Tamaki, Koichi Omori","doi":"10.5981/jjhnc.48.301","DOIUrl":"https://doi.org/10.5981/jjhnc.48.301","url":null,"abstract":"Although multidisciplinary treatment, including resective and reconstructive surgery, should be considered for the management of advanced oropharyngeal cancer, severe postoperative dysphagia may occur, and laryngeal preservation may be diﬃcult. Oropharyngeal carcinoma of the posterior wall is rare and has a poorer postoperative swallowing function and prognosis than that of other subsites, but detailed reports on swallowing dynamics are limited. Here, we report the case of a 76-year-old man. He underwent resection of the oropharynx, neck dissection, cricopharyngeal myotomy for oropharyngeal posterior wall carcinoma, and reconstruction with an anterolateral thigh flap. After surgery, the patient presented severe dysphagia but recovered the ability to swallow food that required mastication in approximately 2 months, and was discharged home. After approximately 4.5 years post-surgery, he remains able to consume regular food without showing any recurrence. We believe that preservation of most of the suprahyoid muscle group and cricopharyngeal myotomy were factors for maintaining swallowing function. Yamada","doi":"10.5981/jjhnc.48.28","DOIUrl":"https://doi.org/10.5981/jjhnc.48.28","url":null,"abstract":"Examination of late stage elderly patients with oral cancer who were classiﬁed as prefrail （vulnerable）: We examined 23 prefrail （vulnerable） patients among patients with late-stage elderly oral cancer treated in our department. The 5-year overall survival rate was 41.9%, and when classified according to treatment approach, the survival rate was higher in the group that received absolute curative treatment （standard treatment）. Uni-variate analysis of each factor affecting prognosis showed significant differences in G8 score （p=0.047） and stage （p=0.022）, and treatment approach （p=0.052） also tended to affect prognosis. In the results of multivariate analysis, only therapeutic approach was extracted as a factor that significantly affected prognosis （p=0.034）. In addition, decision tree analysis showed that the G8 score was divided into 14 points or more and less than 14 points, suggesting that it may be a predictor of prognosis. Prefrail （vulnerable） is a conceptual category in which it is diﬃcult to apply standard treatment, but it is thought that aiming for standard treatment without easily reducing the treatment intensity will lead to improvement in prognosis.
