This study examines fulfillment of development targets across all registries for cancer and non-cancer diseases at national, regional, and hospital levels.</div></div><div><h3>Methods</h3><div>Annual summarized data from Danish clinical quality registries (2019–2023) were accessed. The proportion of fulfilled development targets was presented overall and in strata according to registries covering cancer or non-cancer diseases, indicators describing processes or results, and established or supplemental indicators. Regional differences and changes over time were evaluated.</div></div><div><h3>Results</h3><div>The data evaluated 13,305,861 patient pathways and 697 indicators from 65 registries. Overall, 57% of hospital-level development targets were fulfilled (median 58%, IQR:44–76%). The 24 cancer registries fulfilled 63% of targets (median 60%, IQR:49–76%), while the 41 non-cancer registries fulfilled 54% (median 54%, IQR:38–75%). Fulfillment was higher for result indicators (68%) than process indicators (44%), and for established (56%) than supplemental indicators (44%). Regional fulfillment ranged from 50% (Region Zealand) to 64% (the Central Denmark Region). Target fulfillment increased by five percentagepoints during the study period.</div></div><div><h3>Conclusions</h3><div>The Danish clinical quality registers varied in the approach to defining development targets, ranging from very conservative to very ambitious. The development targets were more ambitious for registries of non-cancer diseases than cancer diseases, more ambitious for process- than for result indicators, and more ambitious for supplemental compared to established indicators.</div></div><div><h3>Policy Summary</h3><div>Setting clinical quality improvement targets requires balancing ambition with realism. The ambitious target setting for supplemental (often new) indicators and process indicators is expected, as these have a high potential for further improvement. The data suggest that the Danish clinical quality registries in cancer diseases could benefit from raising some of the development targets.</div></div>","PeriodicalId":38212,"journal":{"name":"Journal of Cancer Policy","volume":"47 ","pages":"Article 100726"},"PeriodicalIF":2.0,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147322373","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Two parallel scenarios were modelled: (i) a base-case scenario reflecting individuals diagnosed in 2022 (incident model) and those living with lung cancer from 2012 to 2021 (prevalent model), and (ii) a counterfactual scenario in which no individuals had lung cancer. The differences between these simulations were used to estimate life years, QALYs, and PALYs lost. Model inputs were derived from previously published data. The economic value of productivity losses was estimated using the 2022 Australian gross domestic product (GDP) per full-time equivalent worker, applying a 5 % annual discount rate.</div></div><div><h3>Results</h3><div>Over the twenty-year period, lung cancer is projected to result in the loss of 28,430 life years, 55,964 QALYs, and 60,310 PALYs, equating to AU$14.45 billion in lost GDP. Sensitivity analyses confirmed the robustness of the model to variations in key parameters.</div></div><div><h3>Conclusion</h3><div>Lung cancer imposes a substantial health and economic burden among working-age Australians. This study aimed to analyze the national trends in consumption and expenditure of targeted anticancer therapies.</div></div><div><h3>Methods</h3><div>A retrospective analysis of national data from 2010 to 2021 was conducted to assess expenditure, consumption volume, therapeutic class distribution, and market authorization status.</div></div><div><h3>Results</h3><div>Over the study period, two distinct trends were observed in the use of targeted anticancer therapies. The volume number has increased markedly, with an increase in expenditures by 88% during the 2010–2017 period. Monoclonal antibodies dominated the market (70% of total spending in 2017). Then, between 2017 and 2020, the total expenditure decreased by 15% despite a 34% increase in unit consumption, and the market share of monoclonal antibodies (56%) and tyrosine kinase inhibitors (44%) became nearly comparable. These trends can be explained by the analysis of the number of market authorizations granted which increased during this period as well as the uptake of biosimilar and generics introduced in 2014 and the local manufacturing.</div></div><div><h3>Conclusion</h3><div>The Tunisian experience highlights the challenge of accessing targeted anticancer therapies while maintaining financial sustainability. Key strategies for ensuring sustainable and fair access to these innovative drugs include implementing pharmacoeconomic strategies, conducting continuous evaluations, introducing strategic market price revisions, price negotiations policies and promoting local manufacturing to ensure sustainable and equitable access to targeted anticancer therapies.</div></div><div><h3>Policy summary</h3><div>The growing use of targeted anticancer therapies in Tunisia has intensified financial pressures. This paper provides an analysis of the current national landscape of these drugs to inform policy solutions aimed at improving access and ensuring sustainability. Based on these findings, it proposes measures related to pricing, reimbursement, pharmacoeconomic evaluation, and the promotion of biosimilars and local manufacturing to support more sustainable and equitable access.</div></div>","PeriodicalId":38212,"journal":{"name":"Journal of Cancer Policy","volume":"47 ","pages":"Article 100720"},"PeriodicalIF":2.0,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"146259338","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
