Given the complex multitude of causes that may generate such condition, a therapeutic algorithm is mandatory when attempting reconstruction. Severity, timing, patient adherence to a rehabilitation program, status of ipsilateral and contralateral facial nerves and particularities of each patient are all criteria which should be accounted when choosing a treatment option. After initial assessment, a variable treatment panel is available based on condition type include medicamentous therapy, rehabilitation program, dynamic and static procedures surgical procedures, having as primary aim functional restoration achieving aesthetic balanced facial features. This paper summarizes current knowledge in facial paralysis reconstruction and presents an algorithmic approach that eases decision making and therapeutic strategy.","PeriodicalId":380281,"journal":{"name":"Medicina Moderna - Modern Medicine","volume":"28 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-12-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125007784","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In liver transplantation, graft implantation includes 4 anastomoses: cavo-cava, portal, arterial and biliary. The Gordon patch represents the plasty performed between the aortic patch of the celiac trunk and the aortic patch of the superior mesenteric artery. It only applies to anatomical variants of the hepatic artery, namely when the common hepatic artery arises from the celiac trunk and the right hepatic artery arises from superior mesenteric artery. The present article describes the aspect of the surgical techniques through the experience at the Digestive Diseases and Liver Transplantation Center, Fundeni Clinical Institute, Bucharest, Romania.","PeriodicalId":380281,"journal":{"name":"Medicina Moderna - Modern Medicine","volume":"18 5 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-12-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"123357224","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

With a crude incidence rate of up to 30 per 100,000 pregnancies, high morbidity and mortality, research is needed to standardize prevention, diagnosis, treatment, and management of these cases. We present stroke particularities during pregnancy and puerperium and the current information from published articles regarding the latest developments in medical conduct. The mechanisms of stroke in pregnancy differ from those in the general population and are mostly in relation to the physiological changes during pregnancy. Stroke during pregnancy and post-partum is a rare but debilitating occurrence; with limited evidence and no clear guidelines, the treatment should be always individualized, and the risk-benefit ratio should always be considered.","PeriodicalId":380281,"journal":{"name":"Medicina Moderna - Modern Medicine","volume":"63 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-12-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114230976","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Immune system activation is highly related to type 2 diabetes incidence, progression, adaptive and innate immunity is involved in the inflammation. Vitamin D insufficiency causes insulin resistance as well as glucose intolerance, which are symptoms of significant vitamin D deficiency. Increased inflammatory biomarkers and insulin resistance are prominent symptoms of this illness. Materials and methods: Serum specimens were taken from 120 people, with 70 patients (35 males and 35 females) with type 2 diabetic individuals and 50 healthy controls (25 males and 25 females) divided into two groups. Patients in the study population ranged from (18-50) years old and were hospitalized at Ballad and Salah Aldeen General Hospitals in Salah Aldeen province from August 2019 to February 2020. Results: Vitamin D levels were considerably reduced in females and male diabetic patients compared to controls, while leptin levels were elevated with ALP levels that were slightly diminished in diabetic males compared to controls but higher in diabetic females. Both C-reactive protein and serum uric acid levels were significantly higher in male and female diabetic patients compared to controls. In addition, these were increased in male diabetic individuals when compared to female diabetic individuals. Conclusion: In type 2 diabetes individuals, severe vitamin D deficiency and increased leptin cause inflammation and complicate bone metabolic illness.","PeriodicalId":380281,"journal":{"name":"Medicina Moderna - Modern Medicine","volume":"2 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-12-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121252168","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: A randomized sample of 360 human individuals, including 110 cadavers with another 250 CT scan cases, was achieved from February 2020 to November 2021. Results: The agger nasi was the most common type of cell demonstrated by 81.8% in cadaveric cases and 94% in CT cases. The frontal bulla cell