We aimed to systematically assess the methodological and reporting quality of studies conducted on acupuncture for experimental cerebral ischemia/reperfusion injury (CIRI).</p><p><strong>Methods: </strong>A comprehensive search in six databases was performed for animal research concerning acupuncture for CIRI. Two authors independently selected articles, extracted data, and assessed the methodological and reporting quality of identified articles using the Systematic Review Center for Laboratory Animal Experimentation (SYRCLE) tool, and Animal Research: Reporting In Vivo Experiments (ARRIVE) guideline, respectively.</p><p><strong>Results: </strong>A total of 24 studies were identified. Only 1 article (4%) achieved a decent overall rating in using SYRCLE (percentage of items with \"low risk\" ⩾50%). Of the 22 items on the SYRCLE tool, only 8 items (37%) were rated as \"low risk\" of bias in more than 50% of the included studies. Of the 39 items of ARRIVE, 20 (51%) items were rated as \"low risk\" in more than 50% of the included studies.</p><p><strong>Conclusions: </strong>The methodological and reporting quality of included studies was generally low, which demands further improvement. The present study was designed to investigate the hypnotic effect of acupuncture stimulation at HT7 on caffeine-induced sleep disorders and locomotor activity in rats. We also evaluated neuronal activity changes in the arousal region of the basal forebrain.</p><p><strong>Methods: </strong>Rats received intraperitoneal injections of caffeine, and then electroencephalogram power spectrum analysis and locomotor activity measurements were performed. Stimulation at HT7 was performed using a mechanical acupuncture instrument (MAI) before caffeine injection, and its effects on caffeine-induced changes in sleep architecture, locomotor activity and c-Fos expression were examined.</p><p><strong>Results: </strong>Caffeine injection (7.5 mg/kg) produced a significant decrease in slow-wave sleep and an increase in wake time compared with saline injection. Caffeine injection also increased locomotor activity and c-Fos expression in the medial septum-vertical limb of the diagonal band of Broca (MS-VDB), one of the arousal regions of the basal forebrain. Stimulation at HT7 with the MAI alleviated the caffeine-induced sleep disturbance and the increase in locomotor activity. This study examined the effects of acupuncture versus cognitive behavioral therapy for insomnia (CBT-I) on serum BDNF and sleep outcomes in cancer survivors with insomnia.</p><p><strong>Methods: </strong>This was an exploratory analysis of a randomized clinical trial (n = 160) comparing acupuncture versus CBT-I for cancer survivors with insomnia. Interventions were delivered over 8 weeks. Outcomes were assessed at baseline and week 8. Serum BDNF was evaluated with enzyme-linked immunosorbent assay (ELISA). Sleep was evaluated with the insomnia severity index and consensus sleep diary. Pearson correlations between BDNF and sleep outcomes were calculated. Data analysis was limited to 87 survivors who provided serum samples.</p><p><strong>Results: </strong>Among 87 survivors, the mean age was 61.9 (SD: 11.4) years, 51.7% were women, and 24.1% were non-White. Mean serum BDNF did not significantly increase in acupuncture (n = 50) or CBT-I (n = 37) groups. When analysis was restricted to patients with low baseline BDNF (i.e. levels below the sample median of 47.1 ng/mL), the acupuncture group (n = 22) demonstrated a significant 7.2 ng/mL increase in mean serum BDNF (P = 0.03), whereas the CBT-I group (n = 21) demonstrated a non-significant 2.9 ng/mL increase (P = 0.28). Electroacupuncture (EA) could inhibit pain development through the peripheral dorsal root ganglia; however, it is unclear whether it can mitigate the transition from acute to chronic pain by attenuating protein expression in the p38 MAPK (mitogen-activated protein kinase)/tumour necrosis factor alpha (TNF-α) pathway in the spinal dorsal horn.</p><p><strong>Aims: </strong>We aimed to determine whether EA could prevent the transition from acute to chronic pain by affecting the p38 MAPK/TNF-α pathway in the spinal dorsal horn in a rat model established using HP.</p><p><strong>Methods: </strong>We first randomly subdivided 30 male Sprague-Dawley (SD) rats into 5 groups (<i>n</i> = 6 per group): control (N), sham HP (Sham-HP), HP, HP + SB203580p38 MAPK (HP+SB203580), and HP + Lenalidomide (CC-5013) (HP+Lenalidomide). We then randomly subdivided a further 30 male SD rats into 5 groups (<i>n</i> = 6 per group): Sham-HP, HP, sham EA (Sham EA), EA (EA), and EA + U-46619 p38 MAPK agonist (EA+U-46619). We assessed the effects of EA on the mechanical paw withdrawal threshold and p38 MAPK/TNF-α expression in the spinal dorsal horn of rats subjected to chronic inflammatory pain.</p><p><strong>Results: </strong>Rats in the EA group had reduced p38 MAPK and TNF-α expression and had significantly reduced mechanical hyperalgesia compared with rats in the other groups.