In the brief communication, we compared various national reports from other countries to see whether current care service for the dementia had integrative approach for the patients with hearing loss in Korea. We also discussed a need of systematic hearing care for the elderly with dementia who was also a suspected case of aged-related hearing loss because many studies had confirmed that untreated hearing loss has a negative effect on dementia and vice versa. After analyzing the national policy from several countries, we found that the systems had been well established to screen dementia patients by using various clinical tests in UK, Canada, and Japan who provided appropriate services by classifying types of the dementia. Remarkably, the hearing test was included as the testing protocol for the dementia in Canada and the experts such as speech pathologists and music therapists had worked in a non-pharmacotherapy program for the elderly with dementia in France. Unfortunately, the dementia policy of Korea needs to be refined when considering the hearing care/management in that the integrated management of hearing and dementia are important. In addition, the clinicians who work for the dementia care home need to know audiological knowledge. Methods: A total of 23 students, consisting of 10 LLD students and 13 typically developing (TD) students, were recruited for this study. All students were tested to check their ability to define and understand homonyms. Results: Comparing the two groups, the overall definition accuracy and comprehension ability of homonyms were significantly lower in the LLD group than the TD group. In addition, when comparing the accuracy of individual definitions and ability to comprehend homophones according to the conditions (dominant/subordinate meaning) between the two groups, the LLD group showed lower performance than the TD group in both abilities. Both groups were found to have more difficulty with the subordinate meaning than with the dominant meaning. LLD group has difficulties at defining homonym than TD group in both meanings. On the other hand, the LLD group showed lower performance than the TD group in their ability to comprehend the subordinate meaning. There was no difference between the two groups in the ability to understand the dominant meaning. Methods: The test list consisted of 127 initial and 94 final consonant test items with meaningful one-syllable words and was developed in two types for each difficulty level. To verify the validity of the developed phoneme perception test tool, twenty children with normal hearing were tested for the validity of the results, and the contents were verified by experts in various fields. Results: As a result of comparing the rate of correct response according to the type of test item (initial vs. final consonant) and difficulty (easy vs. difficult item), the initial and easy responses were higher than the final and hard item, respectively, so the difficulty level of the test list was appropriate. In addition, there was no statistical difference in the rate of correct answers according to the test speakers, and the content validity analysis for experts verified that the developed test list was appropriate as a phoneme perception test tool for school-age children. Conclusion: The initial and final consonant test items developed through this study consisted of one target word and three examples which are a pair of minimum confrontations with the target word and are heteromorphic test lists consisting of easy and hard types according to the difficulty of the example. Jang, Yu Gyeong Choe, Seunghee Ha","doi":"10.21848/asr.200072","DOIUrl":"https://doi.org/10.21848/asr.200072","url":null,"abstract":"정상적으로 말을 산출하기 위해서는 호흡, 발성, 공명, 조음, 운율과 같은 말 산출의 하위 기제 간에 협응과 역동적인 움직 임이 필요하다. 그러나 마비말장애, 말실행증 등 신경학적 손상 으로 인한 말운동장애나 말운동문제에 기인한 말소리장애를 보이는 아동과 성인의 경우는 말 산출기관의 협응 및 움직임상 에 어려움을 보인다. 말장애를 평가하는 임상현장에서 말운동 기술을 살펴보는 대표적인 과제로 조음교대운동(diadochokinesis, DDK)이 있다. 조음교대운동(DDK)은 조음기관의 움직 임, 속도, 정확성, 일관성, 협응도를 종합적으로 살펴볼 수 있다. 또한 호흡, 발성, 공명과 같은 다른 말 산출 하위 기제 간의 협 응 정도를 동시에 파악할 수 있기 때문에 성인 말운동장애뿐만 아니라 아동 말소리장애를 감별 진단할 수 있는 과제로 유용하 다. DDK 과제에는 1음절 자음-모음(consonant-vowel, CV)구 조를 반복하는 교대운동속도(alternating motion rate, AMR) 와 조음점이 서로 다른 2~3음절을 반복적으로 산출하는 일련 운동속도(sequential motion rate, SMR)가 있다. 일반적으로 초당 음절 반복 횟수를 토대로 AMR과 SMR을 측정하고 보고 한다. 말장애 평가에 있어서 DDK 과제의 유용성으로 인해 국 내외적으로 많은 연구가 일반 아동과 성인을 대상으로 발달적 특성과 규준을 제시하고 있다. 아동을 대상으로 DDK의 수행력을 살펴본 대부분의 선행 연 구는 연령이 증가함에 따라 DDK 수행력이 증가함을 보고하고 있다(Baken & Orlikoff, 2000; Canning & Rose, 1974; Fletcher, 1972; Kim & Kim, 2016; Kim et al., 2018; Robbins & Klee, 1987; Sehr, 2013; Williams & Stackhouse, 2000; Yoo, 2018; Yoss & Darley, 1974). 대표적으로 국내 연구로 Yoo(2018)는 조 음 문제가 없는 만 4~6세 아동 총 248명을 대상으로 [퍼], [터], [커]를 사용하여 연령과 성별에 따른 AMR의 차이를 살펴보았 다. 