Aim: We assessed the utilization of autopsy by medical doctors in a southeastern part of Nigeria. Subjects and Methods: This was a cross-sectional study among medical doctors practicing in Anambra State, southeastern Nigeria. A self-administered, structured questionnaire was used for data collection, including demography, and information related to knowledge, experience, and attitude of respondents. Statistical analysis used: Data were analyzed using SPSS version 22.0. Results: Two hundred and forty-one doctors responded, 68.9% of whom were males and 48.1% consultants. More than 80% of the respondents show that autopsy is of great benefit to clinical practice, including but not limited to serving as a tool for clinical research (n = 227), identification of atypical disease presentation (n = 217), and clinical audit (n = 210). Hindrances to autopsy requests include cultural/religious belief, lack of enabling government, and hospital policies on autopsy. There is a significant relationship between undergraduate autopsy exposure and period of graduation (χ2 = 12.451; P = 0.014) and age of respondents (χ2 = 9.860; P = 0.043), but not the status of respondents (χ2 = 6.699; P = 0.350)Conclusion: Autopsy is an indispensable tool in clinical practice notwithstanding the global decline in its rate. A multifaceted collaboration involving pathologists, clinicians, educators, hospital administrators, government, and policy makers is needed to stem the tide.","PeriodicalId":374861,"journal":{"name":"Journal of the Nigerian Academy of Medicine","volume":"2 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"129187567","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Several nonoperative treatment modalities have been proposed. Of these, epidural steroid injection has been proffered as a treatment modality to patients not responding to other noninvasive treatment plans. Subjects and Methods: This is a retrospective study analyzing the efficacy of patients with lumbar disk prolapse who received interlaminar epidural steroid injection. The injectate was made up of 80 mg of methylprednisolone and 2 mL of plain bupivacaine. Result: Out of 42 patients, there were 25 males and 17 females. The mean preinjection Oswestry Disability Index (ODI) score was 60.83 ± 7.48, whereas the mean postinjection ODI score at 1 and 6 weeks, respectively, was 34.76 ± 10.06 and 35.60 ± 14.15, respectively; this was statistically significant (P < 0.05). Conclusion: Interlaminar epidural steroid injection is an effective treatment modality in patients with symptomatic lumbar disk prolapse.","PeriodicalId":374861,"journal":{"name":"Journal of the Nigerian Academy of Medicine","volume":"920 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127591045","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Educators must be well-versed in the prevalent signs and symptoms of this disease, including first aid care and associated psychosocial factors to curb this trend. Aim: To determine teacher’s knowledge of SCD and factors that affect this knowledge and care of children with SCD. Subjects and Methods: This was a descriptive survey of all Anambra State teachers invited to a health seminar at the State Education Ministry. Following a verbal informed consent, a pre-tested questionnaire was administered with information on socio demographics, school cadre of employment, marital status, and perception and care of SCD school children retrieved. The correct responses were scored on a scale of 100%. Collated data were cleaned and analyzed using Python 3.10.0. P values of less than 0.05 were considered statistically significant. Results: The average age of the 182 responders was 44.2 ± 7.14 years. The majority (90.7%) of teachers were women with post-secondary academic degrees (76.4%). Approximately 57.1% of responders were elementary school teachers. Despite the fact that only 23 (12.6%) of the teachers had attended SCD courses in the past, a good number possessed a reasonable knowledge on SCD. Although the male instructors had less knowledge than females, this difference was not statistically significant (P = 0.142). The highest educational level, prior exposure to SCD seminars, and the type of class the teacher teaches (primary or secondary) were found to impact their awareness and care of SCD patients (P = 0.003, 0.025, and 0.04, respectively). Conclusion: More seminars and health conversations on SCD should be held for teachers, with special attention to teachers who work in public schools, male teachers, and those with less than a bachelor’s degree.","PeriodicalId":374861,"journal":{"name":"Journal of the Nigerian Academy of Medicine","volume":"70 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126065815","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, the treatment of patients with visual impairment (PVI) in Nigeria is yet to embrace the biopsychosocial model thereby leaving huge unmet mental health needs. Aims: The aim of the study was to determine the burden of anxiety disorders and stress symptoms in PVI and highlight its relationship with vision-related health status. Subjects and Methods: This was a cross-sectional study involving 400 PVI using standard criteria. Screening for anxiety disorders and stress symptoms was done using the 21-item Depression, Anxiety and Stress Scale (DASS). Diagnosis of anxiety disorders was confirmed using relevant modules of the Mini-International Neuropsychiatric Inventory (MINI). The vision-targeted health status was measured using the 25-Item Visual Function Questionnaire (WHO-NEI-VFQ-25). Multivariate linear regression model was used to determine the potential predictors of general health and vision sub-scales of quality of life. Results: The prevalence (95% confidence interval [CI]) of anxiety disorders and significant stress symptoms were 29.5% (25.1%–34.2%) and 34.5% (29.9%–39.4%), respectively. Vision-related quality of life scores in the domains of general health, general vision, ocular pain and mental health were significantly lower in participants with anxiety disorders when compared with those without (P < 0.05). Conclusion: The findings of this study support the thinking that the presence of significant anxiety and stress symptoms lower the quality of life of the visually impaired. This has implications for clinical practice in the areas of routine screening for psychiatric morbidities and integration of psychosocial care into ophthalmic care.","PeriodicalId":374861,"journal":{"name":"Journal of the Nigerian Academy of Medicine","volume":"122 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"123251814","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A diagnosis of myasthenia gravis may be missed in patients presenting with ptosis without associated musculoskeletal weakness. We report a case of ocular myasthenia gravis in a 6-year-old girl whose only symptoms were bilateral ptosis and diplopia. Both worsened with exertion but there was no