</p><p><strong>Conclusion: </strong>Our findings indicate that EA could increase the mechanical pain threshold in rats and inhibit the transition from acute pain to chronic pain. Walker et al.1 summarized diagnostic and treatment steps including the ENTUK flowchart for UK patients with loss of smell during the COVID19 pandemic. Here, we report the case of a 55-year-old nurse (D.B.) with anosmia and PVOD for 18 months with no improvement despite steps. She reported a 95% improvement after acupuncture (treated pre-COVID-19 pandemic). On account of the hypertrophic adenoid, symptoms of AH mostly manifest as nasal obstruction, snoring, apnea, mouth breathing and lack of attention due to chronic hypoxia. Nasal endoscopy and lateral neck radiography are the two most common diagnostic methods used to evaluate severity and the necessity of surgery for AH.1 The most recognized therapies for AH include glucocorticoid nasal sprays, oral cysteinyl leukotriene receptor antagonists and adenoidectomy.2 Nevertheless, adenoidectomy carries risks of general anesthesia and complications like bleeding, adenoid tissue recurrence and postoperative respiratory problems. Evidence for the safety and long-term efficacy of treatment using medicine is limited. Recently, acupuncture has been widely used as a therapeutic modality for various otolaryngological disorders. There has been an increasing number of complaints related to mental health and psychiatric illnesses since the beginning of the pandemic, especially among healthcare workers (HCWs).1 In this context, several integrative care approaches have been adopted in many countries in order to help HCWs face mental health complaints related to or aggravated by the COVID-19 pandemic.2 Acupuncture is a therapeutic modality that is increasingly used in the context of mental health care.3 This brief report describes the experience of using acupuncture in an integrative initiative conducted by the psychosocial care unit at the University Hospital of the Federal University of Santa Catarina (HU-UFSC) to provide care to HCWs with mental health disorders that have emerged during the pandemic. The acupuncture service at HU-UFSC has offered outpatient and inpatient care as well as educational, research and extension activities since it was established over 25 years ago. Acupuncture practice is integrated into hospital care and well accepted by the hospital’s health professionals.4 Due to the COVID-19 pandemic restrictions, most elective outpatient care activities were temporarily suspended in March 2020. The university hospital implemented a contingency plan to cope with the new demands that emerged as a result. Within this contingency plan, the “mental health support team to HU-UFSC workers during the COVID-19 pandemic” was established. It began its activities on 30 March 2020, under the coordination of the psychosocial care unit with the participation of the acupuncture service. The initiative offered a comprehensive integrative approach that included acupuncture, psychiatry, psychology and homeopathy. The target population included not only HCWs but also students and residents from different health fields, as well as workers from administrative areas and outsourced professionals involved in the activities of the hospital. They spontaneously sought support as they presented evolving symptoms or worsening of chronic conditions due to the repercussions of the pandemic. The project’s objectives were: (1) to promote well-being by assisting in coping with stressful situations that can lead professionals to greater risks and damage to psychological health; (2) to minimize anxiety and fear, promoting feelings that favor a healthier work environment and that promote satisfactory responses to the crisis; (3) to promote self-care strategies and empathic behavior; and (4) to therapeutically approach situations of psychological distress and other associated physical complaints. From April 2020 to July 2020, the acupuncture team completed a total of 340 appointments. A survey of chief complaints reported by patients showed that 83% of patients had complaints related to mental health. The analgesic effect of acupuncture has been well-documented.</p><p><strong>Objective: </strong>The aim of this study was to investigate the involvement of chemokine CXCL1 in the serum on manual acupuncture (MA)-induced antinociception.</p><p><strong>Methods: </strong>Rats with inflammatory pain of the right hind paw were induced by intraplantar (i.pl.) administration of complete Freund's adjuvant (CFA). After wards, the CFA-injected rats were treated daily with MA at ST36 from Day 1 to Day 7, and thermal nociceptive thresholds (paw withdrawal latency; PWL) were analyzed. The concentration of CXCL1 in the serum of the rats was measured by enzyme-linked immunosorbent assay (ELISA) after the first and the last MA treatment. Subsequently, the rats were injected with two doses (5 or 10 μg) of recombinant CXCL1 through the tail vein daily from Day 1 to Day 7 or injected with two doses (6.4 or 16 μg) of anti-CXCL1 antibody using the same methods and course at 30 min before MA, and the PWLs were measured again. Finally, naloxone (500 μg, 0.1 mL) was administered by i.pl. injection into the inflamed paw 5 min before the last MA treatment or last injection of recombinant CXCL1.</p><p><strong>Results: </strong>MA significantly increased the PWLs and upregulated the expression of serum CXCL1 in the CFA-injected rats. Without acupuncture, repeated tail vein injection of recombinant CXCL1 showed an analgesic effect on CFA-induced inflammatory pain. Conversely, the neutralization of serum CXCL1 by anti-CXCL1 antibody decreased MA-induced antinociception in a time-dependent manner. Anti-CXCL1 antibody injected just once before the first MA did not affect MA-induced antinociception. The analgesic effects of MA and recombinant CXCL1 were reversed by an i.pl. injection of naloxone.</p><p><strong>Conclusion: </strong>This study indicates MA at ST36 had an analgesic effect on inflammatory pain and found a novel function of CXCL1. Increased serum CXCL1 had an antinociceptive effect on inflammatory pain induced by CFA. CXCL1 in serum appeared to be a key molecule involved in the peripheral mechanism of MA-induced antinociception.
