Kassamy","doi":"10.21608/ejentas.2021.51633.1290","DOIUrl":"https://doi.org/10.21608/ejentas.2021.51633.1290","url":null,"abstract":"Background: There is a lot of controversy about the role of adenoidectomy and myringotomy with ventilation tubes insertion (MVT) in management of otitis media with effusion (OME). In a previous study we found that endoscopic-assisted peritubal adenoidectomy (EPA) resulted in cure of most patients without the need of MVT. We adopted this technique and modified it in the last 20 years. Aim: To describe and evaluate the results of EPA in management of OME in both new and recurrent cases after previous surgery, and whether we can avoid MVT or not. Patients and Methods: Our techniques for EPA and MVT were described. Three studies were performed, including 282 patients. Study 1: randomized (prospective) study included 110 new non-operated cases. Study 2: Retrospective study, included 120 new non-operated patients with selected criteria. Study 3: Recurrent cases: It included 52 patients after previous surgery. Each study included 2 subgroups: a- EPA. b- EPA+MVT for comparison. Results: There were no statistically significant differences between the 2 subgroups in the three studies. EPA had the same success rate as compared with EPA+MVT. Complications were recorded and discussed. Conclusion: Endoscopic examination and removal of peritubal adenoid remnants were major cornerstone step in management of OME. Patients and Methods: Three groups of children were enrolled in this case-control study with an age range of 3- 8 years. They were selected from pediatric and phoniatric clinics in Egypt. Control group (CG): thirty normal children. Case groups were divided into GroupI: Nineteen Delayed language development children had ID only, Group II: Thirty Delayed language development children had iron deficiency anemia (IDA). Groups were subjected to (1) Full history taking (focusing on nutritional history). (2) CBC by automatic cell counters. (3) Iron status through estimation of serum ferritin level by ELISA. (4) Assessment of cognitive functions by assessment of Non- verbal Intelligent Quotient (NV-IQ) using Stanford Binet 5 th Edition measuring (fluid reasoning, knowledge, quantitative reasoning, visual-spatial processing, and working memory). (5) Assessment of language-by-language test. Results: Under the umbrella of faulty eating habits, there was a direct correlation between the NV-IQ and the deterioration of iron status with their consequence on delaying language development. The efficacy of the materials regarding voice outcomes (voice handicap index, computerized speech lab) and duration will be statistically analyzed. Patients and Methods: A prospective study conducted on 38 randomly selected patients with unilateral vocal fold paralysis presented to ORL outpatient clinic at faculty of medicine – Cairo University randomized to receive either hyaluronic acid (HA) perfecta deep ® group A and autologus fat for group B. Postoperative evaluations were made based on Auditory Perceptual Assessment of Voice (APA), Computerized Speech Lab (CSL), jitter, shimmer, noise to harmonic ratio, patient satisfaction, phonatory gap and the need for another injection. Results: One month postoperatively, both groups showed significant improvement in CSL, APA and phonatory gap while six months postoperatively there was significant improvement in favor of group B. No serious adverse events were observed. 18 patients had been re-injected in group A while only 5 had been re-injected in group B due to insufficient voice. Conclusion: No side-effects were found for either hyaluronic acid or fat groups after injection. Both treatments after one month resulted in significantly improved voice as rated by the patients and significantly improved glottal closure. Otoacoustic emissions are used to assess cochlear function. Inner ear disorders commonly affect both cochlea and vestibular labyrinth. Posturography is used to evaluate the incorporation of sensory inputs; visual, vestibular and somatosensory which maintain posture and can be used in patients with cervicogenic dizziness. Objective: Analyze audiological, vestibular and postural findings in patients with spondylo-degenerative changes of the cervical spine with and without dizziness. Patients and Methods: This study was carried out on 70 patients with spondylo-degenerative changes of the cervical spine; 35 associated with dizziness and 35 without dizziness as a control group. Radiological assessment of the cervical spine was done and cervical degenerative index was used to assess severity. Basic audiological evaluation and videonystagmography were done to exclude peripheral and central vestibular lesions. Self-report of dizziness was measured using Dizziness Handicap Inventory. Otoacoustic emissions and posturography were assessed in all patients. Results: CDI total score was significantly higher in patients with dizziness. A positive correlation between CDI score and DHI scores was found. Lower OAES amplitudes were found in the patients with dizziness. SOT scores were lower in dizziness group compared to no-dizziness group. Conclusion: Severity of cervicogenic dizziness could be associated with severity of radiographic findings. Two possible mechanisms may have a role in cervicogenic dizziness include vascular compression mechanism, which was explained by otoacoustic emissions results. This study aimed to assessed the outcomes of intraoperative repair of the recurrent laryngeal nerve. Patients and Methods: In the period of the study we retrived the data of patients who had been underwent thyroid surgery and they sufferd from RLN injery or scarification with immediate intraoperative nerve repair. This retrospective study has been conducted between 2012 and 2020, 813 patients underwent thyroid surgery, 27 (3.3%) of them diagnosed with RLN injury and divided into 2 groups: group A (n = 9), with intraoperative proof of iatrogenic injury of the RLN, and group B (n = 18), in which malignant invasion was diagnosed intraoperative or recognized during thyroidectomy with a therapeutic transection. Immediate microsurgical reconstruction of the RLN was performed. The evaluation was performed at 3, 6, and 9 months post-surgical repair utilizing aspiration and voice improvement as subjective tests, fiberoptic direct laryngoscopy as an objective test. Results: Aspiration significantly improved in both groups ( p < 0.05 ). Voice quality improved in both groups but better in group A ( p = 0.02 ). Hyponatremia can be induced by excessive water drinking due to dysgeusia. Case report: Thirty-two years old Indian male patient admitted to Mesaieed Hospital at Hamad Medical Corporation, Qatar. The