Rodrigues","doi":"10.25796/BDD.V2I3.21333","DOIUrl":"https://doi.org/10.25796/BDD.V2I3.21333","url":null,"abstract":"Infection-related complications in patients on peritoneal dialysis (PD) is a leading complication. \u0000Our aim was to evaluate the type and natural course of ESI events in a cohort of PD treated in last decade of our PD program. \u0000Registry data of ESI events (n=126, in 74 patients) were retrieved. ESI protocols followed standard international guidelines. A systematic quality control is performed. \u0000               The median follow-up was 29.1 (14.0-47.4) months. In this population the adverse outcomes of TI rate and peritonitis rate was 0.12 and 0.13 patient/year, respectively.                Male sex (0.048), older age (0.007) and Staphylococcus aureus (SA) agent (0.006) were predictive of TI while non-optional PD and lower levels of albumin were predictive of peritonitis.                 After grouping the ESI events according to the date of the occurrence of infection (group 1: 2008 to 2012, group 2: 2013 to 2017 and group 3: 2018) a substantial increase of TI in 2018 was evident (P <0.001 when comparing group 3 vs 1 and 0.005 when comparing group 2 and 3).               When ESI occurs simultaneous with TI, the probability of not reaching cure is 65%. Drop-out occurred in 50% of ESI without peritonitis vs 86% with peritonitis (P <0.001). SA is the microorganism most implicated in the failure to heal (P 0.002) and drop-out (P 0.010). \u0000In spite of a number of efforts to reduce ESI, a regular audit still point to the need for protocols review in order to avoid adverse outcomes. Difficulties in relation to the isolation of the teams, the size of the centers, the number of participants, the lack of knowledge in the technique are undeniably a hindrance to the development of home treatment. \u0000This mailing list is exclusively reserved for home dialysis nurses after registration by the RDPLF. They need an email address, if possible, personal. Nephrologists and nurses at the material supply commercial providers may have access to alloDP, upon request, but only as «readers». \u0000To break her isolation, or sense of isolation, the nurse simply sends her question, request, or need to the nursing community to the address allodp@rdplf.org. For a long time reserved for frail subjects in whom hemodialysis appeared more dangerous, it is considered nowadays as a technique equivalent to hemodialysis, provided its indications and contraindications are respected and associated with the respect of the patient’s free choice after objective information. In particular, there is now a consensus to recognize its interest in patients waiting for transplantation, usually in order to preserve vascular access. Over the past decades French teams have played their role nationally and internationally, very often in partnership with their European neighbors, to improve the results achieved and to improve the different modalities. Rostoker","doi":"10.25796/bdd.v2i3.20983","DOIUrl":"https://doi.org/10.25796/bdd.v2i3.20983","url":null,"abstract":"Background: Anemia is commonly observed in patients with chronic kidney disease (CKD) as soon as the glomerular filtration rate falls below than 30 ml/min. CKD patients frequently have iron deficiency. The use of both erythropoiesis-stimulating agents (ESA) and iron therapy is the backbone of anemia management in CKD. For this reason, an adequate iron supply is mandatory to achieve the optimal therapeutic benefit of erythropoiesis stimulating agents (ESAs). Many groups agree that anemia in peritoneal dialysis (PD) patients is less severe than in hemodialysis (HD) patients and that there are important differences in treatment practices for anemia between PD and HD patients. \u0000Methods: Analysis of the Anemia module of the French Language Peritoneal Dialysis Registry (RDPLF) register from the database set up in 2005 with a study of the period 2010-2017. \u0000Results: Data from 568 patients who participated in the Anemia module were analysed during the 2010-2017 follow-up period. Their median age were 71 years, 42% were female, median dialysis vintage was 13 months, 40,5% of patients had diabetes mellitus, 74% of patients were treated with ESA, 23% were on oral iron and only 11% have received intravenous iron. In terms of biological assessment, the average hemoglobin level was close to 12 g/dl and median CRP was close to 5 mg/l. For the iron balance, ferritin reached an average level of 270 µg/l in 2013 and stabilized in 2017 at 200 µg/l. Verger","doi":"10.25796/BDD.V2I3.21383","DOIUrl":"https://doi.org/10.25796/BDD.V2I3.21383","url":null,"abstract":"Peritoneal catheter infections : data from the French language peritoneal dialysis registry (RDPLF), risk factors \u0000The French Language Peritoneal Dialysis Registry (RDPLF) record since 1997 all data dealing with peritoneal catheter insertion and follow up. \u0000The aim of this study is to analyze catheter infections on 10801 catheters in 144 centres from January 1, 1997 and December 31, 2018. \u0000Infections are more common in the first month and 50% occur before the tenth month. The risk of infection increases in case of wall hematoma, initial fluid leakage, and obesity. It decreases with the use of prophylactic antibiotics at the time of catheter insertion, with experienced operator, if the first dressing is delayed for 7 days, and with mupirocin as exit-site prophylaxis. \u0000During last two decades the percentage of Staphylococcus aureus infections has decreased, whereas the proportion of Pseudomonas, Corynebacterium and other Gram + cocci increased. \u0000The incidence of catheter infections is low compared to literature data: it decreased to 0.16 episodes per year for the 2013-2017 period. \u0000Adherence to ISPD guidelines: preoperative antibioprophylaxis is the most followed guideline (70.7% of catheter implantations in 2018). Local antibioprophylaxis concerns only 15.6% of catheters, and remains concentrated in a few centres ; mupirocin is the most frequently used agent. Toute évaluation ou étude à partir de données biologiques ou cliniques nécessite le recours à des calculs statistiques. Si les modèles complexes relèvent du spécialiste, nombre de calculs de base, tels que moyenne, médiane, etc.. sont à la portée de tous moyennant un minimum effort. Il existe un logiciel gratuit, «R», extrèmement puissant, qu’il est facile d’installer sur tout ordinateur et qui permet les calculs les plus simples comme les plus compliqués en fonction de ses compétences. \u0000Nous avons pensé que les lecteurs du BDD, infirmiers et infirmières comme médecins seraient intéressés à suivre une formation de base qui leur donnera l’autonomie pour des études simples ou compliquées qu’ils ou elles souhaiteraient mener. Le RDPLF se tient par ailleurs à leur disposition pour leur adresser tout fichier d’export anonymisé. \u0000Nous débutons avec ce numéro une série d’articles rédigée par Claire Della Vedova que nous remercions vivement pour l’aide apportée. \u0000Claire est Ingénieure en biostatistique / data analyste, Elle utilise quotidiennement le logiciel R pour analyser des données. Elle a travaillé pendant plus de 15 ans dans les domaines de l’environnement et de la santé, et a formé de nombreux étudiants et chercheurs à l’utilisation de R. En dehors des réseaux sociaux les principaux supports formels sont la présentation orale lors d’un congrès ou la rédaction d’un article dans une revue. Les deux modes de communication sont soumis à l’accord d’un comité de lecture qui décide de l’opportunité d’accepter le travail. Afin d’aider nos jeunes lecteurs et lectrices, nous rappelons dans ce numéro les règles de soumission d’un résumé pour un congrès.
