The use of commensal microflora can be a promising direction in treatment of atopic dermatitis. The aim of this study is the selection of the optimal safe isolate of S. hominis for bacteriotherapy in patients with atopic dermatitis.Sensitivity of isolates of S. hominis ssp. hominis to antibacterial drugs was determined by the disk-diffusion method on Mueller–Hinton agar (HiMedia, India). The ability to form a biofilm was determined by measuring the amount of dye absorption by the biofilm on a microplate reader MR-96A (Mindray, China) at a wavelength of 495 nm. Antagonism in vitro was determined by the method of perpendicular strokes on the blood agar. In total, 24 adult volunteers (aged 18 to 60 years) were screened and included in the study. The results were calculated according to the zone of S. aureus growth retardation under the influence of metabolic products of S. hominis ssp. hominis. As a result of the study of 8 isolates of S. hominis ssp. hominis, which were obtained from the swabs taken from healthy skin of 24 people, one isolate of S. hominis ssp. hominis Hom-2 met all criteria of safety – Hom-2 demonstrated sensitivity to the studied antibiotics and formed a biofilm of low density (OD 0.15), and effectiveness (morphological and cultural properties, antagonistic effect on S. aureus). In this work, an isolate of S. hominis ssp. hominis Hom-2 met all criteria of safety and efficacy and will be used in the further study of bacteriotherapy in patients with atopic dermatitis.

Elderly homeless are in a particularly difficult situation. Being rejected from professional, social and cultural life, homeless seniors may develop depression and cognitive dysfunction. The aim of the study was to assess socio-demographic variables of homeless seniors and to assess the intensity of depression symptoms and the cognitive functions of older people from different life environments.Forty people aged 65 and over from the Opolskie Voivodeship were examined. The study group consisted of 20 homeless people. The control group consisted of 20 people living independently. Socio-demographics included age, gender, marital status, education and working status. The 15-point geriatric depression scale (GDS) was used to assess symptoms of depression. The clock drawing test (CDT) in the Sunderland version was used to assess cognitive functions. The homeless seniors and those living independently differed statistically significantly in terms of education (P < 0.001) and working status (P = 0.004). The differences in GDS between the groups were not statistically significant. The differences in CTD between the groups were statistically significant (P = 0.029). Homeless seniors have significantly greater cognitive impairment. The collected results in our own study, using the GDS and CDT scale, do not indicate a diagnosis of depression and cognitive disorders, but may only suggest their occurrence.Homeless seniors most often have a low level of education and are unemployed. Seniors experiencing homelessness demonstrate lower cognitive functioning.

The results of genetic tests may present a stressor related to the fear associated with the impact of the diagnosis on an individual’s future. The aim of the study was to verify the level of depression, anxiety, and stress in individuals waiting for the results of BRCA genetic tests. How personality traits, stress coping strategies and social support influence the level of mental discomfort was examined.82 consecutive individuals completed an anonymous online survey consisting of basic information and five questionnaires: the Inventory for Measuring Coping with Stress (Mini-COPE), the Depression, Anxiety and Stress Scale (DASS-21), the Generalized Anxiety Disorder 7-Item Scale (GAD-7), Polish adaptation of the Ten Item Personality Inventory test (TIPI-PL), and the Berlin Social Support Scales (BSSS).Participants obtained the highest scores in Mini-COPE strategies: active coping, planning, positive reframing, acceptance, and seeking emotional support. The results of DASS-21 and GAD-7 indicate the level of depression, anxiety, and stress mainly in the normal range. The highest scores in the TIPI-PL test were obtained on the conscientiousness, agreeableness, and extraversion scales. Participants, who have already obtained the results had higher scores on the Need for Support Scale (BSSS).In general, the situation of BRCA genetic testing is not causing significant negative mental discomfort. Nonetheless, the results indicate a significant role of social support in coping with stress in individuals who have already obtained the results of their genetic test.

KRUS (Farmer’s Social Security Fund) provides medical rehabilitation to all farmers entitled to it.\u0000Aim. The research concerning the effectiveness of the medical rehabilitation program for farmers was underdtaken following the assumption that there existed significant differences between the evaluation of patients’ health condition by physicians (based on the differences between the preliminary and final examinations) and patients’ own opinions, presumably underestimated, concerning their health. \u0000Materials and methods. This study was conducted between January 1, 2000 and June 30, 2006 in the Farmers’ Rehabilitation Centre in Jedlec. 1100 farmers were randomly selected to be examined from among 8312 participants of the medical rehabilitation program. Spondylosis and slipped disc were the main conditions which necessitated rehabilitation, as well as lower limb injuries among post-traumatic conditions. Research tools included patients’ case records, especially locomotor system examination cards. Data obtained from patients’ medical histories and preliminary and final physical examinations as well as medical documentation were compared. The results were presented in a tabular form. A comparison of preliminary and final examinations provided grounds for evaluating medical rehabilitation results.\u0000Results and discussion. According to medical evaluations, improvement after the rehabilitation course was observed in 82.9% of the cases. These results significantly differed from positive assessments expressed by the farmers themselves. Only 23% reported improvement during their final examination.\u0000Conclusions. The evaluation of the effectiveness of the medical rehabilitation program for farmers is a very complex issue; nevertheless, its positive impact on patients’ health is clearly noticeable. We can observe a great impact of extra-health factors on the evaluation of rehabilitation effects reported by the farmers. The evaluation of the effectiveness of the medical rehabilitation program for farmers carried out by the Centre physician was much more positive than the subjective evaluation reported during their final examinations by the farmers themselves. The needs of the farmers who undergo rehabilitation in the Centre concerning health care promotion and education are extensive. The program of health care promotion should be directed to all farmers, not only those who participate in structured rehabilitation courses.

