Female sex is considered a risk factor for dementia. A limited number of studies have studied the role of gender differences on the cognitive function of type 2 diabetes mellitus (type 2 DM) patients with varying results. This study aims to determine the impact of gender differences on the cognitive impairment (CI) risk in type 2 DM patients. Material and methods. This study used a case-control research design. Samples were recruited consecutively. Data were analyzed by Chi-square test and logistic regression, expressed in odds ratio (OR) (95% CI) with a significance level of P <0.05 Outcomes. Sixty patients with diabetes met the eligibility criteria. The bivariate analysis found that female gender [OR=3.75 (95% CI 1.23-11.38), P=0.017], low education level, and hypertension were associated with CI in type 2 DM. Independent risk factors for CI in type 2 DM from the multivariate analysis were low education level and hypertension. Conclusions. {"title":"Experience in the use of a multimodal staging system for Parkinson’s disease in clinical practice","authors":"I. Khubetova, Y. The aim of the study was to evaluate the information content of the multimodal staging system for Parkinson’s disease in clinical practice. Material and methods. The study was conducted on the basis of the Regional Clinical Hospital (Odesa). 364 patients with Parkinson’s disease were examined. Clinical manifestations were assessed according to the recommendations of Levin O.A. et al. (2019). Outcomes. Average age of the patients was 64.6±0.5 years, the sample was dominated by men – the gender ratio was 1/1.14. Left-sided lesions were noted in 126 (34.6%) patients, right-sided – in 127 (34.9%), bilateral – in 111 (30.5%). The akinetic-rigid form was observed in 92 (25.3%) cases, trembling – in 27 (7.4%) cases, mixed rigid-trembling in 157 (43.1%) cases, tremulous-rigid in 92 (7.4%) cases). Stage 1 according to Hoehn-Yahr was determined in 33 (9.1%), stage 1.5 – in 58 (15.9%), stage 2 – in 104 (28.6%), stage 2.5 – in 36 (9.9%), stage 3 - in 102 (28.0%), stage 3.5 – in 6 (1.6%), stage 4 – in 25 (6.9%) cases. No cases of stage 5 were reported. 65.4% of patients had cognitive impairments. 121 (33.2%) patients had pain syndrome of varying severity. Vegetative manifestations of varying severity were observed in 201 (55.2%) patients, affective manifestations – in 170 (46.7%) patients. Conclusion. Measure whether the hematocrit can be the disability predictor in 30 days post ischemic stroke. Methods. This study is a cohort retrospective using secondary data from medical records of patients with stroke ischemia. The total sample in this study is 54, consisting of group with hematocrit <43% and group with hematocrit >43.5%. Disability is measured with modified Rankin Scale (mRS) which consist of independent group (score 0-2) and dependent group (score 3-5); and Barthel Index score which consist of independent group (score >12) and dependent group (score <12) using Mann Whitney test. Results. The mean age of the population was 60.333+10.97338 which the most age group that suffering from stroke is late elderly (56-65 years old). Majority of subjects is men (70.37%). The mean of hematocrit value 42.62+3.550467%, there are 34 subjects (62.96%) in group with hematocrit <43.5% and 20 subjects (37.04%) in group with hematocrit >43.5%. There are no significant associations between hematocrit and mRS (p-value = 0.735) also BI (p-value = 0.49), but there are significant associations between age (p value = 0.043) and sex (p-value = 0.002). Conclusion. Awake surgery is currently a safe and reliable alternative for identifying and preserving functional areas. However, this protocol has evolved over time to minimize the occurrence of electrical stimulation-induced seizures and postoperative deficits. Objective. The aim of this study is to highlight both internal and external factors influencing the language and sensorimotor thresholds of electrical stimulation mapping (ESM) during gliomas awake surgery. Material and methods. From October 2016 to April 2022, we performed a retrospective study on 74 patients underwent awake craniotomy of gliomas in two series based on ESM: group 1 and group 2. Preoperatively and three months after surgery, general performance and neurological deficits were assessed according to the type, location, and side of the tumor in correlation with ASA, BMI, Mallampati and KPS indexes. Results. The outcome shows that the median patient age was 49 years old in a range of [13-70], functional mapping was performed in language areas for 96% of patients and motor functions for 54%. In addition, 55.5% of the patients had HGG, 40.5% had LGG, and 4% had AVMs. Most lesions were in the temporal area (40.5%), followed by the frontal area (31%), 24.5% for insular area, 4% for the parietal area, and 85% of