그 결과 성별에 따른 AMR은 유의미한 차이가 없었지만, 학 령전기 아동에서 연령이 높아짐에 따라 AMR이 유의하게 증가 하는 것으로 나타났다. {"title":"Development and Usefulness of Hypernasality Control Device: A Preliminary Study","authors":"Yun-Kyung Song","doi":"10.21848/asr.200060","DOIUrl":"https://doi.org/10.21848/asr.200060","url":null,"abstract":"과다비성(hypernasality)은 비음을 제외한 자음 및 모음을 정상적인 방법으로 산출하는 동안 비강공명이 지나치게 많이 발생하는 것을 의미한다. 보통 이와 같은 증상은 연인두 부전 (velopharyngeal dysfunction)에 그 원인을 두고 있는데, 이는 비강으로 음파가 전달되지 않아야 할 때, 즉 비음이 아닌 소리 를 낼 때 비강 안으로 음파가 부적절하게 전달되는 것을 말한 다. 또한 비강누출(nasal emission)은 파열음, 파찰음, 마찰음과 같은 압력 자음을 산출할 때 연인두 부전으로 그 폐쇄가 적절 하게 이루어지지 않아 코로 기류가 새어나가는 현상이다. 이 새 어나가는 기류가 소리로 들리면 청취 가능한 비강누출(audible nasal emission)이라고 하며, 소리로 들리지 않으면 무음성 비 강누출(inaudible nasal emission)이라고 한다. 연인두 부전이 있는 대상자는 과다비성에 비강누출이 동반될 수도 있고 과다 비성만 있는 경우도 있다(Kummer, 2016). 일반적으로 중추신경계나 말초신경계의 질환이 연인두 부전 을 초래할 수 있으며, 특히 이완형과 경직형 마비말장애를 보이 는 대상에서 과다비성 및 비강누출의 문제는 빈번하게 말 명료 도에 부정적 영향을 미치고 있는 것으로 보고되고 있다(Freed 2014; Duffy, 2016). 또한 선천성 기형인 구개열(cleft palate)이 있는 경우, 1차 구개열 봉합 수술 후에도 흔히 연인두 부전을 나타낼 수 있다(Kummer, 2016). 한편 모든 말장애의 궁극적 치료 목표는 명료하고 자연스러 운 말이다. 의사소통장애 대상자의 최종 치료 목표가 말 명료 도의 향상이라고 할 수 있을 만큼 말 명료도는 아주 중요한 영 역이다. 말 명료도에 영향을 미치는 요인으로는 자음을 정확하 게 말하는 정도를 나타내는 자음정확도, 조음기관의 운동능력 을 나타내는 조음교대운동능력, 공명, 운율, 호흡, 발성 등이 있 다(Kim, 2002; Han, 2009). 이 요소들 중에서 공명문제는 행 동적 접근법인 언어치료 행위만으로 해결하기에는 한계가 있 다. 즉 연습을 통하여 기형이 있거나 약화 또는 마비된 근육이 정상적인 기능을 할 수 있게 하는 것은 어려움이 있고, 완전함 을 기대하는 것도 곤란하다. 이와 같은 이유로 구개열 아동은 1차 봉합수술 이후 연인두 부전이 있는 것으로 판단될 경우 보통 인두피판술(pharyngeal flap)과 같은 2차 수술을 하여 비강으로 소리가 새어나가는 것 을 조절하고자 한다(Yoo et al., 2000). 그러나 이 2차 수술은 만 4세는 되어야 가능해지기 때문에 초기 말소리 발달 문제가 Development and Usefulness of Hypernasality Control Device: A Preliminary Study","PeriodicalId":36841,"journal":{"name":"Audiology and Speech Research","volume":"4 1","pages":"373-382"},"PeriodicalIF":0.0,"publicationDate":"2020-10-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"85524285","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"Changes of the Korean Consonant-Vowel Syllabic Recognition Scores According to the Compression Threshold Control of Multi-Channel Hearing Aid","authors":"Dajung Yun, Kyoungwon Lee","doi":"10.21848/asr.200061","DOIUrl":"https://doi.org/10.21848/asr.200061","url":null,"abstract":"Purpose: While adjusting the compression threshold (CT) of low and high frequency bands of multi-channel hearing aids in noise, we tried to evaluate any changes in Korean consonant-vowel (CV) syllabic recognition scores, sound quality, and loudness of noise with white noise.Methods: A total of twenty one subjects with hearing loss (mean age of 66.2 years) participated voluntarily. Their pure-tone average was 46.0 dB HL and the word recognition score was 72.1%. The CTs of low/high frequency bands were adjusted to 50/50 dB SPL, 50/65 dB SPL, 65/50 dB SPL and 60/65 dB SPL in multi-channel hearing aids. While presenting white noise to the non-test ear, Korean CV syllables were presented to the test ear to evaluate the recognition scores, clarity of conversational speech, and loudness of noise. The intensity of CV syllables presented to the subject was 50 dB HL, and the signal-to-noise ratios (SNRs) were 5 dB and 10 dB.Results: First, when the CT of low/high frequency bands was adjusted to 50/50 dB SPL, the CV syllabic recognition scores were higher compared to 65/50 dB SPL. The first N100 component has been conceptualized as a pre-attentive response related to perception. The N100 response was modulated by the physical characteristics of stimuli such as type, intensity, and duration, and also depended on subject factors such as the internal state, neural activity, and cognition. As it was the first stimulus in a series, the N100 amplitude was relatively high but lowered for subsequent repetitions of stimuli (Brown et al., 2002; Clearwater et al., 2008). N200 has been known to be a comprehensive response of cortical, thalamic, and brainstem regions, although the source of N200 activation relied on some extent to the limbic and complex generating system in the region of the thalamus (Ponton et al., 2002). N200 was also thought to be sensitive to cognitive aspects such as task demands and attention (Näätänen & Picton, 1986). However, the latency of N200 exhibited conflicting findings depending on the age of participants (Amenedo & Díaz, 1998). As observed only during conscious stimulus attention, P300 was often found as a part of complex components of N200, which were categorized as endogenous potentials because even mere mental counting tasks for target stimuli could elicit them well (Kaga et al., 2014). P300 reflected the brain activity required to update working memory when changes occurred in the stimulus and variation of the amplitude indicated the degree of inclusion of the central nervous system activity involved in processing information (Verleger et al., 2014).