Stroke is a major medical, economic and social problem. Thrombolytic therapy is the only form of stroke treatment which is truly effective. This therapy is applied too rarely in patients with ischaemic stroke. Aim. To present prehospital stroke treatment procedures, from recognizing the symptoms by the patient or his relatives at home to hospital admission, considering local circumstances in the Department of Neurology with Stroke Unit in the hospital in Działdowo. Discussion. Medical procedures in the prehospital period are based on the so-called chain of survival for stroke patients. The chain of survival consists of five elements. The first element is the recognition of the symptoms and providing first aid on the spot by witnesses and the medical staff. The second step is correct emergency communication, next obtaining qualified prehospital aid, then specialist transport by a resuscitation ambulance, and finally – treatment in a hospital emergency unit or admission room performed by stroke unit staff. Every element – every stage of the treatment must be of the highest quality and must be performed within the shortest possible time. Undertaking thrombolytic therapy is only possible within 3–4.5 hours after the first symptoms. Disqualification from thrombolytic therapy may result from a delayed recognition of stroke by the patient and his family, calling for medical help too late, and a delayed arrival to a specialized stroke centre which can provide alteplasis. Conclusions. Prehospital procedures based on the chain of survival for stroke patients are of fundamental significance to enable undertaking thrombolytic therapy of ischaemic stroke (rt-Pa, recombined tissue plasminogenactivator), the only specific method of treatment so far. Educating the society as concerns stroke and training professionals involved in stroke treatment can increase the percentage of patients treated with rt-Pa.

Carotid-cavernous arteriovenous fistula is an abnormal connection between the internal or external carotid arteries and the brain venous system within the cavernous sinus. Post-traumatic aetiology is most frequent; spontaneous fistulas\u0000are rare. The main clinical symptom is pulsating exophthalmia; other symptoms depend on the localization and haemodynamic features of the fistula. Frequently, palsies of the IIIrd, VIth and VIIth cranial nerves dominate in the clinical picture, contributing to diagnostic problems.\u0000Aim. To present a case study of a post-traumatic patient with a head injury and with atypical symptoms of carotid-cavernous arteriovenous fistula. Discussion. A 22-years old patient was admitted to the Department of Neurology with a periorbital ecchymosis and symptoms of the unilateral palsy of the IIIrd and the VIth nerves, after a head injury which had taken place a few days before. The\u0000symptoms of carotid-cavernous arteriovenous fistula appeared late and the clinical picture was dominated by damage to the cranial nerves. Initially, these symptoms were treated as complications of the skull fracture. A slightly intensified pulsating exophthalmia was observed. A gradual asymmetric swelling of the eyeballs, a lack of improvement following the treatment of the damaged cranial nerves as well as ophthalmologic consultations necessitated further diagnostic examinations. MRI and angio-MRI of the brain were performed, revealing a left carotid-cavernous arteriovenous fistula. The patient was transferred to an endovascular centre to undergo the embolisation of the fistula. \u0000Conclusions. Carotid-cavernous arteriovenous fistula may manifest itself after several days following a head injury. Occasionally, the intensity of the pulsating exophthalmia is relatively small and the symptoms of damage to the cranial nerves predominate. Early diagnosis, even of atypical forms of carotid-cavernous arteriovenous fistula, may prevent irreversible blindness or considerable loss of sight.

Kloc
Introduction. Low back pain is a very common G-force (Gravity-force) illness. Approximately 80% of the population suffer from low back pain at least once in their lifetime. The main causes of this condition are: an improper preparation of the spine to the erect position within the evolutionary process, weak muscles and a lack of basic knowledge concerning the biomechanics of the human body. Low back pain most frequently affects people at the working age, i.e. from 30 to 60 years old. It has been observed that low back pain tends to be recurring and frequently becomes chronic.\u0000Aim. The aim of this study was to analyse the prevalence of low back pain in the group of 40 students. This group was selected for the research because of their lifestyles which may contribute to the appearance of low back pain. The following factors were analysed: causes of low back pain, its frequency and intensity.\u0000Materials and methods. This study involved a group of 40 third year students of Nursery at the Faculty of Medical Science of the University of Warmia and Mazury in Olsztyn. The selected students were between 21 and 27 years of age. The questionnaire method was applied to carry out the research.\u0000Results and conclusions. The obtained results indicate that the age of people who suffer from low back pain is dropping. 75% of the respondents, aged between 21 and 27, manifested low back pain. The risk factors for such ailments include: additional work, sitting lifestyle and mental exhaustion. The presence of low back pain was connected with: G-force (Gravity-force) – in 35.09% of the cases, work – 21.05% of the cases, standing for a long time – in 21.05% of the cases. The frequency of low back pain was high in 50% of the cases, i.e. the respondents experienced pain several times a month. 50% of the respondents suffered from low back pain more often than once a month, 30% once a month, and the remaining 20% less frequently. The intensity of low back pain amounted, on average, to 4.17 points in the visual–analog scale. The obtained results should not be generalized. Further research needs to be carried out involving a bigger group of people and the results should be verified.