patients had left-sided lesions. The overall rate of intraoperative complications fell from 16.2% in group 2 to 1.35% in group 1. Conclusions. There is limited information on neurologic complications associated with influenza in adults. Objectives. To identify headache features and high fever in influenza outpatients with migraine in the Department of Internal Medicine by a neurologist. Methods. In 2022, a retrospective cohort study was performed to review the medical records during 2016 to 2019. A total of 64 (44 women; 53 unvaccinated; 47 type A) influenza patients were included. All patients were physically examined, and axillary temperatures were measured at around 9 am. The mean body temperature in 34 patients without headache was compared to that in 22 with headache and 8 with headache and an underlying migraine. Results. Among the 27 unvaccinated influenza type A patients (18 women) under 50 years old, the mean body temperatures were 37.79, 38.10, and 38.86 degrees C for the 12 (6 women) without headache, 10 (8 women) with headache, and 5 (4 women) with headache and a history of migraine, respectively. The mean [SD] ages were 33.58 [7.27], 37.30 [9.58], and 32.40 [7.50] years, respectively. Levene`s Test showed no equal variances between the body temperatures of the 12 without headache and those of 5 with headache and a history of migraine (P=0.007). Welch`s Test showed significant differences in their mean body temperatures (P=0.012). Conclusion. The acute or reactive hypertensive response occurs both in patients with a history of hypertension and in normotensive patients. The greatly increased BP values, not controlled therapeutically, can be followed by the increase in the volume of the cerebral infarct and hemorrhagic transformation or the expansion of the hematoma and the increase in intracerebral pressure, respectively in ischemic or hemorrhagic stroke. This paper especially refers to the therapeutic control of hypertension in various situations (conditions): 1. Laboratory emergencies; 2. Hospital treatment with out thrombolytics or thrombolysis; 3. Endovascular treatment in acute stroke; 4. BP control in hemorrhagic stroke; 5. Continuation or discontinuation of antihypertensive treatment. The aim of our study was to assess the probability of cognitive impairment (CI) in general and in separate domains, depression, anxiety and sleep disorders onset depending on their association with MRI findings in patients with multiple sclerosis (MS). Materials and methods. 137 patients with MS were enrolled into the study. All participants were divided into two groups: group A included study subjects with relapsing remitting multiple sclerosis (RRMS) and B consisted of patients with secondary progressive multiple sclerosis (SPMS). Results. Participants with progressive forms of MS had a higher risk of CI development compare to the patients with RRMS (p=0,0361). Memory decline onset depended on the presence of the brain atrophy in combination with lesions of the parietal lobe (OR=2.74 (0.85-8.77), p <0.0001) in RRMS cases, furthermore, on presence of combined demyelination in temporal lobe with corpus callosum (OR=17.33 (2.92-103.02), p=0.0006) and parietal lobe separately (OR=7.5 (1.14-49.26), p=0.0239) in patients with SPMS. Conclusions. {"title":"The use of the gold compound (AuCl4) solution as an alternative contrast medium for iodine in computed tomography (CT-Scan) imaging","authors":"M. But the iodine solution has different side effects for patients. In the present work, one of the gold compounds AuCl4 was used as an alternative contrast agent. The study used rabbits in an in vivo method to examine both the organs of the heart and the kidneys. Hounsfield unit (HU) values from computed tomography of rabbit and kidney angiography findings were found and compared with HU values for iodine contrast medium. Iodine solution is the only contrast medium currently used in the present study, gold tetrachloride (AuCl4) solution has been chosen as an alternative contrast medium in computed tomography scanning (CT-scan). It was found that using AuCl4 solution as an alternative contrast medium in rabbits enhanced the CT-scan imaging in the resolution and increasing the of heart and kidney organs in comparison with the iodine compound without any side effects. Rabbit’s heart and kidney were selected to study the effect of using 1.4 M iodine solution and 0.1 M AuCl4 as an alternative contrast medium in CT-scan. The HU value for the heart has 53 HU when used high concentration of Iodine complex, while the HU value at low concentration of alternative contrast medium of 0.1 M of gold compound has 50.5. The same results were found in the kidney organ. From these results, the resolution of the CT image is good and cleared when an alternative contrast medium (AuCl4) used.
