Materials and Methods: This investigation’s design was a randomized controlled trial. One hundred and ten term pregnant women were recruited and allocated equally in the study and control groups. The study group (UCM) undertook umbilical cord milking toward the infant five times in 30 seconds after the umbilical cord was immediately clamped at a 20 cm, length from the newborn. The control group (ICC), by contrast, underwent immediate umbilical cord clamping without milking at 2 cm from umbilicus as the standard manner. Neonatal outcomes such as neonatal hematocrit (Hct) at 48 hours after birth, microbilirubin level, respiratory rate and length of stay in hospital were recorded. Results: Neither neonatal outcomes nor neonatal Hct at 48 hrs revealed statistical differences between the two groups. There were no serious maternal and fetal complications in either group. The two groups showed no significant differences in demographic and operative data. Conclusion: UCM at a point 20 cm from umbilicus had no effect in raising the Hct level at 48 hours following birth and no harmful effect in the term newborn delivered by cesarean section.","PeriodicalId":36742,"journal":{"name":"Thai Journal of Obstetrics and Gynaecology","volume":"25 1","pages":"35-43"},"PeriodicalIF":0.0,"publicationDate":"2017-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"49240915","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and Methods: A randomized clinical study was done in Women Health Hospital – Assiut University – Egypt. All women presented with symptoms suggestive of VVC to the clinic had been examined and approached for participation. Eligible participants were randomly assigned to one of three groups: Group (A) received lactobacillus containing vaginal capsules daily for one week postmenstrual. Group (B) received vaginal miconazole 400 mg once daily for 3days at bedtime postmenstrual. Group (C) received vaginal miconazole 400 mg once daily for 3 days at bedtime postmenstrual followed by lactobacillus containing vaginal capsules twice daily for one week. The primary outcome was to study the rate of recurrence of symptoms after 1, 3, 6 months of treatment. Results: During the study period, 202 participants with recurrent VVC were approached to participate in this study. No significant differences were found between the three study groups with regards patients’ age, residence, parity. After 1 month, symptoms of VVC recurred in 68.4%, 24.6% and 17.2% of women in group A, B, C consecutively. The recurrence rate increases after 3 months to become 87.5%, 60% and 33.3% consecutively in the three groups. Finally after 6 months, 94.4%, 88.7%, and 44.6% of women in group A, B, C consecutively suffered from RVVC. The recurrence rate is lower in group C (combination group) with statistically significant difference (p=0.0001). Conclusions: Vaginal administration of Lactobacillus Rhamnosus twice daily for 1 week after vaginal miconazole leads to vaginal colonization and associated at 6 months follow up with decrease the recurrence rate of VVC.","PeriodicalId":36742,"journal":{"name":"Thai Journal of Obstetrics and Gynaecology","volume":"25 1","pages":"62-68"},"PeriodicalIF":0.0,"publicationDate":"2017-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48027566","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and Methods : A retrospective cohort study was carried out in 210 patients who underwent laparoscopic adnexal surgery from August 2008 to November 2013 at the Songklanagarind Hospital. The patients were divided into three groups (n = 70 each) according to analgesic management. Group I received parecoxib 40 mg intravenously 2 hours preoperatively with postoperative paracetamol/NSAIDs around the clock, Group II received parecoxib 40 mg intravenously 15-30 minutes preoperatively with postoperative paracetamol/NSAIDs as needed, and Group III received only postoperative paracetamol/NSAIDs as needed. Morphine or fentanyl was used during operation and morphine was used as needed for severe postoperative pain in all cases. Patients in each group were matched by the operation in the same time period. The consumption of analgesic agents during surgery and 24 hours postoperation, pain scores, and adverse events were evaluated. Results: Intraoperative morphine consumption was not different among the 3 groups. However, in the 24 hours postoperation, 40% of patients in Group I received morphine (mean 1.1 mg) compared to 68.6% in Group II (mean 6.1 mg) and 80% in Group III (mean 9.6 mg) (p < 0.01). Group I received more postoperative paracetamol/NSAIDs than both Group II and Group III (p < 0.01). Group I had 88.5% morphine-sparing effect compared to Group III and 82% compared to Group II. The pain scores were similar between the groups. Conclusions: Preemptive parecoxib and postoperative paracetamol/NSAIDs provide a significant morphine-sparing effect in laparoscopic adnexal surgery.","PeriodicalId":36742,"journal":{"name":"Thai Journal of Obstetrics and Gynaecology","volume":"25 1","pages":"44-51"},"PeriodicalIF":0.0,"publicationDate":"2017-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"42747508","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Material and methods:          This comparative study was conducted from April 2014 until July 2015 at Rajavithi Hospital. Patients were recruited from women who had planned and undergone repeated cesarean delivery both scheduled and in emergency conditions. Women who enrolled in this study were having single pregnancy, without classical cesarean