{"title":"Efectividad de un programa de educación sanitaria con empoderamiento de pacientes con epilepsia","authors":"","doi":"10.1016/j.sedene.2022.12.001","DOIUrl":"10.1016/j.sedene.2022.12.001","url":null,"abstract":"<div><h3>Aims</h3><div>To evaluate the effectiveness of an educational intervention with empowerment for patients with epilepsy in a Spanish reference hospital for epilepsy.</div></div><div><h3>Methods</h3><div>A nonrandomized clinical trial was started in an epilepsy center. An empowerment group intervention program was conducted in which volunteers were recruited from outpatient clinics. It contained common topics in epilepsy health education, such as knowing your disease, types of seizures, treatment, how to act in the event of a seizure, seizure precipitating factors, lifestyle and medication management, as well as adapting to patients’ requests for information. The patients were empowered by applying their knowledge and skills, using the nurse as a facilitator. Questionnaires were administered before and after the intervention to assess anxiety and depression, quality of life, and satisfaction with care and attention to patients with epilepsy.</div></div><div><h3>Results</h3><div>33 patients completed the sessions and study. Satisfaction with the information improved significantly after the intervention (<em>P</em> <!--><<!--> <!-->.005). The information items with significant improvements were satisfaction with information about epilepsy, treatment, provision of aid and treatment of the impact of epilepsy (<em>P</em> <!--><<!--> <!-->.005). Satisfaction with communication decreased (<em>P</em> <!--><<!--> <!-->.005), being the items with the most changes, the “time to answer the questions” and “consideration with their individual needs”. No significant changes were shown in depression and anxiety, quality of life and satisfaction with the organization.</div></div><div><h3>Conclusions</h3><div>Empowerment interventions demonstrated benefits to improve satisfaction with information in patients with epilepsy. The educational intervention designed allows sharing knowledge, solving problems and improving skills, empowering the patient in a new concept of education, which the nurse is a facilitator who collaborates with the demands of the patients. {"title":"Presence of palinopsia as a sign of selective CNS tractopathy: A case report","authors":"","doi":"10.1016/j.sedene.2023.12.001","DOIUrl":"10.1016/j.sedene.2023.12.001","url":null,"abstract":"<div><div>Palinopsia, described as the persistence of visual images or their recurrence after the stimulus has disappeared, represents a rare but important neuro-ophthalmic complication. Until now, the pathophysiology and the main location of this visual mainteanance condition are not been clearly understood. This study presents a 30-year-old woman with palinopsia who had her selective optic radiation lesion assessed by diffusion tensor imaging (DTI). The finding of magnetic resonance imaging (MRI) did not reveal any damage in the selective optic pathway whereas DTI<span> showed a decreased density of the optic radiation tract. Furthermore, the mean diffusivity (MD), axial diffusivity (AD), and radial diffusivity (RD) were increased in suspected regions of lesions ds, while fractional anisotropy (FA) was decreased. To the authors’ knowledge, this is the first report of a case of palinopsia representing the central nervous system (CNS) tractopathy. Neurological deficit was measured with the National Institute of Health Stroke Score upon admission and 24<!--> <!-->hours after treatment with mechanical thrombectomy, and functional outcome with the modified Rankin scale at three months.</div></div><div><h3>Results</h3><div>The degree of dependency at three months was 2,4 points. The mean neurological deficit score on admission was 12,1 points, and 9,3 points after 24<!--> <!-->hours. Neurological deficit at 24<!--> <!-->hours has been found to be predictive of functionality at three months.</div></div><div><h3>Conclusion</h3><div>The mean of the neurological deficit at admission was situated in a moderate neurological deficit. The mean of the neurological deficit at 24<!--> <!-->h has been placed in a moderate neurological deficit. The score for the degree of dependency at three months was placed in the mild disability category. Responses to triggers can lead to suicide, social isolation and poor quality of life. However, scale studies of misophonia usually focus on triggers and responses to triggers. Therefore, there is a need for scales that focus on measuring the effects of misophonia on daily functioning, interpersonal relationships and overall functional capacity.</div></div><div><h3>Objective</h3><div>This research aims to create a scale and validate its psychometric properties to assess the effects of misophonia on daily functioning, interpersonal relationships and overall functional capacity.</div></div><div><h3>Methods</h3><div>This scale for misophonia was applied to 326 participants reached online. The data were analyzed through exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) and internal consistency was evaluated using Cronbach's alpha.