associated weakness of any other part of the body. A neostigmine test resulted in marked improvement in the patient’s ptosis thus confirming the diagnosis of ocular myasthenia gravis. It is concluded that ocular myasthenia gravis, though rare, may occur in children. Health workers should be aware of the possibility of this disease among pediatric age group.","PeriodicalId":374861,"journal":{"name":"Journal of the Nigerian Academy of Medicine","volume":"13 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126463383","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Diagnosis is through histopathology of symptomatic or incidentally detected thyroid nodules. The management depends on the tumor stage and risk stratification. Monitoring, surgery, radioactive iodine, radiotherapy and targeted therapy play various role in management depending on stage and risk of recurrence. Radioactive iodine is a form of iodine which is unstable and emits both gamma (γ) rays and beta (β) rays. This enables its usage in both diagnosis and therapy for well differentiated thyroid cancers that retain the physiological process of concentrating iodine. This review article highlights, in summary, the role of radioiodine in the management of well differentiated thyroid cancers and also helps create awareness among physicians in Nigeria.","PeriodicalId":374861,"journal":{"name":"Journal of the Nigerian Academy of Medicine","volume":"2 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130515402","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Aims: The study aimed to assess the relationship between the burden of care and the quality of life in carers of individuals with schizophrenia. Subjects and Methods: This was a cross-sectional study involving 246 adult participants (outpatients with schizophrenia and their caregivers in ratio 1:1) in a psychiatric hospital. Patients within age 18–65 years, and their caregiver who were older or equal to 18 years were recruited via convenience sampling. The Mini International Neuropsychiatric Interview (MINI-6), Brief Psychiatric Rating Scale, and demographic questionnaire were used to confirm the diagnosis of schizophrenia, obtain the details of the severity of psychopathology, and record the demographic data of the patients, respectively. The Zarit Burden Interview and WHO Quality of Life-Bref scale were deployed to obtain information on carers’ care burden and their subjective quality of life, respectively. Results: One in five of the caregivers’ experiences moderate-severe burden of care. Caring for unemployed patients (P = 0.03) and patients with severe psychopathology (P = 0.01), older age of caregiver (P = 0.033), and being a parent (P = 0.01) were significantly associated with high burden of care. Conclusion: The quality of life of carers of individuals with schizophrenia significantly, negatively correlated with high level of care burden. We recommend that clinicians should endeavor to address some modifiable correlates, such as reducing the patients’ illness severity and liaising with occupational therapists to empower patients with employable job skills.","PeriodicalId":374861,"journal":{"name":"Journal of the Nigerian Academy of Medicine","volume":"2 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130731214","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The disorder is characterized by chronic hemolytic events that result in increased generation of free radicals leading to excessive consumption and depletion of body antioxidants. Aim: This study evaluated the total antioxidant status (TAS) of children with SCA, aged 1–17 years, comparing findings among steady state, vaso-occlusive crisis and hemoglobin AA (HbAA) matched controls. Materials and Methods: Study participants were 50 children with SCA and 50 HbAA matched controls. Sociodemographic characteristics and clinical history were documented using pre-tested interviewer-administered questionnaires. Physical examination was conducted and blood samples collected for serum TAS estimation. Data analysis was carried out using SPSS version 21 and P-value of significance was set at <0.05. Results: SCA subjects in vaso-occlusive crisis had lower mean serum TAS level than those in steady state (7.4 ± 3.5 vs 8.4 ± 4.0mmol/L) but this difference was not statistically significant (P = 0.18). The mean TAS level of SCA patients either in steady state (8.4 ± 4.0 mmol/L) or vaso-occlusive crisis (7.5 ± 3.5 mmol/L) was higher than 7.0 ± 4.1 mmol/L for matched (HbAA) controls but the difference was also not statistically significant (F = 1.54, P = 0.09). Conclusion: This study shows that the TAS level of SCA children in steady state was higher than that of those in vaso-occlusive crisis, though not statistically significant. This may be an adaptive mechanism to counteract the oxidative stress seen in chronic inflammatory conditions like SCA.","PeriodicalId":374861,"journal":{"name":"Journal of the Nigerian Academy of Medicine","volume":"44 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132939156","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Among these, foot length has been found to be a reliable predictor of height in adult and pediatric populations. Aims: This study was designed to derive correlation equations for estimating height from foot length among a cohort of Nigerian school-age children. Subjects and Methods: Foot length, weight, and height measurements were obtained from healthy primary-school students of Nigerian descent aged 5–13 years in Benin City, Nigeria. Results: In total, 1677 subjects consisting of 778 boys and 899 girls were included in this study. The mean height for boys and girls was 134 ± 11 cm and 135 ± 13.5 cm, respectively, while the mean foot length was 21.9 ± 2 cm in boys and 21.9 ± 2.1 cm in girls. A significant correlation was observed between the length of both feet in boys and girls. No significant gender differences were observed in the foot length. A significant correlation was observed between height and the average foot length in boys and girls (P < 0.0001 for both sexes). We derived linear regression equations to determine the height from foot length for boys (height [cm]) = (4.6238 × average foot length [cm]) + 32.843 and girls (height [cm]) = (4.9276 × average foot length [cm]) + 27.373. Wilcoxon matched-pairs signed rank test determined that there was no statistically significant difference between the actual and predicted height based on the regression equations in both sexes. Conclusion: Our findings show that foot length can be used to estimate the height of school-aged Nigerian children. This finding is valuable in forensic medicine and in the estimation of height when actual height measurement is not possible.","PeriodicalId":374861,"journal":{"name":"Journal of the Nigerian Academy of Medicine","volume":"11 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131236523","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
