionals’ greater success, satisfaction, self-confidence and a sense of self-efficacy. The aim of this work is to present the essential features of MI as well as some interesting examples of research showing the benefits of using MI and ideas for training it. As a result of involution processes and diseases, changes occur in the human body, as a result of which, psychophysical fitness declines with age. Comprehensive rehabilitation is the way of improving and maintaining health condition, including, among others, the appropriate level of physical activity and the quality of affective functioning. This type of program was carried out as part of the Outpatient Healthcare Home (DDOM) at the Witold Chodźko Institute of Rural Health in Lublin. Aim. The objective of the present study was to assess the impact of comprehensive ambulatory rehabilitation, including tailored endurance training preceded by an ergospirometry test, on indicators demonstrating the level of involvement in daily physical activities and the severity of depressive symptoms of patients over 60 years of age receiving health services at DDOM. Material and methods. The study involved 60 seniors participating in the rehabilitation cycle implemented as part of the services provided to patients at DDOM of the Witold Chodźko Institute of Rural Health in Lublin. The tests were carried out in the test-retest model on the first and last day of the kinesiotherapy cycle. The tests were performed with use of International Physical Activity Questionnaire IPAQ and Geriatric Depression Scale GDS. The patient rehabilitation program included adapted systemic kinesiotherapy (endurance training with a load determined according to individual exercise capacity, as determined on the basis of the ergospirometry test) and local kinesiotherapy and physical therapy adapted to the needs resulting from the condition of the musculoskeletal system. Results. After the completion of the rehabilitation cycle we compared the tests carried out before it, and the patients received higher scores in the scales of IPAQ questionnaire for measuring weekly, intensive and mode-rate physical activity and time required for walking and were less likely to spend their time sitting or lying down. There were also lower scores of the respondents in GDS scale used to assess the severity of depressive symptoms. Conclusions.