presents just above the ethmoidal bulla and may produce convexity on the frontal sinus floor. Its prevalence was 10.9% in cadaveric cases and 22.8% in CT cases. The suprabullar cell is on top of bulla ethmoidalis and represents 7.3% of cadaveric cases and 14.8% of CT cases. Concha bullosa presents as a comprehensive pneumatization in the middle turbinate. Its prevalence was 34.5% in cadaveric cases and 44% in CT cases. The Haller cell reveals as a pneumatized air-filled cell shown at the lower medial side of the orbit. It was in 29% of cadaveric cases and 42% of CT cases. In addition, the sphenoethmoid (Onodi) cell is a posterior ethmoidal cell with a prevalence of 41.8% in cadaveric and 64% in CT cases. Also, some recent studies recommend the safe use of single aortic infusions in the case of multiple organ sampling from the brain-dead donor. In order to test the effectiveness of this method we made a retrospective study between 2017 and 2019 at the Center for General Surgery and Liver Transplantation of the Fundeni Clinical Institute, Bucharest, Romania. First of all, we performed a comparative analysis between the single aortic sampling group and the group in which we used the standard sampling. Awareness regarding infection prevention and control techniques are important to reduce the burden of such infections, ensuring better quality healthcare. Infection control education is a core component of infection control programs. Objectives: To assess the knowledge and awareness of Medical students in a tertiary care teaching hospital in Central India and to evaluate the impact of educational interventions in eliminating any existing gaps in the same. Methods: This interventional study based on self-administered questionnaires (Google forms) involved fifty medical Students who were administered a pre-structured validated questionnaire as pre-test and post-test before and after an induction training program on infection prevention & control measures. The impact of the educational intervention was evaluated by determining the learning gain. Result: The study reveals a highly significant improvement in knowledge levels after training sessions (p<0.0001). Before training most of the study subjects (68%) showed poor levels of knowledge, 22% exhibited moderate levels while only 10% were found to have good levels of knowledge. After training 36% of trainees showed good levels of knowledge, 40% exhibited moderate levels while only 24% were left with poor knowledge. Conclusion: While evaluating the impact of training in this study, we have found a statistically significant absolute learning gain and a medium level of class average normalized learning gain. These findings prove the effectiveness of such targeted training sessions as an important strategic tool in preventing healthcare-associated infections.","PeriodicalId":380281,"journal":{"name":"Medicina Moderna - Modern Medicine","volume":"23 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-09-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"122280612","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Data from the clinical experience of non-small cell lung carcinoma (NSCLC) treatment demonstrate the benefit of tyrosine kinase inhibitors (TKIs) in cases of EGFR mutation. The next generation sequencing technique (NGS) allows the identification of hot spots involved in mutations, exon 20 insertion being associated with the unfavorable response. Exon 20 insertions are more common in head and neck squamous cell carcinoma (HNSCC) compared to NSCLC, which could explain a resistance to targeted therapy in head and neck cancers. Taking into account the data reported in the NSCLC, Amivantamab, a bi-specific EGFR-MET antibody with potential immune cell modulation of activity, but also other innovative therapies validated in exon 20 EGFR mutation could be part of the therapy of sino-nasal cancer, but also of other HNSCC sites exon 20 mutant EFGR.","PeriodicalId":380281,"journal":{"name":"Medicina Moderna - Modern Medicine","volume":"35 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-09-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"123511921","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Thyroid cancer is the most frequent endocrine malignancy and despite the fact that differentiated thyroid carcinoma (DTC) has an excellent survival rate (>90% 10-year survival), persistent and recurrent disease is still an issue nowadays. Epidemiological studies confirmed lower levels of vitamin D in patients with DTC and correlation of 25-hydroxy vitamin D3 [25(OH)D3] status with clinicopathologic features and poor prognosis, being considered a biomarker for aggressiveness.Vitamin D, through it active form 1α,25-dihydoxyvitamin D3 [1,25(OH)2D3] and its receptor (VDR) exerts genetic changes to both healthy and neoplastic cells thus controlling their proliferation, differentiation and apoptosis. Also, vitamin D binding protein (DBP) has been recently discovered to have many different biological functions.
