{"title":"[Description of an intervention to prevent pressure-induced skin lesions caused by personal protective equipment (PPE) during the care of patients with COVID-19].","authors":"Ignacia Chercoles López, Roser Terradas, Carmen Lasso de Vega Panillo, Paloma Garcimartín Cerezo, Lourdes García Zaldívar, Ana María Giménez Arnau","doi":"10.12961/aprl.2021.24.03.03","DOIUrl":"https://doi.org/10.12961/aprl.2021.24.03.03","url":null,"abstract":"<p><strong>Objective: </strong>To describe a strategy designed to avoid pressure-related skin lesions related to the use of personal protective equipment (PPE).</p><p><strong>Methods: </strong>This was a prospective descriptive observational study.</p><p><strong>Study population: </strong>healthcare professionals at the Parc Salut Mar (Barcelona) who used PPE for protection against COVID-19 between March 30 and June 15, 2020.</p><p><strong>Intervention: </strong>Each professional was provided with an individual kit to prevent PPE-induced lesions, friction, or moisture. A multidisciplinary surveillance team was deployed for the early detection of these adverse events.</p><p><strong>Results: </strong>Individual kits were provided to 1044 professionals, 32 of whom were scheduled for in-person medical assessments. Six participants (0.57%) developed Stage III or IV pressure ulcers, 24 (2.3%) had dermatitis, folliculitis, eczema and two participants did not show for their appointments. The overall rate of pressure ulcers was 0.57%; for dermatitis it was 2.3%.</p><p><strong>Conclusions: </strong>The use of preventive measures appears to be effective in preventing facial pressure ulcers induced by EPIs among healthcare professionals. The Search Equation was developed by means of the descriptors «Pregnancy», «Hazardous Substances» and «Occupational Exposure», as well as using related Entry Terms and the filters «Humans» and «Adult: 19+ years». The quality of the articles was evaluated via the STROBE questionnaire and the level of evidence and the grade of recommendation via the SIGN criteria.</p><p><strong>Results: </strong>Out of the 420 references retrieved (366 digitally and 54 manually) and after applying the inclusion and exclusion criteria, 35 articles were selected. The STROBE evaluation obtained a median of 15.32, and the SIGN criteria, a 2+ level of evidence and a C grade of recommendation. Article obsolescence was high (Burton-Kebler half-life: 22.0; Price index: 0.00%). In 25 out of 35 articles, the exposure is to organic solvents. In 22 articles, congenital anomalies were notified.</p><p><strong>Conclusions: </strong>The reviewed articles exhibited high obsolescence and a degree of evidence and recommendation that did not allow to fully ensure the validity and reliability of the observations made. The results manifested the association between the exposure to dangerous substances and the occurrence of different damages regarding the health of a pregnant worker and her offspring. Del total de casos notificados, corresponden al personal sanitario y sociosanitario unos 80 065 (2.26%) casos de COVID-19 notificados con diagnóstico posterior al 10 de mayo de 2020, siendo significativamente mayor el porcentaje entre las mujeres (77.95% o 62 416 casos) que entre los hombres (21.95% o 17 582 casos). The use of appropriate personal protective equipment (PPE) is essential to prevent this occupational disease. Surgical masks and filtering face piece (FFP) respirators are important parts of this PPE. European standard EN 149 establishes three protection levels for FFP respirators (FFP1, FFP2, FFP3), depending on the particle infiltration degree through their materials, and these, in turn, are based on their filtration effectiveness. The aim of this laboratory test is to determine and quantify the filtration and fit rate of different FFP respirators, singly and in combination with surgical masks, by performing a series of fit tests and consequently, to check whether this combination improves protection levels for healthcare workers who care for COVID-19 patients.</p><p><strong>Material and methods: </strong>Several FFP respirators and surgical masks, singly and in combination, were fit tested with a PortaCount Pro + 8038, which fulfills OSHA standards, in a series of tests performed on healthcare workers in seven different breathing situations when taking care on COVID-19 patients, in order to determine and quantify their fit to the workers' face.</p><p><strong>Results: </strong>Wearing a surgical mask together with a highly efficient FFP respirator provided increased respiratory protection. Accidents that caused an injury to any ocular structure were analysed, and both the incidence of work-related eye-injuries per 100 000 insured workers and the relative risk of suffering an eye injury were compared.</p><p><strong>Results: </strong>We observed a general decrease in work-related eye injuries in all of the communities, although with two distinct time periods. The annual percentage change decrease occurred in all communities until 2013. From 2013 to 2018, however, this decreasing trend was only observed in Asturias, Castilla-La Mancha, Canary Islands, Madrid, Murcia and Navarra. Castilla-La Mancha was the Autonomous Community with the highest incidence (886.56/100.000), as well as the highest relative risk of suffering an eye injury (RR 2.66; IC 95% 2.58 - 2.75). In the different possible future of work scenarios, factors related to globalization, demographic changes, the massive use of tech - nology, the digitization of a significant number of processes, the multidisciplinary nature of the approach to worker care and some approaches close to public health, are driving a new approach to occupational health and safety, while eliciting an ethical debate.