section, previous intra-abdominal surgery from other indication, and HIV infection backgrounds. Subjects were divided into two groups, transverse and vertical skin incisions. Data was collected from antenatal care records, anesthetic notes, operative notes and the research record forms. Results:          According to the inclusion and exclusion criteria, 500 subjects were selected. There were 286 and 214 subjects in the transverse and vertical skin incision groups, respectively. Incision-to-delivery interval in transverse was longer than in the vertical group by about 2 minutes (p < 0.001). The total operative time ± S.D. was 84.67±30.96 and 79.69±26.83 minutes in the transverse and vertical skin incision groups, respectively (p = 0.043). There was no statistical difference in internal organ injuries, estimated blood loss, post-operative outcomes, Apgar scores and fetal injuries. Conclusion:    In repeated cesarean delivery, transverse skin incision illustrated more incision-to-delivery interval and total operative time than vertical skin incision, but no difference in adverse maternal and neonatal outcomes.","PeriodicalId":36742,"journal":{"name":"Thai Journal of Obstetrics and Gynaecology","volume":"24 1","pages":"265-272"},"PeriodicalIF":0.0,"publicationDate":"2016-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"66680181","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and methods: Computer-based delivery records of Rajavithi Hospital were retrospective reviewed between January 2009 and December 2013. All data including maternal characteristics, neonatal outcomes, operative records and complications of cesarean delivery were collected. Prevalence of fetal laceration injury during cesarean delivery was calculated. Comparison was made between those with and without fetal laceration injury. Results: During the study period, cesarean deliveris were performed in 10,540 of total 29,532 deliveries (35.7%). Five hundred and fifty cesarean deliveries were met the exclusion criteria. The remaining 9,970 deliveries were analyzed. Thirty-two newborns (0.3%) had observed laceration injury from cesarean delivery. Common sites of laceration injuries were on faces and most of injuries were mild lacerations. Membranes rupture before undergoing cesarean delivery (OR=2.04, 95%CI=1.02 – 4.08) and young maternal age group (OR=3.93, 95%CI=1.58-9.78) are significant risk factors for fetal laceration injury associated with cesarean delivery. Experience of operator, urgency of cesarean delivery and neonatal outcomes show no related association with the injury. Conclusion: Fetal laceration injury associated with cesarean delivery is uncommon. Rupture of membranes before undergoing cesarean delivery and young maternal age group are major risk factors for the injury. Women with risk factors should be carefully operated during cesarean delivery.","PeriodicalId":36742,"journal":{"name":"Thai Journal of Obstetrics and Gynaecology","volume":"24 1","pages":"280-286"},"PeriodicalIF":0.0,"publicationDate":"2016-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"66680247","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and methods: Singleton pregnancies delivered at Chonburi Hospital with vertex presentation with MSAF during the 15 May 2011 – 15 May 2014 period were reviewed. These pregnancies included women who delivered at or beyond 24 weeks of gestation or birth weight at least 700 grams but excluded those who delivered infants with lethal congenital abnormalities. They were divided into 2 groups depending on whether the neonates developed MAS or not. The two groups were compared retrospectively regarding baseline characteristics, and then binary logistic regression analysis was used to identify independent significant factors associated with MAS. Results: Among 776 pregnancies who met the inclusion criteria, 111 neonates developed MAS, 665 neonates did not. Thick meconium, abnormal external fetal monitoring, non-vigorous newborn, Apgar score at 1 minute less than or equal to 7, and low birth weight were significantly associated with MAS (p<0.05). Three quarters of patients with ovarian cancer are diagnosed after the disease has progressed to advanced stage. Thus, improvement of an effective screening tool could result in better prognosis and survival. To date, cancer antigen 125 (CA-125) is the most commonly used serum biomarker for detecting ovarian cancer. However, the test has low sensitivity for the disease detection (50-62% for early stage epithelial ovarian cancer) and limited specificity. Therefore, CA-125 as a single test is not recommended for ovarian cancer screening in asymptomatic women. CA-125 in combination with human epididymis protein 4 (HE4) and Risk of Ovarian Malignancy Algorithm (ROMA) is becoming widely used in clinical practice for the evaluation of adnexal masses. Clinical examination, transvaginal ultrasonography (TVS) and serum biomarkers (ROMA) more effectively classify the patients into low and high risk of ovarian malignancy. The classification allows to direct the patients to appropriate management by gynecologic oncologists, and thus the survival rate could be improved. Role of serum biomarkers as the screening and diagnosis of ovarian cancer is described in this review.","PeriodicalId":36742,"journal":{"name":"Thai Journal of Obstetrics and Gynaecology","volume":"24 1","pages":"226-230"},"PeriodicalIF":0.0,"publicationDate":"2016-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"66679363","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