patient was admitted during the second wave of pandemic because of mild to moderate Covid-19 pneumonia. The patient experienced cough and fever associated with hyposmia and dysgeusia, the later manifested by a bitterness sensation of tongue. The patient received medications according to the protocol, after two days of treatment his blood laboratory results showed hyponatremia (129 mmol/L) compared to the initial reading. Hyponatremia workup excluded the syndrome of inappropriate antidiuretic hormone secretion (SIADH). History revealed that the patient was drinking large amounts of water, around 4-5 liters/day because of bitterness sensation of tongue, the provisional diagnosis was dilutional hyponatremia, fluid restriction up to 1.5 liter/ day which showed dramatic improvement of his sodium blood level. Conclusion: Hyponatremia is not only secondary to SIADH in Covid-19 patients, but it can be due to other etiologies. Patients and Methods: Stuttering severity index-Arabic version (ASSI) and the Brief Social Phobia Scale (BSPS) were applied on 33 Egyptian sample who stutter with age range from15-23 years old, 14 subjects were adolescents and 19 were adults. Twenty-three were males and ten females. The sample included3 with very mild stuttering, 3 mild stuttering,10 moderate stuttering, 12 severe stuttering and 5 very severe stuttering according to the stuttering severity index-Arabic version (ASSI). Results: The Pearson’s Correlation test revealed a strong correlation between fear, avoidance, physical symptoms and stuttering severity in the whole sample, p value = 0.000, p value =0.001, p value =0.001respectively. As well as there was a strong correlation between fear, avoidance, physical symptoms and stuttering severity in the adolescent’s sample, p value = 0.003, p value =0.010, p value =0.004 respectively. There was a strong correlation between fear, avoidance, physical symptoms and stuttering severity in the adult’s sample, p value = 0.013, p value =0.042, p value =0.078 respectively. Conclusion: These findings provide a strong correlation between fear, avoidance, physical symptoms, and stuttering severity in the whole sample. The incidence is slightly higher in female than male. Bilateral choanal atresia in newborns caused respiratory distress, as they were an obligate nasal breather. They were rarely survived without emergency treatment. A transnasal choanoplasty was the mainstay treatment for choanal atresia. Case report: a 15-years-old male reported with bony and membranous type of bilateral choanal atresia. He initially complained of nasal obstruction and rhinorrhea. There was a history of respiratory distress that was alleviated with crying when he was born. The nasal endoscopic and CT-scan examinations showed septal deviation and bilateral choanal atresia of bony and membranous type in the right and left nasal cavity, respectively. The patient underwent septoplasty before choanoplasty. A stent was used to prevent stenosis. However, one month after choanoplasty, a total restenosis of choana was observed in the right nasal cavity and followed by mild stenosis in left nasal cavity. Further revision of choanoplasty with steroid diluted tamponade and steroid injection in the choanal circumference were done to repair the restenosis. The post-operative evaluation showed a mild restenosis with a patent choana. Conclusion: Treatment of bilateral choanal atresia with a transnasal endoscopy approach facilitates choanoplasty with minimal complications. Elfiky","doi":"10.21608/ejentas.2022.130339.1489","DOIUrl":"https://doi.org/10.21608/ejentas.2022.130339.1489","url":null,"abstract":"Introduction: Clinicians in dysphagia field use validated questionnaires extensively in clinical and research settings to detect oropharyngeal dysphagia and provide information for diagnosis and management decisions. The study aims to develop an Arabic version of the Sydney Swallow Questionnaire (A-SSQ) and assess its reliability, consistency, and validity in individuals with oropharyngeal dysphagia who are speaking the Arabic language. The questionnaire was administered to 15 participants with oropharyngeal dysphagia of various etiologies. It was a case control study and the sample was 100 adults aged more than 18 years old; 50 cases with different etiologies and 50 healthy controls. Both the cases selected and controls received a copy of both the validated Arabic version of Eating Assessment Tool (A-EAT-10) as well as a copy of the Sydney Swallow Questionnaire translated into Arabic to answer their questions. Retesting was done on all cases to determine the reliability of test-retest, with an interval of 14 days. Validity was proven to be high. In addition, the reliability of tests was significant. Results: The Arabic version of SSQ is found to be a reliable and valid tool to be used to screen and measure the subjective severity of oropharyngeal dysphagia. It showed excellent internal consistency among cases (Cronbach’s α = 0.924), excellent test-retest reliability (Intra-class correlation coefficient = 0.99). A significant difference was found in the scores of A-SSQ between controls as well as the oropharyngeal dysphagia cohort. ABSTRACT Background: Cochlear implantation (CI) became an effective procedure in restoring hearing in patients with severe to profound hearing loss (HL). It can help youngsters who do not benefit from hearing aids improve their auditory skills and speech perception. The outcome depends on several factors which affect it one at a time or in combination. Aim: This study was designed to document factors that might affect the outcome of Cairo University’s CI program. Patients and Methods: This prospective study was conducted on fifty children with bilateral severe to profound sensorineural HL who underwent unilateral CI at the CI Unit, Kasr Al-Ainy Hospital, Cairo University. All patients were subjected to careful history taking, full medical examination, audiological and preoperative investigations, phoniatric and IQ assessment then subjected to audiological and language assessment after one year of using the device. Results: After one year of CI, significant improvement in hearing was found with a significant association with the duration of using hearing aids. The language age and vocabulary size were significantly improved. Regarding vocabulary size, it was significantly improved and associated with the preoperative IQ level and preoperative vocabulary size. There was a statistically significant improvement in categories of auditory performance scores with a significant association with the postoperative vocabulary size. Conclusion: CI is an effective and safe treatment in children with sensorineural HL.