</div></div><div><h3>Results</h3><div>The scale shows a four-factor structure with EFA and explains 63.41% of the total variance. With CFA, the standard regression coefficients ranged between 0.51 and 0.98. Subjects diagnosed with myasthenia gravis were recruited from associations, foundations and social media in the first quarter of 2022. The Medical Outcomes Study (MOS-SSS), the Myasthenia gravis activities of daily living profile (MG-ADL) and the 15-item Myasthenia Gravis Quality of Life scale revised (MG-QOL15R-S) instruments were included.</p></div><div><h3>Results</h3><p>Mean time to diagnosis (t=2,38; <em>P</em><.05), history of thymoma (χ<sup>2</sup>=5.18; <em>P</em><.05), MG-ADL global score (t=4.29; <em>P</em><.001), and MG-QOL15R-S global score (t=7.67; <em>P</em><.001) were related to perceived social support in MG subjects. We also found that MOS-SSS score correlated significantly with MG-ADL (r=−.15; <em>P</em><.001) and MG-QOL15R-S (r=−.27; <em>P</em><.001) scores.</p></div><div><h3>Conclusions</h3><p>A history of thymoma reduces the likelihood of high perceived social support, and diagnosis of the disease at less than two years has been associated with a higher frequency of high perceived social support. COVID-19, in addition to respiratory symptoms, has neurotropic characteristics that can impact the nervous system. We present a 22-year-old female who developed diplopia and numbness of her limbs eight days after affecting by COVID-19 and she showed a left-sided outward gaze palsy without any history of previous a specific illness or medication. Supratentorial, infratentorial, and cervical cord enhancing plaques confirmed an acute demyelinating process in favor of Acute disseminated encephalomyelitis (ADEM). Her oligoclonal bands (OCB) were positive as well. As a result, a provisional diagnosis of ADEM was made. Serie de casos","authors":"Macarena Marcos Osuna, Elena González Holgado, Maddi Llorente Olasagasti, Arantza López de Turiso López, Maite Martínez Zabaleta, Patricia de la Riva Juez","doi":"10.1016/j.sedene.2023.04.001","DOIUrl":"10.1016/j.sedene.2023.04.001","url":null,"abstract":"<div><h3>Background</h3><p>Femoral artery access site complications are uncommon but likely to become more frequent with the increased use of endovascular procedures in stroke patients.</p></div><div><h3>Aims</h3><p>This study aims to describe the clinical, diagnosis and treatment features of a consecutive case series of femoral artery pseudoaneurysms (FAP).</p></div><div><h3>Method</h3><p>A retrospective case series was performed for consecutive patients diagnosed with FAP at our institution between January 2016 and January 2020. Variables regarding patient demographics, endovascular procedural features and FAP description, diagnosis, treatment and outcomes were included.</p></div><div><h3>Results</h3><p>We identified a total of nine cases in a 5-year period; six happened after acute endovascular therapy for stroke, two after diagnostic arteriography and one after subarachnoid hemorrhage related ruptured aneurysm embolization. Most patients experienced local symptoms but in three cases the FAP was incidentally discovered during the stroke diagnostic study. Only one of the patients experienced severe anemia related to the FAP. Regarding treatment, local compression and patient rest was enough in two cases, thrombine percutaneous injection was used in three patients and open surgery in three. Nurse reports included evaluation of the groin status (pain, lump, and bruise) prior to the FAP diagnosis in six patients.</p></div><div><h3>Conclusions</h3><p>The clinical presentation of FAP after endovascular procedure is heterogeneous and its diagnosis and management may be challenging. {"title":"Nurses’ knowledge regarding dysphagia: A cross-sectional study","authors":"Patrícia Batista Olímpio , Alexandre Santos Pereira , Filipe Alexandre Bettencourt , Simão Pinheiro Gonzalez Machado , Isabel de Jesus Oliveira","doi":"10.1016/j.sedene.2023.03.001","DOIUrl":"10.1016/j.sedene.2023.03.001","url":null,"abstract":"<div><h3>Background</h3><p>Dysphagia has a high prevalence with associated complications, such as respiratory infections and recurrent institutionalizations, factors associated with the burden of malnutrition and dehydration that negatively affects the patient's quality of life and entails added health costs.</p></div><div><h3>Objective</h3><p>To assess the knowledge of a group of Portuguese nurses regarding dysphagia and perform the cross-cultural adaptation of the questionnaire used.</p></div><div><h3>Methods</h3><p>Cross-sectional descriptive study based on an online self-ministered questionnaire sent to nurses, regardless of the length of professional experience and area of clinical practice. Data treatment was performed using the IMB SPSS computer software, and the open responses were treated using the QDA Miner Lite.</p></div><div><h3>Results</h3><p>241 nurses were enrolled between May and June 2021, of which 192 (79.6%) were female, with an age of 37.8<!--> <!-->±<!--> <!-->8.3 years and a length of service of 14.4<!--> <!-->±<!--> <!-->8.5 years. Overall knowledge of dysphagia was high (80.1%). Participants were dissatisfied with their knowledge and expressed a need for further training, mainly in therapeutic intervention.</p></div><div><h3>Conclusions</h3><p>It is essential to understand why nurses still consider themselves to lack knowledge despite higher levels of knowledge. Future research should explore these aspects. These findings corroborate the gap between the nurses’ perceived knowledge and their actual knowledge about dysphagia, which may influence practice by influencing the decision-making process.
