Pour cela tout doit être mis en place pour que les patients puissent avoir le choix, aidés par leur équipe soignante. La gestion de l’accessibilité à un traitement d’eau à domicile reste encore de nos jours difficile malgré les progrès notables lors des dernières décennies. Le développement de l’HDQD sans avoir à installer un traitement d’eau au domicile des patients est un progrès notable qui a donné un élan nécessaire au développement du traitement du patient au domicile. It is a comparative analysis of different diagnostic, therapeutic or preventive strategies, based on their costs and health outcomes. We have only one medico-economic study on the costs of dialysis in France, carried out by the Haute Autorité de Santé, which covered the year 2012 and consisted of cross-referencing 3 databases: the Registre Réseau Épidémiologique et Information en Néphrologie and the Système National d’Informations Inter-régimes de l’Assurance Maladie et de l’Hospitalisation. Analyses focused on prevalent patients, incident patients and key patient characteristics: age, presence of diabetes and care trajectories. The average monthly cost varied considerably according to the management modality, from 3774 euros/month in autonomous continuous ambulatory peritoneal dialysis (CAPD) to 7253 euros/month in center-based hemodialysis. The monthly cost of therapies according to the patient profile appears to be strongly affected by the diabetic status of the patients. This analysis shows the high cost of transport for patients on in-center hemodialysis (20% of total expenditures), and even higher for nursing care in assisted CAPD (37% of total expenditures). The total tariff decreases from 2014 to 2020 were -17.6% for center-based hemodialysis and -9.8% for the low medicalized dialysis unit (LMDU), while the total 2014–2021 tariff increases were +9.3% for CAPD and +9.1% for automated peritoneal dialysis (APD). Paradoxically, these price changes have increased the cost of assisted peritoneal dialysis, which is now almost at the same level of overall cost as in-center hemodialysis for the French health insurance system. Monthly or quarterly PD peritonitis rate statistics are used to identify special cause variation within or between individual PD centres, to highlight any need for quality improvement. Unfortunately, many PD centres do not accurately “patient flow” (i.e., when patients start and finish on PD), and therefore cannot measure PD peritonitis rate. In this study, we validate an estimating formula for month-on-month annualised PD peritonitis rate, that calculates time-at-risk from “patient stock” (i.e., the number of prevalent patients on PD at the beginning and end of the month). We compared centers’ estimated peritonitis rates with gold-standard measurements in the Australia and New Zealand Dialysis and Transplant Registry / New Zealand PD Registry, and Le Registre de Dialyse Péritonéale de Langue Française et hémodialyse à domicile. A total of 268 centers from 9 countries with 1,020,260 patient-months of follow-up and 19,669 episodes of peritonitis were modeled. Overall agreement was excellent between estimates and gold-standard measurements with a concordance correlation coefficient (CCC) of 0.998 (95% confidence interval [CI] 0.998-0.998) in both registries. There was statistically significant lower agreement for smaller centers, although the CCC was still greater than 0.995. There were no instances of clinically significant misclassification of centers as being compliant or non-compliant with PD peritonitis standards with the use of the estimating formula. It is only in some parts of the world, such as Hong Kong, Mexico, Thailand, the Netherlands, Finland, Denmark, Iceland, Australia, Canada and New Zealand, that home dialysis is used for more than 20% of the dialysis population. It is observed that in most high- and middle-income countries, home dialysis is generally more economical than center-based dialysis. \u0000According to the results of the REIN 2020 registry, the share of out-of-center dialysis in France has been increasing since 2012. This is mainly due to the increase in the number of patients treated in low medicalized dialysis units (LMDUs) (+7.3% per year between 2012 and 2016, then +3.8% between 2016 and 2020). In contrast, the proportion of patients treated at home has changed little. The percentage of patients on peritoneal dialysis is decreasing (-1.8% per year). Home hemodiaysis is increasing (+10.3% per year) but remains very marginal (1% of dialysis patients). Finally, the advanced age of dialysis patients, which is constantly increasing, cannot be ignored. The proportion of these very old patients has increased from 10.5% in 2012 to 12.5% in 2020. The testimony we report is the travel story of Mr Alhassane DIALLO, a dialysis patient who wanted to join his family in Guinea at a great distance from any dialysis clinic, a journey that home dialysis allowed, but not without hardships. We hope that this experience can help other patients with independent travel plans. The success of this project was due to the unwavering determination of Mr DIALLO but also to a collective organization that requested the administrative, logistical and pharmacy services of the AUB Santé Foundation and close cooperation between the French and Guinean medical teams. This experience shows us how important travel limitations are in the experience of dialysis patients, and particularly in that of expatriate patients. The kidney is one of the most affected organs, often requiring dialysis. Our study examines the outcomes of a cohort of patients with CED treated with peritoneal dialysis (PD). As anticoagulants can favor emboli, peritoneal dialysis may theoretically have an advantage. \u0000From the database of the French Language Peritoneal Dialysis Registry (RDPLF), between January 1, 1995, and December 31, 2021, we selected patients treated with PD for >90 days and with an age >18 years. On the basis of the variables of patient autonomy, diabetes, BMI, modified Charlson index, age, sex, pre-PD treatment, causes of death, causes of transfer to hemodialysis, and peritonitis, three types of survival were estimated (patient, technical, and composite). After adjustment by propensity scores and taking into account competitive risks, 2 groups of patients were constituted on the basis of baseline nephropathy: emboli group vs. control group. \u0000Patient survival and strict technical survival are not associated with the type of nephropathy (CED versus others). Composite technical survival (uncensored for deaths and transfers to hemodialysis) was only associated with cholesterol emboli nephropathy in the multivariate analysis adjusted for diabetes, autonomy, and age of the patient at the start of the dialysis treatment (p=0.011; 95% CI [0.736 [0.581-0.931]]). \u0000Our study from the RDPLF database shows no difference in technical and patient survival in a cohort of patients with CED vs. a control group.
